Abstract Psychosis after traumatic brain injury (TBI) occurs in up to 10% of cases. Although guideline consensus is lacking regarding drugs of choice for this condition, current literature points to the use of atypical antipsychotics. This case describes a 58-year-old male with major neurocognitive disorder due to TBI with behavioral disturbance that was successfully treated with paliperidone palmitate. In addition to the off-label use of paliperidone, this case also explores the use of forced medication as the initial injection was given per guardian consent. After completion of a literature review, this appears to be the first case report describing the use of a long-acting antipsychotic for the treatment of TBI-related psychosis. This case suggests that paliperidone palmitate may be efficacious for psychosis following TBI; however, further study is warranted.

Abstract  Paraneoplastic neurological syndromes (PNS) can manifest with every type of malignancy. A well-known syndrome is myasthenia gravis (MG) in combination with thymomas. No association between primary brain tumors and neuromuscular disorders has been described. Here, we present a case of a 65-year-old patient who developed MG, following an uncomplicated, gross-total resection of a glioblastoma. To our knowledge, this is the first case describing the onset of MG during the early postoperative phase after glioblastoma resection. Current criteria of PNS are insufficient when the neurological syndrome is diagnosed at the time of a malignancy or shortly thereafter and should be revisited.

Il moltiplicarsi dell'offerta di opportunità di gioco d'azzardo anche in luoghi a bassa soglia di accesso e non dedicati in maniera esclusiva (bar, centri commerciali, tabaccherie), ha rinnovato l'interesse degli esperti riguardo ai costi sociali, economici e psicologici associati a tale pratica.Il comportamento del gioco d'azzardo può condurre ad una dipendenza, riconosciuta e classificata dal DSM5 e può comportare la compromissione del paziente dal punto di vista psichico, familiare e sociale. Questa patologia colpisce una percentuale variabile dall'1% al 3% della popolazione generale. Pertanto, in Lombardia, il numero di persone affette da questo problema potrebbe variare fra 100.000 e 300.000.Nel presente case report viene discusso il caso di un giovane con scompenso psicotico di tipo schizoaffettivo e GAP, che si rivolge al SerD.
