Abstract Background: This study aims to evaluate the effects of bracing on the Cobb’s angle (CA) and spinopelvic parameters in adolescent idiopathic scoliosis (AIS) patients. Methods: A total of 51 AIS patients who received bracing treatment between January 2018 and August 2019 were retrospectively analyzed. The pro-bracing and in-bracing radiographs were analyzed with regard to the spinopelvic parameters. The CA, pelvic coronal obliquity angle (PCOA), thoracolumbar kyphosis (TK), lumbar lordosis (LL), pelvic incidence (PI), pelvic tilt (PT), sacral slope (SS), sagittal vertical angle (SVA) and coronal vertical angle (CVA) were measured. Results: The mean age at the initiation of bracing was 13.6 ± 1.5 years. The mean pro-bracing CA was 24.0° ± 6.3°. There were no statistically significant differences between pro-bracing and in-bracing measurements of SVA and CVA. However, there were statistically significant differences between the pro-bracing and in-bracing measurements of the CA, PCOA, TLK, LL, PT and SS. A significant correlation was observed between PT variation and TLK variation in the sagittal plane. In the coronal plane, the PCOA variation was correlated to pro-bracing PCOA. Conclusion: Bracing effects of AIS can be extended to the pelvis. The pelvis should retro-rotate correspondingly to TLK hypokyphosis on sagittal plane, whereas in coronal plane, pelvic obliquity was improved independently.

The main types of braces used in complex treatment of these patients are described. The indications for orthotic treatment and its results were analyzed. The orthotics potentialities in conservative correction of spinal deformities were shown.

ABSTRACT Objective: There are no values defined as standard in the literature for the parameters of assessment of cervical sagittal balance in patients with idiopathic scoliosis. This study describes the sagittal cervical parameters in patients with idiopathic scoliosis. Methods: Study carried out in a tertiary public hospital in patients with adolescent idiopathic scoliosis, through the evaluation of panoramic radiographs in lateral view. The Cobb method was used to evaluate cervical lordosis from C2 to C7, distance from the center of gravity (COG) of the skull to C7, measurement of T1 slope, thoracic inlet angle (TIA), neck tilt, and plumb line from C7 to S1 (SVA C7-S1). A statistical analysis was performed, to demonstrate the relationship between the alignment of the thoracic spine in the sagittal plane and the cervical sagittal balance of patients with scoliosis. Results: Thirty-four patients were female (69.4%) and 15 male (30.6%). The mean values for COG-C7 were 0.71 mm (median 0.8 mm/standard deviation [SD]= 0.51 mm). For Cobb C2-C7, the mean was -11.7° (median -10°/SD= 20.4°). The mean slope of T1 was 23.5° (median 25°/SD= 9.5°). The mean cervical version was 58.8° (median 60°/DP= 15.4°). The mean TIA was 81.8° (median 85°/SD= 16.7°). The mean plumb line C7-S1 was -0.28 (-0.3/SD= 1.0). Conclusion: The analysis of the results showed that the mean values for the cervical lordosis are lower than the values described as normal in the literature, suggesting a loss of sagittal cervical balance in these patients.

Abstract Orthosis-wearing compliance is considered to be an important factor in the outcome of orthotic treatment for idiopathic scoliosis (Rowe, et al. 1997; Katz, et al. 1997). However, at the present time a reliable and objective method of measuring orthosis-wearing time is lacking. Current estimates are based on self-reported compliance and estimated wear and tear of the orthosis itself. Availability of an accurate and reliable technique to measure how long a patient wears a prescribed spinal orthosis will allow clinicians to objectively study the relationship between patient compliance and outcome of orthotic treatment, and arrive at rational guidelines for prescribing orthosis wearing hours. The purpose of this study was to develop an accurate and reliable method to objectively measure spinal orthosis-wearing time.
