Physical activity could improve the muscle fitness of youth, but the systematic analysis of physical activity elements and muscle fitness was limited. This systematic review and meta-analysis aim to explore the influence of physical activity elements on muscle fitness in children and adolescents. We analyzed literature in Embase, EBSCO, Web of Science, and PubMed databases from January 2000 to September 2020. Only randomized controlled studies with an active control group, which examined at least 1 muscle fitness evaluation index in individuals aged 5–18 years were included. Articles were evaluated using the Jaded scale. Weighted-mean standardized mean differences (SMDs) were calculated using random-effects models. Twenty-one studies and 2267 subjects were included. Physical activity had moderate effects on improving muscle fitness (SMD: 0.58–0.96, p < 0.05). Physical activity element subgroup analysis showed that high-intensity (SMD 0.68–0.99, p < 0.05) physical activity < 3 times/week (SMD 0.68–0.99, p < 0.05), and < 60 min/session (SMD 0.66–0.76, p < 0.01) effectively improved muscle fitness. Resistance training of ≥ 3 sets/session (SMD 0.93–2.90, p < 0.01) and < 10 repetitions/set (SMD 0.93–1.29, p < 0.05) significantly improved muscle fitness. Low-frequency, high-intensity, and short-duration physical activity more effectively improves muscle fitness in children and adolescents. The major limitation of this meta-analysis was the low quality of included studies. The study was registered in PROSPERO with the registration number CRD42020206963 and was funded mainly by the Ministry of Education of Humanities and Social Science project, China.

Background: Lack of exercise is a prevalent problem in patients receiving dialysis. Although guidelines recommend these patients to undertake suitable exercise, no exercise type or intensity has been suggested, and the effect of exercise on muscle fitness in dialysis patients is not clear. This study investigated the effect of exercise on muscle fitness, including muscle mass, muscle strength, and physical performance, in patients on dialysis. Methods: A systematic review and a meta-analysis of randomized controlled trials (RCTs) were conducted. Five English and 4 Chinese databases were searched from their inception to July 2018. Two independent reviewers searched the different databases, selected trials, conducted bias assessment, and extracted the data. Results: A total of 21 RCTs meeting the inclusion criteria were included in this review. Pooled results demonstrated that resistance training significantly improved leg mass (standard mean difference [SMD] 0.34, 95% CI [0.06–0.62], p= 0.02) whereas aerobic training did not (SMD 0.87, 95% CI [–0.11 to 1.86], p = 0.08). Resistance training increased both grip strength (weighted mean difference [WMD] 4.71 kg, 95% CI [2.42–6.99], p < 0.00001) and knee extension strength (WMD 3.93 kg, 95% CI [0.59–7.28], p = 0.02) significantly. Aerobic training improved grip strength (WMD 7.70 kg, 95% CI [3.35–12.05], p= 0.005) and the time of finishing short version of the sit-to-stand test (STS; WMD –4.69 s, 95% CI [–9.01 to –0.38], p = 0.03) but with insufficient evidence. In the dimension of physical performance, both aerobic training and resistance training have some beneficial effect on improving the score of 6-min walking test (WMD 85.76 m, 95% CI [63.43–108.09], p < 0.00001; WMD 41.92 m, 95% CI [8.06–75.75], p = 0.02, respectively) and median version of STS test (WMD 4.30 repetitions, 95% CI [1.22–7.39], p = 0.006; WMD 2.60 repetitions, 95% CI [0.64–4.56], p = 0.006, respectively). Conclusions: Regular resistance training with a moderate to high intensity may lead to improvement in muscle mass and muscle strength of patients undergoing dialysis, especially for the trained muscles. Both aerobic training and resistance training may help dialysis patients improve physical performance.

AbstractNitrate (NO3−) is an ergogenic nutritional supplement that is widely used to improve physical performance. However, the effectiveness of NO3− supplementation has not been systematically investigated in individuals with different physical fitness levels. The present study analysed whether different fitness levels (non-athletes v. athletes or classification of performance levels), duration of the test used to measure performance (short v. long duration) and the test protocol (time trials v. open-ended tests v. graded-exercise tests) influence the effects of NO3− supplementation on performance. This systematic review and meta-analysis was conducted and reported according to the guidelines outlined in the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) statement. A systematic search of electronic databases, including PubMed, Web of Science, SPORTDiscus and ProQuest, was performed in August 2017. On the basis of the search and inclusion criteria, fifty-four and fifty-three placebo-controlled studies evaluating the effects of NO3− supplementation on performance in humans were included in the systematic review and meta-analysis, respectively. NO3− supplementation was ergogenic in non-athletes (mean effect size (ES) 0·25; 95 % CI 0·11, 0·38), particularly in evaluations of performance using long-duration open-ended tests (ES 0·47; 95 % CI 0·23, 0·71). In contrast, NO3− supplementation did not enhance the performance of athletes (ES 0·04; 95 % CI −0·05, 0·15). After objectively classifying the participants into different performance levels, the frequency of trials showing ergogenic effects in individuals classified at lower levels was higher than that in individuals classified at higher levels. Thus, the present study indicates that dietary NO3− supplementation improves physical performance in non-athletes, particularly during long-duration open-ended tests.
