A rare case of an osteochondral lesion of the tarsal navicular with a subacute stress fracture in a high level athlete

Abstract Background Scaphoid stress fractures are rare and typically present in young, elite male athletes. Due to the infrequency in which these injuries are encountered, the optimum management is not established. Case Description We present the case of a 20-year-old male gymnast with bilateral stress fractures of the scaphoid waist. Following conservative treatment, clinical and radiological signs of union were seen bilaterally. Eight months after return to normal activities, a unilateral recurrence of the stress fracture occurred. This was successfully treated with internal fixation and bone grafting. Literature Review Scaphoid stress fractures are most frequently seen in gymnasts but also occur in participants of other sports involving repetitive loading of an extended wrist. This action conveys force predominantly through the waist of the scaphoid. Most case studies have reported high union rates, whether treated operatively or conservatively, and the patients returned to high-level sport with no further problems. Stress fracture recurrence has been reported in other bones such as the metatarsal and tibia but never before in the scaphoid. Our case is unusual in that the patient suffered ongoing problems due to a recurrence of the scaphoid stress fracture after returning to normal activities. Clinical Relevance Clinicians should be aware that scaphoid stress fracture recurrence can occur, counsel patients accordingly, and remain vigilant after apparent union. We would recommend early fixation to allow a quicker return to function and prevention of recurrence.

Type IV diaphragmatic hernia is a rare case which has various clinical manifestations. The clinical manifestations as a result of protruded organ towards thoracic cavity in diaphragmatic hernia are varied from gastrointestinal symptoms such as epigastric pain, dysphagia and acid reflux. However, in a rare occasion, dyspnea may present as a result of compression atelectasis due to excessive increased in intrathoracic pressure. Here, we report a 57-year-old male patient who had spontaneous diaphragmatic hernia admitted with dyspnea as a chief complaint. This report might add another perspective to the physicians in facing the patient with atelectasis as the result of a high level of lung compression secondary to type IV diaphragmatic hernia.

Background and Importance: Sacrococcygeal Teratoma (SCT) with medular invasion is rare. Case Presentation: We report a case of an 11-month male infant, with no prenatal history of any abnormality, presenting since birth, a mass in the buttock extended to retrorectum associated with a right side hypotonic limb and monoplegia. Medullar and abdominal pelvic Magnetic Resonance Imaging (MRI) showed a sacrococcygeal tissue mass and intradural lumbosacral invasion with a high level of alpha-fetoprotein. Surgical excision was done successfully with an uneventful follow-up. Conclusion: Despite of the intradural invasion, those SCT are in major cases mature and present low risk of malignancy or recurrence.

ABSTRACT Introduction Pseudocyst formation is a well known complication of pancreatitis. Intrahepatic pancreatic pseudocyst is very rare event and only about 30 cases have been reported in literature. We report here a case of 32-year-old male who was previously diagnosed as a case of hepatic abscess. He was referred to our department for ultrasonography (USG) and contrast enhanced computed tomography (CECT) abdomen with complaint of recurrent pain in upper abdomen. On the basis of findings of CECT Abdomen, diagnosis of large intrahepatic pancreatic pseudocyst in left lobe of liver is made. Conclusion Intrahepatic pseudocyst should be considered a differential diagnosis of cystic hepatic lesions in the patients with chronic or recent episode of acute pancreatitis. Dedifferentiated liposarcomas most commonly arise in the retroperitoneum, accounting for 10% of liposarcomas. Heterologous differentiation occurs in 5-10% of dedifferentiated liposarcomas; however, divergent osteosarcomatous differentiation is rare. We report a rare case of initial presentation of dedifferentiated liposarcoma with osteosarcomatous component as a colonic mass in a 72-year-old man. The tumor is mainly composed of bony trabeculae with intervening highly atypical cells and adjacent high-grade mesenchymal nonlipogenic tumor, as well as areas of well-differentiated liposarcoma. Immunohistochemical studies showed diffuse positivity for SATB2 in the atypical cells and fluorescence in situ hybridization revealed high-level amplification of MDM2 gene, supporting the diagnosis of well-differentiated and dedifferentiated liposarcoma with heterologous osteosarcomatous differentiation.

Abstract Background Talus osteochondral lesion is commonly associated with trauma, avascular necrosis or even genetic factors, but gouty tophus as a cause of Hepple stage V type talus osteochondral lesion is rare. Case presentation Here, we report a case of an 18-year-old man who complained of left medial deep ankle pain on ambulation. This young man had an extreme liking of sea food rich in purines and also sugar-sweetened drinks. He was diagnosed with a Hepple stage V type talus osteochondral lesion and was treated with medial malleolus osteotomy and an osteochondral graft. The talus osteochondral lesion was found to be a gouty tophus and was completely removed. Hypouricemic therapy was prescribed for 2 months, which allowed the patient to walk with a visual analogue score (VAS) score of 1. He was followed up for 12 months. Conclusions Young people with an extreme liking of sea food rich in purines and also sugar-sweetened drinks may be at a risk of developing gout. Acute onset of ankle atraumatic pain, swelling with a high level of serum uric acid and a talus osteochondral lesion with cyst formation should make physicians consider a diagnosis of gout.
