Direct prediction of amniotic fluid volume in the third trimester by 3-dimensional measurements of intrauterine pockets: A tool for routine clinical use

Objective:To determine the effect of maternal (oral) hydration on amniotic fluid volume in patients with third trimester oligohydramnios. Design: Interventional study. Place and Duration of Study: Department of Obs & Gynae Unit III, Sir Ganga Ram Hospital, Lahore from May 2002 to October 2002. Patients and Methods: Twenty five women with third trimester oligohydramnios (AFI ?5.0cm) and twenty five controls with normal amniotic fluid volume (AFI 8-24 cm) were prospectively recruited for this study. Maternal urine specific gravity and amniotic fluid index were determined before and after maternal hydration by asking them to drink 2 L of water in 2-4 hours before repeat amniotic fluid index and recorded on printed proformas. Results: Hydration increased amniotic fluid volume in women with oligohydramnios (mean change in amniotic fluid index 4.3 cm, 95% confidence interval 4.02 to 5.06; P value <0.001); as well as in women with normal fluid volume (mean change in Amniotic fluid index 2.7 cm, 95% confidence interval 2.23 to 3.21; P value <0.01). However, percentage increase in mean AFI was 58.6% in the oligohydramnios group, which was significantly greater (P value <0.05) than the percentage increase of 28.4% in control group. Hydration was associated with decrease in urine specific gravity in both groups. Conclusion: Maternal (oral) hydration increases AFV in women with oligohydramnios as well as in women with normal AFV and may be beneficial in the management of oligohydramnios.

Objective: Coffee is frequently (one or two cups/day) consumed throughout pregnancy. Although there are a few studies evaluating caffeine effects on pregnancy; however, a diuretic effect of caffeine on fetal kidneys has not been reported. Therefore, after drinking coffee whether changing of amniotic fluid index (AFI) and fetal renal artery blood flow (FRABF, RI, Resistive index; PI, Pulsatility index) were evaluated in this study. Methods: This clinical study was performed with two groups. For the study group, 63 participants with isolated borderline oligohydramnios who agreed to drink one cup of instant coffee were included in this study while 63 participants with isolated borderline oligohydramnios who did not drink one cup of instant coffee formed the control group. AFI, RI and PI were evaluated both before and after coffee intake.Results: Maternal characteristics of all study population were homogenous. FRABF indices were similar in both before and after coffee consumption. AFI was increased significantly six hours after drinking coffee (p<0.001). Conclusions: The coffee consumption increased the amniotic fluid volume. However it does not seem to affect on FRABF. Objectives: To assess amniotic fluid volume in second and third trimester ofpregnancy in Pakistani population. Period: Three months. Setting: Talha ultrasound, Multan.Material and Methods: I have started my work by collecting data from the month of November,2015 to February, 2016. The machines which have been used for data collection were Xarioand Z-5 ultrasound Doppler machines with transabdominal (3.5-5MHz) probes. A group of 100Pakistani married female patients of different age groups was included. These patients werehaving normal criteria of second and third trimester (14-40 weeks gestational age). Results:100 married female patients were selected having different age group. There were no significantdifference found in the means of large pocket volume and four pocket volumes of second andthird trimester of pregnancy. Conclusion: Ultrasound plays an important role in assessment ofamniotic fluid volume. If there is one unit change in large pocket volume (LPV) than four pocketvolume (FPV) will change by 2.37 and 2.146 in second and third trimester, respectively.
