Effects of an Aquatic-Based Exercise Program to Improve Cardiometabolic Profile, Quality of Life, and Physical Activity Levels in Men With Type 2 Diabetes Mellitus

Background: Physical inactivity and Type 2 diabetes mellitus (T2DM)–associated inflammatory biomarkers are correlated with poor quality of life (QoL). However, no study has investigated the synergistic effect of physical activity (PA) and lower neutrophil–lymphocyte ratio (NLR) on QoL. Objective: We examined the independent and synergistic effects of PA and inflammatory biomarkers on three domains of QoL in T2DM. Methods: This cross-sectional study included 294 patients with T2DM from community clinics in Indonesia. The 36-item Short Form Survey and a questionnaire about PA engagement were used to measure QoL and metabolic equivalent of task (MET)-hr/week, respectively. Inflammatory biomarkers were measured in fasting blood. Adjusted coefficients β and 95% confidence interval (CI) were estimated using multiple linear regression. The synergistic effect was analyzed using additive interaction for linear regression. Results: Patients with PA ≥ 7.5 MET-hr/week exhibited significantly higher total QoL (β = 8.41, 95% CI = [6.04, 10.78]) and physical component score (PCS; β = 13.90, 95% CI = [10.52, 17.29]) than those with PA < 7.5 MET-hr/week. Patients with NLR < 1.940 had significantly higher total QoL (β = 4.76, 95% CI = [3.41, 6.11]), mental component score (MCS; β = 2.62, 95% CI = [0.75, 4.49]), and PCS (β = 6.89, 95% CI = [4.97, 8.82]) than patients with NLR ≥ 1.940. PA ≥ 7.5 MET-hr/week and NLR < 1.940 exhibited a synergistic effect on total QoL, MCS, and PCS. Conclusions: High PA level and low NLR had a positive synergistic effect on QoL among patients with T2DM.

ABSTRACT                Diabetes mellitus is a non-communicable disease whose prevalence continues to increase in the world, both in developed and developing countries. Physical activity is also a major risk factor in triggering Diabetes Mellitus. This study aims to determine the relationship of physical activity with the quality of life of elderly people with type 2 diabetes mellitus. The research method used is descriptive analytic with cross sectional design. The sampling technique in this research is purposive sampling with 58 samples. The instrument used in this study was a GPAQ physical activity questionnaire and quality of life using a WHOQOL questionnaire. The statistical test used is the chi square test. The results showed the majority with low activity of 23 elderly (38.6%) and the quality of life of the majority of good elderly totaling 30 elderly (51.7%) obtained p value of 0.001 <0.05 then H0 is rejected and Ha is accepted and can be conclusions that there is a significant relationship between physical activity with the quality of life of elderly people with diabetes mellitus type II. As health workers are expected to provide education to the community, especially in the elderly about physical activity one of them is elderly exercise, because of good physical activity and regularly provide benefits for the body, especially for people with Diabetes Mellitus type 2.   Keywords: Physical Activity; Quality of life; Elderly; Type 2 Diabetes Mellitus ABSTRAK Diabetes mellitus merupakan salah satu penyakit tidak menular yang prevalensinya terus mengalami peningkatan didunia, baik dinegara maju maupun negara berkembang. Aktivitas fisik merupakan faktor resiko mayor dalam memicu terjadinya Diabetes Mellitus. Tujuan penelitian adalah untuk mengetahui hubungan aktivitas fisik dengan kualitas hidup lansia penderita diabetes mellitus tipe 2. Metode penelitan yang digunakan yaitu Deskriptif Analitik dengan rangcangan cross sectional. Teknik pengambilan sampel menggunakan purposive sampling sebanyak 58 sampel. Instrumen yang digunakan adalah koesioner aktivitas fisik GPAQ dan kualitas hidup menggunakan koesioner WHOQOL. Uji statistik yang digunakan ialah chi square test. Hasil penelitian didapatkan mayoritas dengan aktivitas rendah sebanyak 23 lansia (38,6%) dan kualitas hidup lansia mayoritas  baik berjumlah 30 lansia (51,7%) didapatkan nilai p value 0,001 < 0,05 maka H0 di tolak dan Ha diterima. Kesimpulannya adalah terdapat  hubungan bermakna antara aktivitas fisik dengan kualitas hidup lansia penderita diabetes mellitus tipe 2 Sebagai tenaga kesehatan diharapkan dapat memberikan edukasi kepada masyarakat khususnya pada lansia mengenai aktivitas fisik salah satunya adalah senam lansia, karena aktivitas fisik yang baik dan teratur memberikan manfaat bagi tubuh, khususnya bagi penderita Diabetes Mellitus tipe 2. Osteoarthritis is the most common condition to co-occur with other chronic health conditions and a broad exercise program on management of chronic conditions may be suitable for this group. This study evaluated the 12-week YMCA Move for Health exercise program among adults with osteoarthritis or with/at risk of chronic health conditions using a mixed-methods study design based on the reach, effectiveness, adoption, implementation, and maintenance (RE-AIM) framework. Participants (n = 66) completed the exercise program at the YMCAs in Cambridge, Kitchener, and Waterloo. Assessments included physical function, health-related quality of life, symptoms of arthritis, and physical activity levels and were conducted at baseline (B), postprogram (PP), and 3-month postprogram. Due to interruption by COVID-19, a subgroup of participants completed the 3-month postprogram assessments after the onset of the pandemic. At PP, participants with OA showed significant improvements in level of disability (B = 0.63 ± 0.45 and PP = 0.55 ± 0.47; p = .049), pain (B = 4.3 ± 2.5 and PP = 3.6 ± 2.4; p = .026), fatigue (B = 3.9 ± 3.1 and PP = 2.8 ± 2.6; p = .003), and several domains related to health-related quality of life. Despite interruption by the COVID-19 pandemic and poor maintenance of physical activity levels, nearly all improvements related to level of disability, symptoms of arthritis, and health-related quality of life observed at PP were maintained 3-months postprogram. The Move for Health program proved to be a feasible and effective community program for people with osteoarthritis. Additional supports may be needed to maintain physical activity levels after the program.

Background:Physical activity (PA) is important in managing Type 2 Diabetes Mellitus (T2DM). This study aimed to determine 1) the number of daily steps taken by older people with T2DM, 2) if T2DM is associated with taking fewer steps per day and less likelihood of meeting PA guidelines, and 3) whether these associations are modified by age or gender.Methods:PA was obtained by pedometer from 2 cohorts of older adults with and without T2DM. Multivariable regression was used to determine associations between T2DM, mean steps per day and meeting a guideline equivalent (7 100 steps per day).Results:There were 293 participants with T2DM (mean age 67.6 ± 6.8 years) and 336 without T2DM (mean age 72.1 ± 7.1 years). In women, T2DM was associated with fewer mean steps per day (β = –1306.4; 95% CI –2052.5, –560.3; P = .001) and not meeting the PA guidelines (OR 0.51; 95% CI 0.28, 0.92; P = .03). Associations were not significant in men (P > .05). Only 29.7% of those with T2DM and 33.3% of those without T2DM met PA guidelines.Conclusions:Greater focus is needed on how to maintain and increase PA in older age with particular focus on women with T2DM.
