Simplified prosthetic rehabilitation of a patient with partial finger amputation after a firearm injury

Implant-supported prostheses hygiene and peri-implant tissues health are considered to be predictive factors for successful prosthetic rehabilitation. Therefore, the purpose of this study was to evaluate the effectiveness of brushing associated with oral irrigation measured as biofilm-removing capacity, maintenance of healthy oral tissues, and patient satisfaction. A randomized, crossover clinical trial was conducted with 38 patients who wore conventional maxillary complete dentures and mandibular overdentures retained by the O-ring system. The patients were instructed to use the following hygiene methods for 14 days: mechanical brushing [MB (brush and dentifrice - Control)]; and MB with oral irrigation [WP (Waterpik - Experimental)]. Biofilm-removing capacity and maintenance of healthy oral tissues were evaluated by the Modified Plaque Index (PI), Gingival Index (GI), Probing Depth (PD), and Bleeding on Probing Index (BP) recorded at baseline and after each method. In parallel, patients answered a specific questionnaire using a Visual Analogue Scale after each hygiene method. Data were analyzed by Friedman and Wilcoxon tests (α=0.05). The results showed significantly lower PI, GI, PD, and BP indices after application of the hygiene methods (P<0.001) than those observed at baseline. However, no significant difference was found between MB and WP. The satisfaction questionnaire responses to both methods showed high mean values for all questions, with no statistically significant difference found between the answers given after the use of MB and WP (P>0.05). The findings suggest that WP was effective in reducing PI, GI, PD, and BP indices and provided a high level of patient satisfaction.

The purpose of this study is to quantify the existing interrelationship between the anesthetic substances used in the implanto-prosthetic rehabilitation procedures with the general state of the patient, as well as the specific implant rehabilitation work.The analyzed lot brought together 110 patients, candidates for implanto-prosthetic rehabilitation, subjected to a general and local evaluation protocol for choosing general anesthesia in accordance with the general state and complexity of the implanto-prosthetic algorithm. Regarding connection between therapeutic approaches in oral implantation and types of anesthesia , we obtained correlative aspects between the use of inhalosedation and implantation algorithm in 80% clinical cases and augmentation with implantation was found in 20%.General anesthesia with Midazolam has been correlated with implantation and augmentation in 77% clinical cases and sinus lift and implantation was found in 23% clinical cases.General anesthesia with Midazolam was used in 64% clinical cases with implantation and sinus lift and 36% clinical cases with implantation.Careful monitoring of the patient and proper dental surgery where the intervention is performed are the key for a therapeutic success regarding sedation and anesthesia.

Treatment of patients with malignant tumor of oropharyngeal region is a difficult problem due to the last detection of diseases and complex anatomic structure, despite the emergence new and improvement methods of the treatment of cancer of oropharyngeal region. Complex rehabilitation of this patients presents great difficulties due to the disturbance of breath, swallowing, speech, and the lack of a single methodological approach to treatment. It is needs to improve the effectiveness of prosthetic care for patients with malignant tumor of oropharyngeal region.

SAŽETAK: Amputacija prsta čest je javnozdravstveni problem koji je moguće spriječiti. Studija definira epidemiologiju amputacije prsta kako bi se utvrdili najvažniji čimbenici i predložila strategija prevencije. Studija je provedena 2015. u glavnoj sveučilišnoj ortopedskoj bolnici u Shirazu. U studiju su bili uključeni svi bolesnici s bilo kojom vrstom amputacije prsta. Za svakog bolesnika je popunjen i analiziran upitnik s demografskim podacima i tipom ozljede. Sudjelovalo je 100 bolesnika (92 muškarca i 9 žena, prosječne dobi 32.5 ± 18 godina). Najveći broj bolesnika bili su mlađi muškarci. Više od polovice (53 slučaja) pretrpjeli su ozljede na radu u vezi s radom. Amputacija prsta zbog nagnječenja u vratima bila je drugi najčešći uzrok amputacije (18 %). Približno 59 % ozlijeđenih navelo je nepažnju pri radu kao uzrok nezgode. Amputacije prsta desne ruke bile su brojčano jednake amputacijama na lijevoj ruci. Najviše je bilo amputacija kažiprsta (27 %) i srednjeg prsta (2 %). Utvrđeno je da su mlađi, slabije obrazovani muškarci te početnici na poslu podložni najvećem riziku od amputacije prstiju, naročito ako imaju povijest zlouporabe droga. Hitno je potrebno provesti sljedeće učinkovite mjere: veći nadzor, više programa osposobljavanja, otkrivanje ovisnosti o drogama i korištenje sigurnijih strojeva.
