Approximately 40% of ischemic strokes have no clearly definable etiology and are termed cryptogenic strokes. Patent foramen ovale, a small communication between the left and right atria, is considered to be a risk factor for cerebral embolism. In this study, we report the case of a 29-year-old woman with diagnosis of ischemic stroke due to patent foramen ovale who has undergone percutaneous endovascular closure. The aim of this report is to discuss the relevant aspects of the patent foramen ovale and the cryptogenic stroke, its clinical presentation, diagnosis, management and recurrence.

Abstract Introduction: Cerebrovascular disease in childhood is relatively rare. Recently, there has been much interest in homocysteine, a sulfur-containing amino acid, as an important risk factor for vascular diseases including stroke. We report the case of a 13-year-old girl presented with ischemic stroke due to hyperhomocysteinemia. Case report: This case report describes a 13-year-old girl with repeated episodes of hemiplegia due to hyperhomocysteinemia. Conclusion: There are strong clues in favor of a causal relationship between hyperhomocysteinemia and athero-trombotic diseases. Homocysteine levels can be lowered effectively by a combination therapy of folic acid, vitamin B12, and vitamin B6.

Summary Cardiac surgery has been shown to be associated with increased risk of acute ischemic stroke. This report presents a case of a successful mechanical embolectomy procedure to treat a patient for an acute ischemic stroke, which was caused by the cardiac embolization of a ‘foreign body’ containing debris following a redo sternotomy procedure for mitral valve replacement and tricuspid valve annuloplasty.
