UNSTRUCTURED DemDx is a differential diagnosis app for students and junior doctors. Starting with a patient’s presenting complaint the app goes through a step-by-process through history, examination and investigation findings to an increasingly refined differential diagnosis list until a single most likely diagnosis is reached. The aim of this project was to assess the accuracy of DemDx in an Emergency Department (ED) setting. Anonymised clinical records for 100 patients were retrospectively obtained from the ED in Beth Israel, Boston, USA. This contained the differential diagnoses from the clerking doctor, who performed the initial assessment (D1). The discharge diagnosis was used as the gold standard diagnosis (D2). D1 agreed with D2 in 74.44% of cases while DemDx agreed with D2 in 85.56% of cases (p=0.0003716). When the first, and thus most likely, differential was taken from D1 and DemDx, they agreed with D2 in 20 and 18.8% of cases, respectively (p=0.1428). This demonstration of the clinical accuracy of the app highlights how it can be a useful medical student education tool.

One of the benefits of collaborating in an interprofesional team is the exchange, and the acquisition of knowledge, skills and operational strategies alongside this community. Here, we share an example and an analogy from the medical field: A biomedical engineer, from a simulation center, who has the task of preserving the functionality, integrity and aesthetics of the simulators and the equipment, acquires a role similar to that of a doctor who seeks strategies that focus on maintaining a healthy population. The following is a review of our experience in the Centro de Enseñanza por Simulación de Posgrado (CESIP) of the National Autonomous University of Mexico (UNAM). It should be noted that none of the strategies stated here can be considered as a reference “of excellence” for the operation of a simulation center, due to the great number of particularities in each of them. Finally, we are convinced that sharing our experience may be useful for other centers, especially due to the lack of information available. Keywords: Simulators, maintenance, biomedical, conservation.

SynopsisPsychosomatic medicine begins with the Greeks. It finds a place in Galen's system as diseases of passion, a concept current until the middle of the nineteenth century. The great French and German clinicians of the nineteenth century were all familiar with psychosomatic diseases. During the twentieth century the field was for a while monopolized by psychoanalysts. The psychosomatic specialist is essentially the doctor who listens to the patient.
