Habilitative therapy among preschool children: Regional disparities in the early intervention population

Background and aims In many countries, early detection and diagnosis of autism spectrum disorder is largely dependent on parents’ initial concern with early symptoms of autism spectrum disorder. Previous research on parental perceptions of the autism spectrum disorder diagnostic process indicates that parental satisfaction may be due to either the timing of the diagnostic notification or the provision of post-diagnostic support. The objective of this research is to study the diagnostic notification process and its impact on parents who are informed of their young child’s diagnosis before they notice a problem and whose child undergoes early intervention therapy. Methods Eighty parents of preschool children diagnosed and undergoing early intervention for autism were surveyed to examine their experience of the diagnostic disclosure process. Results Of 68 respondents, 39 (58.2%) approved of the timing of diagnostic notification, while 10 of 13 dissatisfied respondents indicated that the diagnosis was communicated too late. However, there was no correlation between a higher degree of parental satisfaction with the diagnostic notification process and earlier timing of notification. Conclusions Although it is preferable to communicate a diagnosis of childhood autism as soon as possible, findings suggest that a highly individualized approach, allowing a degree of latitude in the timing of notification, may be permissible, depending on the individual case and parental readiness to receive the diagnosis. Implications Findings have clinical implications related to the concept of optimality of diagnostic disclosure as related to the diagnostic notification process, though later notification tends to lead to more dissatisfaction.

Introduction: Ling’s six sound test is a quick and simple test to ascertain access to speech sounds essential for development of optimum listening and speaking skills. However, most of the normative data available for Ling’s six sound test is adult based. Therefore, there is a need to develop normative data for subsequent use with children who are receiving early intervention procedures. This study aims at obtaining the awareness and identification thresholds for Ling’s six sounds for normal hearing preschool children between 3 to 6 years of age. Methods: Each of the six sounds of Ling’s six sound test namely; |a| |i| |u| |s| |sh| |m| were presented in the sound field through an audiometer to fifty 3 to 6-year-old children with normal hearing sensitivity in a sound treated room. Each child was explained the task and was conditioned well before the testing. Both awareness and identification thresholds were obtained. The lowest level at which the child responded was noted as the threshold. Results: The lowest threshold for awareness and identification was obtained for the sound |a| (10.3 dB HL and 15.4 dB HL respectively). whereas the highest threshold was obtained for the sound |s| (18.2 dB HL and 24 dB HL respectively. A significant difference was seen in thresholds across all the sounds for both awareness and identification. Conclusions: The differences seen in thresholds across all the sounds for both awareness and identification are due to several higher order factors as well as the acoustic and spectral features of each of the Ling’s six sounds.

ABSTRAK Keterlambatan perkembangan anak pada aspek tertentu saat ini masih menjadi slah satu masalah kesehatan bagi anak di Indonesia yang dapat mengurangi kualitas sumber daya manusia di masa mendatang. Melakui pendekatan famly cantered-care orang tua anak dapat melakukan intervensi dini terhadap masalah keterlambatan perkembangan anak sebagai upaya pencegahan terhadap keterlambatan perkembangan anak lebih lanjut dengan pemberian stimulasi perkembangan terarah selama 2 minggu. Stimulasi adalah perangsangan dari luar lingkungan anak dimana dengan mengasah kemampuan anak secara terus-menerus kemampuan anak dimana dengan mengasah kemampuan anak secara terus-menerus kemampuan anak akan meningkat. Kata kunci : Anak Praskolah, Keterlambatan Perkembangan Anak, Stimulasi Perkembangan  ABSTRACT The delay in children's development on specific aspects is still one of the health problem for Indonesian children which can reduce the quality of the human resources in the future. Through a family-centered-care approach, parents will be able to do early intervention for the delay in children development problem prevention against further delay in children development with a directional development stimulation for 2 weeks.
