An overview of issues related to trauma is offered. Freud’s influence, definitions of trauma and complex trauma, epidemiology, adverse childhood experience studies, neurobiological aspects of trauma, characteristics of trauma clients, trauma-informed care and its implications for treatment are all outlined and reviewed.

People with intellectual disabilities are more likely to experience adverse childhood experiences than those in the general population. Additionally, the conceptualization of ‘trauma’ is far broader than traditionally understood in order to encompass the far reaching relational nature of people with intellectual disabilities traumatic experiences. This reflective account details the first steps one service took to embrace trauma-informed care as a whole systems approach. The paper is a response to calls following conference presentations about our work, to share the process of the beginning of this journey, it also aims to provide key learning points, practical considerations and questions for reflection in order to support other services to begin their own relationships with trauma-informed care.

Objectives. To assess police contact as a potential adverse childhood experience by measuring its prevalence, nature, and distribution among urban adolescents. Methods. Detailed US population-based data on youth‒police contact were collected in the Fragile Families and Child Wellbeing Study (n = 2478) from 2014 to 2017. Using regression modeling, I assessed adolescents’ police exposure and the magnitude and robustness of racial disparities in police contact. Sensitivity analyses examined disparities by behavior and socioeconomic context. Results. Urban youths are heavily policed, beginning in preadolescence. Exposure to policing is unevenly distributed, with non-White adolescents—particularly Black boys—reporting more, and more aggressive, contact than their White counterparts. Hispanic‒White differences and disparities in girls’ experiences were less pronounced but present, particularly in how intrusive stops were. Intrusion disparities were robust to most behavioral controls, but not observed among youths with higher socioeconomic status. Conclusions. Given extant literature documenting adverse health consequences of police encounters, findings implicate policing as a driver of health disparities in adolescence and throughout the life course. Public health infrastructure dedicated to the prevention and treatment of adverse childhood experiences is well suited for mitigating these harms and inequities. (Am J Public Health. Objectives: This article presents the theoretical basis, initial deployment strategies, and resulting preliminary findings of a program implemented in residential treatment centres (RCs) in child welfare. “Program Penguin” aimed to help workers develop trauma-informed attitudes and implement trauma-informed practices, make the workplace more responsive to the well-being of RC workers, and reduce the use of restraints and seclusion among school-aged children in RCs. Methods: Informed by the theories of complex trauma (National Child Traumatic Stress Network Complex Trauma Task Force, 2003), polyvicitimization (Finkelhor et al., 2007), Attachment, Self-Regulation and Competency (ARC; Blaustein & Kinniburgh, 2018) and Positive Behavioural Interventions and Supports (PBIS; Sugai & Horner, 2002), Program Penguin was developed and deployed using the social innovation approach (Fixsen et al. 2005). The key stages of social innovation will here be used to describe the implementation process. Results: Changes in practices were observed, RC worker attitudes towards trauma-informed care were assessed and showed strong effects between multiple covariables. RC worker support needs were identified, and a reduction in the use of restraints and seclusions was shown. Key strategies towards the development and maintenance of buy-in and meaningful change in practices are also described. Implications: Changes observed at all levels of this implementation suggest Programme Penguin is a promising approach, despite local issues that arose and the challenges inherent to program deployment within child protection settings. It appears a trauma-informed program using positive behavioural approaches and leveraging existing organizational strengths may impact intervention strategies, worker attitudes, and the use of restraints and seclusions against children in RCs.
