ABSTRACT Miramistin is a topical antiseptic with broad antimicrobial action, including activity against biofilms and a clinical profile showing good tolerability. Miramistin was developed within a framework of the Soviet Union Cold War Space Program. It is available for clinical use in several prior Soviet bloc countries, but barely known outside of these countries and there is almost no mention of miramistin in the English literature. However, considering emerging antimicrobial resistance, the significant potential of miramistin justifies its re-evaluation for use in other geographical areas and conditions. The review consists of two parts: (i) a review of the existing literature on miramistin in English, Russian and Ukrainian languages; (ii) a summary of most commonly used antiseptics as comparators of miramistin. The oral LD50 was 1200 mg/kg, 1000 mg/kg and 100 g/L in rats, mice and fish, respectively. Based on the results of the review, we suggest possible applications of miramistin and potential benefits over currently used agents. Miramistin offers a novel, low toxicity antiseptic with many potential clinical uses that need better study which could address some of the negative impact of antimicrobial, antiseptic and disinfectant resistance.

The provision of healthcare is being challenged by a ‘perfect storm’ of forces including an increasing population with multiple comorbidities, high expectations and resource limitations, and in the background, the pre-eminence of the ‘curative medical model’. Non-beneficial (futile) treatments are wasteful and costly. They have a negative impact on quality of life especially in the last year of life. Among professionals, frequent encounters with futility cause moral distress and demoralisation. The factors that drive non-beneficial treatments include personal biases, patient-related pressures and institutional imperatives. Breaking loose from the perceived necessity to deliver non-beneficial treatment is a major challenge. Curative intent should give way to appropriateness such that curative and palliative interventions are valued equally. Goals of treatment should be shaped by illness trajectory, the risk of harms as well as potential benefits and patient preferences. This strategy should be reflected in professional training and the design of acute services.

PurposeThe purpose of this paper is to gain understanding into the human factors which might impede the change process. Change is inevitable in contemporary organizations and particularly within the healthcare field with respect to information technology (IT). Regardless of the amount of literature surrounding change management process organizational leaders will often ignore the human factors associated with the introduction of new IT.Design/methodology/approachThis study sought to examine physician resistance surrounding the Electronic health record (EHR) change process through the lens of each of these three aspects of the Bovey and Hede (2001a) model through semi-structured interviews with physicians. Semi-structured interviews were conducted with physicians from hospitals within the Midwest.FindingsThe findings suggest that physicians have been impacted by the EHR change management system within their hospitals. Though each of the participants experienced different issues; it was clear from the data the change to an EHR system was disruptive to their day-to-day routines and caused various challenges. EHR change management research demonstrates physicians are resisting the change despite recognizing its potential benefits.Originality/valueThis paper contributes to the change management literature by examining how physician resistance can have a negative impact on healthcare organizations during a precipitous technology change. The study also provides a unique understanding of how technology resistance can disrupt an organizational change process.

 Given increased consumer interest in green products and practices, this paper will focus on examining sustainable practices within the shipping, packaging and delivery supply chain.  “Green” shipping, packaging, and delivery focuses on transporting products from providers (e.g., manufacturers, retailers) to customers to reduce CO2 emissions, landfill waste, and overall negative impact to the environment. For example, (a) the use of electric powered vehicles in the delivery supply chain has the potential to reduce CO2 emissions; and (b) using recycled cardboard or materials that are biodegradable would reduce landfill waste. This paper examines the potential benefits of these green practices in the delivery supply chain.  Our results are that a case can be made for use of electric-power vehicles and their potential benefit to reduce CO2 emissions in an environment where power generation in non-coal based.  In other words, using electric cars in all delivery supply chains isn’t always a solution; firms must recognize the significant impact of the power generation process in their operating area.   From a packaging perspective, there is documented consumer interest on using environmentally friendly material for consumer products.  However, depending upon the market segment size, this might not always result in better financial outcomes for companies.
