Background.Studies from low- and middle-income countries (LMIC) indicate that the use of audio computer-assisted self-interviewing (ACASI) is associated with more accurate reporting of sensitive behaviors (e.g. substance use and sexual risk behaviors) compared with interviewer-administered questionnaires. There is a lack of published information on the process of designing, developing, and implementing ACASI in LMIC. In this paper we describe our experience implementing an ACASI system for use with a population of orphans and vulnerable children in Zambia.Methods.A questionnaire of mental health, substance use, and HIV risk behaviors was converted into an ACASI system, tested in pilot and validity studies, and implemented for use in a randomized controlled trial. Successes, barriers, and challenges associated with each stage in the development and implementation of ACASI are described.Results.We were able to convert a lengthy and complex survey into an ACASI system that was feasible for use in Zambia. Lessons learned include the importance of: (1) piloting the written and electronic versions; (2) proper and extensive training for study assessors to use ACASI and for those doing voice recordings; and (3) attention to logistics such as appropriate space, internet, and power.Conclusions.We found that ACASI was feasible and acceptable in Zambia with proper planning, training, and supervision. Given mounting evidence indicating that ACASI provides more accurate self-report data and immediate data download compared with interview-administered measures, it may be an effective and economical alternative for behavioral health research studies in LMIC.

Objective To assess the relationship between the frequency of being drunk and high during sex, and condom use errors and problems (CUEP) among a sample of high-risk young Black males recruited from the United States. Methods: Data were collected in clinics treating sexually transmissible infections in three cities in the southern United States. Males 15–23 years of age (n = 697) who identified as African-American and reported recent (past 2 months) condom use were eligible. Measures of alcohol and drug use, as well as condom use behaviours were assessed by audio-computer assisted self-interview. Eighteen CUEP were included in this assessment. Results: Sixteen bivariate correlations were obtained. The magnitude of the coefficients was small, ranging from 0.01 to 0.13. Only three were significant. These were positive associations between the frequency of being drunk and the frequency of unprotected vaginal sex, as well as the frequency of the 18-item measure of CUEP. A significant correlation was also found between the frequency of being high during sex and the frequency of unprotected vaginal sex. Adjustments for age did not change the findings. Conclusions: Interventions designed to promote safer sex behaviours among young Black males attending sexually transmissible infection clinics are no more likely to benefit patients through the inclusion of messages and training attempting to dissuade the use of alcohol and drugs before or during sex.

This paper will discuss the integration of electronic Case Report Forms (e-CRFs) into an already existing Android-based Audio Computer-Assisted Self-Interview (ACASI) software solution that was developed for a public health project in Kampala, Uganda, the technical outcome results, and lessons learned that may be useful to other projects requiring or considering such a technology solution.  The developed product can function without a connection to the Internet and allows for synchronizing collected data once connectivity is possible.  Previously, only paper-based CRFs were utilized at the Uganda project site.  A subset or select group of CRFs were targeted for integration with ACASI in order to test feasibility and success.  Survey volume, error rate, and acceptance of the system, as well as the operational and technical design of the solution, will be discussed.

Objective Marijuana use is associated with placenta-mediated adverse pregnancy outcomes including fetal growth restriction, but the mechanism remains uncertain. The objective was to evaluate the association between maternal marijuana use and the feto-placental weight ratio (FPR). Secondarily, we aimed to compare placental histology of women who used marijuana to those who did not. Study Design This was a secondary analysis of singleton pregnancies enrolled in a multicenter and case–control stillbirth study. Prior marijuana use was detected by electronic medical record abstraction or cord homogenate positive for 11-nor-delta-9-tetrahydrocannabinol-9-carboxylic acid. Prior tobacco use was detected by self-report or presence of maternal serum cotinine. Stillbirths and live births were considered separately. The primary outcome was FPR. Association of marijuana use with FPR was estimated with multivariable linear modeling adjusted for fetal sex, preterm birth, and tobacco use. Comparisons between groups for placental histology were made using Chi-square and stratified by live birth and stillbirth, term and preterm deliveries, and fetal sex. Results Of 1,027 participants, 224 were stillbirths and 803 were live births. Overall, 41 (4%) women used marijuana during the pregnancy. The FPR ratio was lower among exposed offspring but reached statistical significance only for term stillbirths (mean 6.84 with marijuana use vs. mean 7.8 without use, p < 0.001). In multivariable modeling, marijuana use was not significantly associated with FPR (p = 0.09). There were no differences in histologic placental features among those with and without marijuana use overall or in stratified analyses. Conclusion Exposure to marijuana may not be associated with FPR. Similarly, there were no placental histologic features associated with marijuana exposure. Further study of the influence of maternal marijuana use on placental development and function is warranted to better understand the association between prenatal marijuana use and poor fetal growth.
