Carcinosarcomas of the urinary bladder are rare biphasic neoplasms, consisting of both malignant epithelial and malignant mesenchymal components, and the prognosis of this tumor is unfavorable in most patients with even possibility of resection of disease. A 77-year-old male with a history of transurethral resection (TUR) of urothelial carcinoma (UC) of the bladder and adjuvant intravesical chemotherapy with pirarubicin 10 years ago revisited our department with a gross hematuria. Cystoscopy demonstrated an approximately 2.5 cm nonpapillary tumor on the right wall of the bladder. Pelvic MRI showed the tumor without extending the base of the bladder wall. The tumor could be completely removed with TUR. The malignant epithelial elements consisted of high-grade UC and the majority of mesenchymal components were fibrosarcomatous differentiation based on immunohistochemical studies. The tumor could be pathologically also suspected to be an early stage on TUR specimens. Although he has received no additional intervention due to the occurrence of myocardial infarction at three weeks after the TUR, he has been alive with no evidence of recurrence of the disease 27 months after the TUR. Some early stages of bladder carcinosarcoma might have a favorable prognosis without aggressive treatments.

Actinomycosis of the urinary bladder is a rare anaerobic bacterial infection caused by Actinomyces isrealii. Initial diagnosis is often difficult and this disease is easily misdiagnosed as a urothelial or urachal tumour. The definitive diagnosis is usually made postoperatively via tissue pathology. We discuss a case of a 54-year-old male with a smoking history and a 2.5-week history of gross hematuria. Ultrasound, computed tomography and cystoscopy revealed a large inflammatory mass adherent to the right, anterior bladder wall, suggesting malignancy. Transurethral resection and histological pathology subsequently confirmed inflammatory urothelium and gram-positive bacteria consistent with actinomyces species.

Percutaneous minimally invasive radiofrequency (RF) ablation has not been described for lymphoma. This image-guided modality is presented in 3 different settings for the treatment of refractory lymphoma. The first patient received RF ablation for the curative treatment of a solitary residual hepatic mass following rituximab-based chemotherapy for a posttransplantation lymphoproliferative disorder (PTLD) and is disease-free 4 years later. The second patient received RF ablation for successful palliation of progressive follicular lymphoma adjacent to the bladder wall following chemotherapy and maximum radiation. The third patient received RF ablation for prevention of airway obstruction from progressive diffuse large B-cell lymphoma of the right neck following chemotherapy and maximum radiation. RF ablation may be clinically beneficial and should be considered for the treatment of local lymphoma that is refractory or not amenable to standard approaches.

We report a case of a 41-year-old man with a solitary functioningleft kidney and history of chronic pelvic discomfort associatedwith lower urinary tract symptoms. Imaging revealed a largecystic structure in the pelvis attached to a dilated tortuous ureteron the right with congenital absence of the right kidney. The patientunderwent laparoscopic removal of the pelvic cyst and dilatedright ureter. Pathological assessment revealed mesonephric remnantsrepresenting dysplastic renal tissue attached to a dilated andobstructed megaureter, extending into the bladder wall and forminga large pelvic cyst. The patient’s symptoms resolved. A laparoscopicapproach represents an excellent surgical option for pelvicpathology.Nous décrivons le cas d’un homme de 41 ans porteur d’un seulrein fonctionnel (gauche) et ayant des antécédents de douleurspelviennes chroniques liées à des symptômes affectant les voiesurinaires inférieures. Les épreuves d’imagerie ont révélé une massekystique volumineuse au niveau du pelvis, une dilatation et unesinuosité urétérales du côté droit et l’absence congénitale derein droit. Le patient a subi une ablation par laparoscopie du kystepelvien et de la section dilatée de l’uretère droit. L’évaluationpathologique a révélé des vestiges mésonéphriques constituésde tissu rénal dysplasique lié à un méga-uretère obstrué avec dilatationkystique se prolongeant dans la paroi vésicale et formant ainsiun kyste pelvien volumineux. Les symptômes du patient ont disparu.Une approche laparoscopique représente une excellenteoption chirurgicale en présence de pathologie pelvienne.

Aim. Study of heart function in the patients with CVD. Materials and methods. 46 patients with varicosity (VD) and 34 patients with post-thrombotic disease (PTD) were examined; the control group was represented by 15 healthy volunteers. The diagnosis was established using the CEAP basic classification. The study did not include patients with a diagnosed arterial hypertension, diabetes mellitus, chronic lung disease, significant hemodynamic heart defects, coronary heart disease. Ultrasound examination of the heart and veins of the lower extremities was performed on a Saote My Lab Alpha, Acuson Sequoia 512 apparatus. In addition to the standard protocol of heart ultrasound examination, the parameters of the right heart were calculated: sizes of the right ventricle (RV), right atrium, thickness of the anterior wall of the pancreas; to assess the ejection fraction (EF) of the pancreas the mobility of the lateral edge of the tricuspid ring was calculated, and the pressure on the tricuspid valve (TV) was measured. Diastolic ventricular function was studied by spectrograms of tricuspid and mitral blood flow. Results. Most of the indicators of cardiac activity in patients with VD were within normal limits, but a tendency to increase increasing of the right heart size was noted. In addition, the thickness of the interventricular septum and the right ventricle (RV) anterior wall was found to increase from 0.8 to 1.1 cm and from 0.3 to 0.5 cm, respectively, according clinical classes from C2 to C6 (CEAP). Eject fraction (EF) of both the RV and the left ventricle (LV) were also within normal limits, but with a tendency to decrease (67.8 % – C2, to 62 % – C6). The growth of the clinical class is followed by the increasing of percentage of non-restrictive blood flow through the tricuspid valve (TV). The restrictive type of blood flow in patients with VD had not been identified. Patients with PTD also showed a tendency to increase the right heart. However, whereas the size of the RV, as a rule, did not exceed 3.0 cm, the size of the right atrium was slightly higher than normal one in the clinical class C4 and C5.6. All the patients had EF of LV within normal limits, but it slightly decreased by the growth of class. Only patient classes C3 and C4 had EF of RV within the normal range. The 18 % of patient class C5.6 had EF lower than normal with value 48%. Diastolic dysfunction (DD) of the RV was detected in 73.3% of patients with class C3 and 100% with classes C4 and C5.6. Moreover, a restrictive type of blood flow through TV appeared from class C4 and the percentage increased up to 27.2% (class C5,6). Conclusions. DD of the RV was the main hemodynamic disorder.
