Role of the obestatin/GPR39 in the synthesis and secretion of pancreatic enzymes

In dogs prepared with either pancreatic duct ligation (PDL) or chronic pancreatic fistula (PF), we investigated the effect of pancreatic enzymes on release of cholecystokinin (CCK) in response to corn oil in the upper small intestine. In control dogs without PDL, intraduodenal administration of corn oil (Lipomul containing 15 mmol of triglyceride) resulted in a marked and sharp increase in plasma CCK concentration during the 1st h, which then decreased during the following 1.5 h. In PDL dogs, on the other hand, no immediate increase occurred during the 1st 45 min after which a modest but gradual increase occurred in plasma CCK concentration during the following 2 h. Likewise, plasma CCK concentration increased significantly when Lipomul predigested with pancreatic enzymes or oleic acid was administered to PDL dogs. The increase was comparable to the response to Lipomul alone in control dogs. The infusion of Lipomul resulted in similar responses of plasma CCK in PF dogs as were found in PDL dogs when pancreatic juice was diverted to the exterior from the duodenum. The increases in plasma CCK paralleled the pancreatic protein secretion, and a significant correlation was found between plasma CCK concentration and the protein output. The present observations indicate that release of endogenous CCK by fat from the upper small intestine depends on an adequate digestion of neutral fat by pancreatic enzymes. Thus pancreatic enzymes play an important role on release of CCK in response to a neutral fat.

1. Two surgical techniques were developed in the calf to study the role of pancreatic secretion in digestion. The secretion was collected through a cannula placed in a small sac of duodenum into which the pancreatic duct drained. The continuity of the duodenum was re-established in the first technique by duodenal re-entrant cannulas, and in the second technique by end-to-end anastomosis of the duodenum with a cannula placed cranial to the anastomosis to return the pancreatic secretion. The accessory pancreatic duct was ligated.2. The flows of digesta through the duodenum of milk-fed calves were 5505, 6369 and 7709 ml/12 h at 7, 24 and 63 d of age respectively, similar to values reported previously in the literature. In a 12 h collection period 297, 441 and 602 ml pancreatic fluid were secreted by calves of 7, 24 and 63 d of age respectively. The secretion from the mucosa of the duodenal sac was 40 ml/12 h in two other calves.3. The rate of secretion from the pancreas varied markedly in milk-fed calves, being lowest 2–3 h and highest 6–10 h after feeding. Changes in the concentration of chloride and bicarbonate with pancreatic secretion rate were indicative of a secretin stimulus to secretion.4. The rates of inactivation of pancreatic enzymes collected from the duodenal sac were measured at 4°, 20° and 39°.

Sepsis is the occurrence of infection in association with the systemic inflammatory response syndrome (SIRS). Bacterial sepsis is one of the major illnesses that shows severe symptoms which leads to organ dysfunction. Procalcitonin (PCT) the precursor of the hormone calcitonin shows a huge impact on the diagnosis of sepsis caused by the bacterial infection. Many studies have also insisted the importance of PCT as a biomarker for sepsis. As early diagnosis is the key factor to overcome the difficulties of the sepsis, PCT acts a prominent diagnostic tool to acquire the desired results. This study focused on the role of PCT in correlation with supporting parameters of the vital organs such as renal markers, liver enzymes and pancreatic enzymes to reduce the organ dysfunction and its complications. The role of fibre intake in the management of patients with pancreatic disease is still controversial. In acute pancreatitis, a prebiotic enriched diet is associated with low rates of pancreatic necrosis infection, hospital stay, systemic inflammatory response syndrome and multiorgan failure. This protective effect seems to be connected with the ability of fibre to stabilise the disturbed intestinal barrier homeostasis and to reduce the infection rate. On the other hand, in patients with exocrine pancreatic insufficiency, a high content fibre diet is associated with an increased wet fecal weight and fecal fat excretion because of the fibre inhibition of pancreatic enzymes. The mechanism by which dietary fibre reduces the pancreatic enzyme activity is still not clear. It seems likely that pancreatic enzymes are absorbed on the fibre surface or entrapped in pectin, a gel-like substance, and are likely inactivated by anti-nutrient compounds present in some foods. The aim of the present review is to highlight the current knowledge on the role of fibre in the nutritional management of patients with pancreatic disorders.
