Abstract Objective By investigating the large-scale HIV antibody screening of more than 1 million people, we explored weather the strategy of integrated universal HIV testing in a public health care program was useful. Methods We used a multi-stage stratified random cluster sampling method in a community-based investigation of 30 sample points within 9 counties in the Zhejiang province. The HIV antibody was detected, and demographic information was collected. Results Of the 1 113 030 people screened for HIV, 310 tested positive (adjusted HIV prevalence, 3.45/10000; 95% confidence interval [CI], 3.41–3.48). The HIV prevalence was higher in men(5.62/10000 ) than in women(1.17/10000) of all ages; those in the 25–34 and 35–44 age groups were highest (compared with the < 15 age group, the adjusted odds ratios were 25.69 and 18.48, respectively). The HIV prevalence at the medium gross domestic product (GDP) level (adjusted HIV prevalence, 5.28/10000; 95% CI, 4.53–6.04) was significantly higher than those at high and low GDP levels Especially in the male 25–34 and 35–44 age groups. Compared with the native HIV positive population, the migrants were younger, did not have a stable sexual partner, and had a lower level of education. Conclusion By using universal HIV testing integrated into a public health care program was feasible and (perhaps) effective in finding new HIV cases. We should pay more attention to the 25–44 age male population, as well as migrants in our HIV/AIDS control strategies, especial in industrial activity district.

At the time of its passage, the Medicare Reimbursement Act of 1988 was hailed as a major improvement in health care funding for the elderly. However, within a year the Act had been repealed largely through the efforts of the elderly citizens in was designed to benefit. This paper examines the problems associated with the funding methods used in this program. It suggests that interaction of the tax surcharge contained in it with other tax provision affecting the elderly made the effects of the law especially objectionable and that future programs for funding national health care program must avoid these effects if they are to succeed.

RESUMOObjetivo: conhecer o perfil de pessoas com feridas crônicas atendidas por um Programa de Atenção à Saúde em uma Operadora de Saúde Suplementar. Método: estudo quantitativo, retrospectivo, descritivo e documental realizado em 67 prontuários de pessoas portadoras de feridas atendidas por uma Operadora de Saúde Suplementar da Bahia. Resultados: perfil feminino; idade entre 60 a 69 anos; ensino médio completo (72%); faixa salarial de 3-4 salários mínimos; 28% dos portadores de feridas crônicas com diagnóstico de pé diabético. Conclusão: a prevalência de portadores de feridas atendidos pelas Operadoras é elevada principalmente entre mulheres e com faixa etária avançada. Este estudo tem relevância por apresentar o perfil dos portadores de feridas atendidos por um Programa de Saúde pertencente a uma Operadora de Saúde ainda carente de políticas públicas e privadas eficazes. Purpose The correlation between health care expenditure, gross domestic product (GDP) and population over 65 years (understood as share of the elderly) is a key question for health economics and demographic impact. The purpose of this paper is to study the role of ageing society to curb rising health care expenditures along the Spanish regions over the period 2002-2013, identifying their geographic differences and explain them based on GDP differences. Design/methodology/approach Cointegration technique is used in order to test if there is a statistically significant connection between variables. Findings They are similar to some obtained when using unit root test. In particular, the authors find how the elderly positively affects health care expenditure per capita. Practical implications The findings suggest that any cooperation policies should aim at improving the access of people to health care services based on public health care expenditures. Originality/value To the best of the knowledge this is one of the first studies which suggest different results by Spanish regions due to mature decentralized system in recent years.
