ABSTRACTIsolated bilateral macrodontia of mandibular second premolars is an extremely rare dental anomaly with only 5 cases reported to date. This case report presents clinical and radiographic findings of isolated bilateral macrodontia in a 12-year-old child. The patient was referred to the clinic with local crowding of mandibular posterior teeth. Radiographic findings revealed the presence of impacted macrodont mandibular second premolars and their distinct morphological appearance, characterized by large, multitubercular, molariform crowns, and tapering, single roots. Following surgical removal of the impacted premolars, orthodontic therapy was initiated to correct the malocclusion. Along with the features and treatment of this rare anomaly, this case report also illustrates the benefits, in terms of treatment planning and surgical technique, of supplementing conventional radiography with cone-beam computed tomography to localize the macrodont premolars and accurately establish their relationship with the neighboring roots and anatomic structures. ABSTRACT Aim The aim of this study was to analyze bone dimensions of the dentate posterior mandible using cone beam computed tomography (CBCT). Objectives The objectives of this study were • To measure thickness of buccal and lingual bone walls in mandibular posterior teeth using CBCT. • To measure alveolar bone width in mandibular posterior teeth using CBCT. Materials and Methods Ten CBCT scans were included in the study (n = 65 teeth). Thicknesses of buccal and lingual walls were measured at measurement point 1 (MP1) and measurement point 2 (MP2). Alveolar width was assessed at most coronal detected alveolar bone (BW1) and at superior border of mandibular canal (BW2). Vertical distance between BW1 and BW2 was measured (H). Data were tabulated and results were statistically analyzed using unpaired t test. Results The study showed that there was an increase in bone wall thickness from 1st premolar to 2nd molar for buccal and lingual alveolar plates. Lingual bone walls were thicker than buccal bone walls at MP1 and MP2 for all teeth. Bone width for premolars was considerably less than bone width of molars. Conclusion Careful preoperative analysis using CBCT is important to assess need of bone augmentation procedures. As the bone thickness and width in the molar region is more adequate, prognosis of implants placed in molar region may be better. Clinical significance Analysis of bone dimensions is of utmost importance for successful outcome of bone augmentation procedures in implant treatment. This case report illustrates the teeth morphology of a chimpanzee and its anatomical variations. A well-preserved skull of a male Pan troglodytes troglodyte chimpanzee was scanned using a cone-beam computed tomography machine. Measurements included tooth and crown height, root length, root canal length and width (posterior teeth), and pulp cavity length (anterior teeth). Nonmetrical parameters included number of canals and foramina per root of every root. Interestingly, the mandibular central incisor was longer than the lateral incisor, and all the mandibular anterior teeth presented with a solitary flame-shaped or conical-calcified structure in their pulp cavity. The premolars are usually dual rooted except for the first maxillary premolar that displayed 3 roots. Other unusual discoveries were the presence of bilateral radicular dens invaginatus in the mandibular first premolars and the possibility of having 2 canals and 2 foramina in the roots of the posterior teeth. The presence of conical stone mineralizations at the pulp cavity and the presence of dens invaginatus were of particular interest.

Abstract Background Endodontic disease can adversely affect the quality of life and therefore early diagnosis and consequent timely treatment is of paramount importance for the Endodontist. Radiology is an essential component in treatment planning, disease monitoring and assessment of treatment outcome. Periapical radiographs and panoramic radiography are frequently utilised but they provide only two-dimensional representation of three-dimensional structures. The advent of cone beam computed tomography (CBCT) offers three-dimensional accuracy of the hard tissue images with a reasonable cost and this has revolutionised imaging of the dentomaxillofacial structures. This imaging system has been seen to overcome some limitations of conventional radiography, as brought out in this review. The improvement in the accuracy is, however, accompanied at the cost of increased radiation exposure to the patient. Nevertheless, smaller areas of exposure are normally appropriate for endodontic imaging, and adjustment in the exposure parameters can further moderate the effective dose (Loubele et al. 37(6):309-18, 2008). Aims and objectives The aim of this review is to present the pertinent literature on the various applications of cone beam computed tomography in the field of endodontics. Methods Literature was electronically searched on the following sources; Medline and Keats Library. Further, a manual search was performed on the following journals: International Endodontic Journal, Journal of Endodontics, Oral Surgery, Oral Medicine, Oral Pathology, Oral Radiology, Endodontology, Journal of Dental Research, European Journal of Oral Sciences & Odontology and Dentomaxillofacial Radiology. A preliminary search was performed to gain an idea of the available literature using keywords ‘Cone Beam Computed Tomography’ to view the volume of the literature evident and identify questions to be addressed in this review. The initial search showed 243 potential articles. After scrutinising the titles and abstracts of the retrieved articles, 70 relevant studies were reviewed in full text. Furthermore, ‘published guidelines on the use of CBCT’ were also searched so as to include the results as an additional source material. All the articles eligible to be included in the review were in the English language and ranged from the year 1960 to the present. Also all the studies reviewed were based on the various uses of cone beam computed tomography in the field of endodontics. The keywords used to search were ‘Cone Beam Computed Tomography (CBCT)’, ‘Conventional radiography’, ‘Applications of CBCT in endodontics’, ‘CBCT and tooth morphology’, ‘CBCT and apical periodontitis’, ‘CBCT and vertical root fractures’, ‘CBCT and resorption’, ‘CBCT and pre-surgical assessment’, ‘CBCT and dento-alveolar trauma’ and ‘CBCT and endodontic outcome’. Results Every case is unique and CBCT should be considered only after studying each case individually. CBCT imaging needs to be adopted or used where information from conventional imaging systems is either inadequate for the management of endodontic problems or inconclusive. Having said that, it is safe to state that CBCT imaging has the potential to become the first choice for endodontic treatment planning and outcome assessment, especially when new scanners with lower radiation doses and enhanced resolution would be available.

The purpose of this article is to present the case of maxillofacial trauma patient with maxillary sinus fracture diagnosed with cone-beam computed tomography (CBCT) and to explore the applications of this technique in evaluating the maxillofacial region. A 23-year-old male patient attempted to our clinic who had an injury at midface with complaints of swelling, numbness. The patient was examined before in emergency center but any diagnosis was made about the maxillofacial trauma. The patient re-examined clinically and radiographically. A fracture on the frontal wall of maxillary sinus is determined with the aid of CBCT. The patient consulted with the department of maxillofacial surgery and it is decided that any surgical treatment was not necessary. The emerging technique CBCT would not be the primary choice of imaging maxillofacial trauma. Nevertheless, when advantages considered this imaging procedure could be the modality of choice according to the case.

Cone beam CT (CBCT) produces threedimensional information on the facial skeleton, teeth and their surrounding tissues; and is increasingly being used in many of the dental specialties. This is usually achieved with a substantially lower effective dose compared with conventional medical computed tomography (CT). Periapical pathologies, root fractures, root canal anatomy and the true nature of the alveolar bone topography around teeth may be assessed. CBCT scans are desirable to assess posterior teeth prior to periapical surgery, as the thickness of the cortical and cancellous bone can be accurately determined as can the inclination of roots in relation to the surrounding jaw. The relationship of anatomical structures such as the maxillary sinus and inferior dental nerve to the root apices may also be clearly visualized. Measurements on CBCT are more accurate when compared with OPG. Therefore, CBCT permits the clinician to have all necessary information when planning dental implants. The purpose of this article is to provide an overview of the unique image display capabilities of maxillofacial CBCT systems and to illustrate specific applications in clinical practice.
