Vouchers and Consumer-Directed Care: Implications for Community Care Services in Hong Kong

Abstract There has been considerable interest in the concept of consumer-directed care (CDC) for ageing populations. One type of CDC employs vouchers that older people can use to procure a panel of social services that best fits their needs. The voucher-based strategy has been institutionalised in Hong Kong as the Community Care Service Voucher for the Elderly (CCSV). Using a qualitative, semi-structured interview method, researchers probed the experiences of two groups (twenty-six CCSV users and twenty-seven CCSV non-users) with the voucher programme between March 2014 and September 2015 during the first phase of CCSV. The interviews revealed a set of social and institutional supports needed to empower clients to effectively utilise the voucher. These supports (or forms of capital) were consistently present amongst active CCSV users and deficient amongst non-users. The research demonstrates that instituting a voucher does not, by itself, lead to optimised choices but, rather, that a network of institutional and social resources must be assembled in support of the voucher programme. In some instances, a case management approach may be needed, as older people (and family) need to combine different forms of institutional and social support to comprehend the voucher programme and utilise it for their particular needs.

In order to solve the problem of care, we suggest community care, and through this, we will provide medical services that meet individual needs. Korea provides care services in advance of the community care project and implements quality control to improve the quality of these social services. Therefore, this study aims is to intend to compare and analyze the factors affecting user satisfaction in care services in both 2013 and 2016.Methods: We used quantified secondary data based on social services performed analysis reports. Based on the evaluation indexes for care service in 2013 and 2016, we used commonly used indexes for analysis.Results: As a result, there was a difference between care services quality evaluation (QE) indexes by profit type, and care services evaluation indexes had an effect on user satisfaction. In addition, the change rate in the care service evaluation scores affects the change rate in user satisfaction.Conclusions: Therefore, in order to increase satisfaction with care service, evaluation indexes by service type should be diversified and differentiated. In addition, it should be composed of field evaluations related to user satisfaction to provide care services appropriate for local characteristics.

A comparison of the development of community care for the elderly in the province of Ontario, Canada, and in the State of Israel is presented in the light of the economic constraints currently challenging the expansion of welfare state services. The inquiry identified several common issues regarding the nature of the policy mandate for long-term care delivered in the home, the structure of the service delivery system, and matters concerning funding arrangements for community care. Issues that emerged in both settings include the relationship between health and social services in the delivery of care at the local level; the separation of purchaser and provider functions; the question of needs-driven versus service-driven social care provisions; accessibility concerns and the aspiration for one-stop entry to the delivery system; the growing involvement of the private sector in the delivery of domiciliary-based personal care services; and the relative paucity of current efforts to address the needs of family caregivers.

This literature review paper examined the development of time banking in Hong Kong. Time banking is a system of transaction using time. Members of a time bank can earn time credits through work which they can spend these credits later. Time banking has become an international movement with participants included people in different age groups, local businesses, staff from participating organizations and even the organizations themselves. This diversity of time banking systems in different areas showed modifications of time banking systems can increase their uses, functions and effectiveness, and also can improve their versatility so as to provide tailor-made applications on different problems in different situations. Although around half a century has passed after the first time bank appeared (Miller, 2008), there was only one time bank in Hong Kong involving the elderly care service and the number of participants were less than 200 (Tsui, 2018; Ho, 2018; Lee, 2018). To improve this situation it is necessary to see whether it is possible to modify the present time banking system so that more extensive and effective applications can be provided. The present paper will address this gap of knowledge through proposing a time banking system on young people with the novel time credit multiplier concept with an aim to alleviate the serious and ever increasing manpower shortage problem of the elderly care services in Hong Kong.

PurposeHong Kong implements a policy for the aging population involving the core themes of “aging in place as the core,” “institutional care as backup” and “continuum of care.” Encouraging elders to live independently at home is a top priority, and elders who are not able to live at home independently are provided with various residential care services, namely Hostels for the Elderly, Homes for the Aged, Care and Attention (C&A) Homes for the Elderly and Nursing Homes (NHs). The purpose of this paper is to analyze the adoption of the publicly funded model of providing residential care services of elderly in Hong Kong.Design/methodology/approachThe paper analyzes the current conundrum encountered by elders in residential care services and makes recommendations. A comprehensive literature review was conducted covering relevant government reports, academics' journal papers and nongovernmental organizations’ reports concerning residential care service of elderly from 1965 to present.FindingsSubsidized residential care homes for the elderly (RCHEs) clearly outperform private RCHEs in terms of space and staff provisions, but the supply of subsidized RCHEs cannot meet the demand. Hence, between 2007 and 2018, the average waiting time was 33 months for NHs and that for C&A homes was 23 months. Several viable measures to meet the demand are purchasing Enhanced Bought Place Schemes (EBPSs) from private RCHEs, subsidizing elders who opt for living in private RCHEs by providing them with Comprehensive Social Security Assistance (CSSA) and residential care service voucher (RCSV) and subsidizing elderly applicants who opt for living in RCHEs in Guangdong. However, these viable measures are problematic because of the inadequate quality of EBPSs and private RCHEs, which is attributed to the costing arrangement of public and private RCHEs that were established in the colonial period. The brief history of RCHEs also indicates a deviation from the original policy aim, namely aging in place, which was introduced in the Green Paper on Services for the Elderly in 1977.Practical implicationsThe supply and quality of community and home care services should be thoroughly examined; effective community and home care services can prevent and even delay unnecessary institutionalization. Another complementary solution is to devise a long-term plan for residential care services. To address disparities in quality standards in different RCHEs, adopting the combination of punitive and compliance models such as conducting frequent inspections and implementing an accreditation system for private RCHEs is imperative.Originality/valueAlthough the principle of “aging in place” originated in 1977, the institutionalization rate 6.8% of elders was unexpectedly high in Hong Kong and even surpassed the Asian counterparts. It necessities to rethink how to implement policy concerning long-term care services of elders.

Introduction: Day care centres (DCC) aim to support older adults living in their own homes by providing a variety of activities to promote the independence of those people and reduce the caregiver’s burden. In Poland, there are no standards for providing this form of care. The provided care is delivered by different organisations, and there is a lack of quality control in the majority of places. Regrettably, in Poland, there is a paucity of research on the holistic needs of the elderly attending DCC. Aim of this study: This is the first study which has aimed to identify the Polish day care centres attendees’ needs to ensure that the increasing number of older people receive the best possible care, and as a part of the quality improvement process for recommendation development of the new day care services model in Poland within the ‘Homely Marina’ project. Methods and Materials: A representative sample (n = 269) was randomly selected from day care centres attendees (estimated as 10,688) in Poland. An anonymous survey for the assessment of needs was developed by the authors. Results: The respondents assessed the level of provided services as very good or good. Best rated services included meals, supportive and welfare services (occupational therapy, music therapy, art therapy, cognitive training). Almost half of the participants indicated the need for company as the main reason to attend a DCC. This research reveals a lack of support with regard to additional staff: e.g., a nurse. Conclusions: In Poland, the services offered in DCC should focus on social isolation and sense of loneliness prevention, and maintenance of social activity of the elderly. The presented analysis of needs in Polish day care centres suggests a need for changes which may improve the quality of services. There is a great need to find a balance between home-based care and in-patient care, using better integration of available services and strengthening support for informal caregivers. Robust research with a collection of meaningful outcomes is required to ensure that in Poland, the increasing number of older people is enabled to access high-quality day care service provision.

Abstract Background To meet the needs of aging and dementia patients in Taiwan, this study designed a nursing system that includes communication, location tracking, and fall detection, and early warning services. The main purpose of this research is to provide timely services to the elderly and patients and hope to reduce the burden when the number of nursing staff decreases. This article is a remote disease care service platform with the Internet of Things (IoT) devices to monitor the location of the elderly and whether they have dropped warning alerts. Results The device is connected to the patient's waist and chest, monitors the patient's movement and behavior, and transmits messages to the back-end system, and informs caregivers through mobile phone applications when unexpected or shocking events occur. The system can identify whether the patient has fallen, accidentally, or long-term inactivity. The device is equipped with sensors that enable it to monitor the patient's location and behavior data through Bluetooth and GPS technology. Finally, we proposed a basic model and an integrated model that will industrialize the system and is expected to play a role in a larger patient population. Conclusions The system developed in this research has passed the Activities of Daily Living (ADL) test and verification, and is expected to provide appropriate safety care services for nursing homes and elderly residences.

This research focuses on a service acquisition mode for parents in urban separated families of China to promote the popularization of elderly care services in Internet economy. Based on interviews and questionnaires, authors find the pain point of the elderly care service acquisition mode and propose a tripartite participatory mode. Using this new service acquisition mode, adult children can play an assistant role with a smartphone-based service purchase platform. Next, in order to determine what services should be provided priorly on the platform, an empirical study based on questionnaires and KANO model is carried out to screen a propriate services items. Through the innovation of new elderly care service acquisition mode, more practitioners are supposed to expands their business effectively in aging market, and to play an ever more active role in the growth of Internet economy.

PurposeUnder the impact of Coronavirus disease 2019 (COVID-19), this paper contributes in the deployment of the Artificial Intelligence of Things (AIoT)-based system, namely AIoT-based Domestic Care Service Matching System (AIDCS), to the existing electronic health (eHealth) system so as to enhance the delivery of elderly-oriented domestic care services.Design/methodology/approachThe proposed AIDCS integrates IoT and Artificial Intelligence (AI) technologies to (1) capture real-time health data of the elderly at home and (2) provide the knowledge support for decision making in the domestic care appointment service in the community.FindingsA case study was conducted in a local domestic care centre which provided elderly oriented healthcare services to the elderly. By integrating IoT and AI into the service matching process of the mobile apps platform provided by the local domestic care centre, the results proved that customer satisfaction and the quality of the service delivery were improved by observing the key performance indicators of the transactions after the implementation of the AIDCS.Originality/valueFollowing the outbreak of COVID-19, this is a new attempt to overcome the limited research done on the integration of IoT and AI techniques in the domestic care service. This study not only inherits the ability of the existing eHealth system to automatically capture and monitor the health status of the elderly in real-time but also improves the overall quality of domestic care services in term of responsiveness, effectiveness and efficiency.
