The Use of Cortical Evoked Response Audiometry in the Assessment of Noise-Induced Hearing Loss

OBJECTIVES: The study aims were to determine the incidence of exaggerated hearing thresholds in individuals complaining of noise-induced hearing loss (NIHL) as a result of impulse noise using cortical evoked response audiometry (CERA) and to identify any associated audiometric features. SETTING: We conducted an office-based study. STUDY DESIGN: In this prospective case series, 1154 males complaining of NIHL were assessed with pure tone audiometry; 673 had CERA. Pure tone averages (PTA) and hearing disability were calculated using the Irish and American Medical Association systems. A PTA of >10 dB worse than the CERA average was considered evidence of exaggerated thresholds. RESULTS: The mean PTA was 33 dB. Seventy-two percent had a hearing disability of an average of 26% when assessed by the Irish system. Fifty-four percent had a hearing disability of an average of 30% when assessed by the American Medical Association system. Twenty-six percent of subjects had exaggerated thresholds based on CERA. A binaural hearing threshold of >25 dB at 500 Hz had a sensitivity of 94% and a specificity of 59% for the detection of exaggerated thresholds. CONCLUSION: Exaggerated hearing thresholds are common. A hearing threshold of >25dB at 500 Hz should be considered an indication for CERA testing.

Introduction: Noise has become a very important stress factor in the environment. Drivers are the most commonly affected persons due to trafc noise. The majority of drivers remain unaware about noise induced hearing loss as this is an insidious process, leading to temporary or permanent hearing impairment, but preventable. Pure Tone audiometric test is used to determine the degree of hearing loss in noise exposed individuals. Aim & Objective: To nd the hearing threshold levels for high and mid frequency sounds for both ears in Professional drivers. To evaluate the preponderance of hearing loss between right and left ear using hearing thresholds. Materials & Methods: This study was conducted in 100 Professional male drivers aged between 30-40 years who are working in Tamil Nadu State Transport Corporation, Madurai. Pure tone audiometry was used to measure the high and mid frequency hearing threshold levels in both ears. Results: The mean (SD) hearing threshold for high and mid frequency sounds was signicantly higher in left ear comparative to right ear with P value < 0.05. The statistically signicant difference in hearing loss was observed between both ears. Conclusion : Our study showed statistically signicant higher hearing threshold for high and mid frequency sounds in left ear compared to right ear, that concludes left ear is more prone for hearing loss than the right ear

It is well known that a 4,000-Hz dip is often observed in the initial stage of noise-induced hearing loss and acoustic trauma, but it does not always occur as a result of these causes, and quite often the cause is unknown. Pure tone audiograms of 159 patients (76 cases of noise-induced hearing loss, 62 cases of hearing loss of unknown cause, 21 cases of familial hearing loss) with a 4,000-Hz dip were studied by detailed audiometry to clarify this configuration. The types of dip were classified into 11 groups; however, the basic types are the cone type, bowl type, and dish type, which show the smooth line of the hearing threshold. There were no clear differences in dip configuration among the 3 causes of hearing loss. It was thought that the dips displayed similar configurations regardless of the cause.

Introduction: Noise-induced hearing loss is a significant, often unrecognized health problem among the military pilots. Advanced aircraft are powerful and more efficient but generate high noise causing noise-induced hearing loss (NIHL) among military pilots. Aim: To determine the prevalence of noise-induced hearing loss among pilots of the Bangladesh Air Force (BAF) and its association with aircraft type and flying hours. Methods: This cross-sectional study was carried out at Central Medical Board (CMB), Bangladesh Air Force (BAF) Dhaka from April 2017 to March 2018. The audiometry was done in the ENT department of CMB, BAF. Pure-tone audiometry was used to measure hearing thresholds and following pure tone frequencies were used for the test: 0.25, 0.5, 1, 2, 3, 4, 6 and 8 kHz. The association between flight hours and the development of noise-induced hearing loss was analyzed in the different age groups. Results: The study revealed NIHL in the aircrew was 16.5% with a higher prevalence among the helicopter pilots (18.6%) as compared with fighter (16.7%) and transport (11.5%) pilots. The left ear was affected in most of the cases than the right ear. Pilots of more than 40 years old and ≥1500 flying hours had a significantly higher hearing threshold compared with other categories. Conclusion: Exposure to noise in aircrew is unavoidable. By avoiding prolonged exposure to loud noise environments and utilization of personal hearing safety devices can prevent noise-induced hearing loss. JAFMC Bangladesh. While unadulterated tone audiometry is an obligatory examination, others, for example, research facility and radiology depend on clinical assessment. This current report's on the examination of connection between self-detailed commotion presentation amid relaxation activities and audiological indicators including measured hearing threshold levels (HTL) and tinnitus. The research was conducted by cross sectional survey of 1432 individuals from 11 to 35 years old. Methodology included of a full audiometric assessment including otoscopy, pure tone audiometry (PTA), otoacoustic emission (OAE) and tympanometry, hearing loss investigation, diagnosis, evoked response audiometry radiology, deafness.

Background/Aim. A more recent method, the auditory steadystate response (ASSR), has become more and more important test method due to difference that was found in previous investigations between hearing thresholds determined by the ASSR and the pure-tone audiometry (PTA). The aim of this study was to evaluate the reliability of the ASSR in determining the frequency specific hearing thresholds by establishing a correlation between the thresholds determined by PTA, as well as to evaluate the reliability of ASSR in determining the hearing threshold with respect to the level of hearing loss and the configuration of the PTA findings. Methods. The prospective study included 46 subjects (92 ears) which were assigned to groups based on their level of hearing loss and audiometric configuration. All the subjects underwent determination of hearing thresholds by PTA and ASSR without insight into their previously obtained PTA results. Results. The overall sample differences between the ASSR and PTA thresholds were 4.1, 2.5, 4.4, and 4.2 dB at 0.5, 1, 2, and 4 kHz, respectively. A high level of correlation was achieved in groups with different configurations of PTA findings. The correlation coefficients between the hearing thresholds determined by ASSR and PTA were significant in subjects with all levels of hearing loss. The differences between hearing thresholds determined by ASSR and PTA were less than 10 dB in 85% of subjects (ranging from 4 dB for moderately severe hearing loss to 7.2 dB for normal hearing). Conclusion. The ASSR is an excellent complementary method for the determination of hearing thresholds at the 4 carrier frequencies, as well as determination of the level of hearing loss and the audiometric configuration.

Objective.To determine the prevalence of hearing loss (HL) in patients with ankylosing spondylitis (AS) and to describe frequency-specific hearing threshold changes in this patient population compared to patients without AS.Methods.A systematic review querying 4 databases (PubMed, OVID Medline, Scopus, Cochrane) was performed to identify studies evaluating HL in patients with AS. Metaanalysis was performed to identify overall prevalence rate and OR of HL, as well as to compare mean differences in frequency-specific hearing thresholds between patients with and without AS.Results.Our metaanalysis included 14 studies and 1083 patients (598 with AS vs 485 without AS). The pooled prevalence of HL in patients with AS was 42.4% (95% CI 29.2–56.2). Patients with AS had a significantly higher OR of HL than patients without AS (OR 4.65, 95% CI 2.73–7.91). Mean differences in pure-tone hearing thresholds ranged from 0–5 decibels (dB) for frequencies of 0.25–4 kHz, and from 5–15 dB for frequencies of 6–16 kHz.Conclusion.Patients with AS have higher odds of having HL than patients without AS. The AS population also presents with significantly impaired hearing thresholds across all conventional and extended pure-tone frequencies. This may manifest as slight to moderate HL. Results of this systematic review might justify increased attention to audiologic manifestations of patients with AS.
