Purpose: Insomnia and short sleep duration have been associated with increased prevalence, incidence and severity of hypertension. However, the relationship between insomnia and use of different antihypertensive drug classes has not been ascertained yet. Methods: 371 hypertensive patients at their first visit in a tertiary Hypertension Outpatient Unit were enrolled. Insomnia Severity Index (ISI), Beck Depression Inventory (BDI), and State-Trait Anxiety Inventory (STAY-Y2) were administered. Insomnia was defined as ISI>8, depressive symptoms as BDI>10, trait anxiety as STAI -Y2>40. Patients with self-reported sleep apneas or snoring (n=29) or with incomplete data (n=12) were excluded. Results: Data from 330 patients were analyzed (males 51%, mean age 57±13 years, antihypertensive treatment 84%, previous CV events 9%, diabetes 7%, obesity 24%, smoking 13%, hypercholesterolemia 67%). Insomniacs (n=70, 21%) were older than non-insomniacs (60±11 vs 56±13 years, p=0.02); female gender (62 vs 46%, p=0.01), anxiety (68 vs 34%, p<0.0001) and depressive symptoms (30 vs 6%, p<0.0001) were more prevalent in insomniacs. Insomniacs were treated with higher number of antihypertensive drugs (1.8±1.0 vs 1.5±1.0, p=0.04) and more frequently with angiotensin-receptor-blockers (ARBs, 49 vs 31%, p=0.009) and diuretics (50 vs 28%, p=0.0007), whereas the use of other drug classes was similar. In a multiple logistic regression analysis, adjusted for cardiovascular and psychiatric variables, ARBs use (OR 2.4, CL95% 1.1-5.2), depressive symptoms (OR 3.2, CL95%1.2-8.7) and anxiety (OR 2.9, CL95%1.4-6.1) were associated with a higher probability of insomnia. Conclusions: This cross-sectional analysis suggests that ARBs use may be associated with insomnia in a cohort of hypertensive patients

Aim: The study sought to determine whether there is any relationship between plasma homocysteine and blood pressure levels in Nigerians with essential hypertension. Method: It was a cross-sectional analytical study done on 120 randomly selected hypertensive patients and 120 normal healthy controls seen at the large Conference hall of the Ahmadu Bello University (ABU) Medical Centre, Zaria as well as the ABU Teaching Hospital, Zaria, Northern-Nigeria. Pearson’s Correlation and Binary Logistic Regression analysis determined the rela- tionship between homocysteine and hypertension. Results: Hyperhomocysteinaemia found in the hypertensive patients (22.8 ± 6.6 µmol/L) differed significantly (p<0.001) from controls (10.9 ± 2.8 µmol/L) with significant (p<0.001), blood pressure difference between both groups. Homocysteine signifi- cantly positively correlated with systolic (r = 0.51, p<0.001) and diastolic (r = 0.47, p<0.001) blood pressures in hypertensive subjects. The relation of plasma hcy to hypertension was statistically significant for SBP; OR: 1.08 (95% CI, 1.05-1.11) and DBP; OR: 1.08 (95% CI, 1.03-1.13) in the unadjusted model. When adjusted for confounding variables, hcy was significantly related to SBP; OR: 1.1 (95% CI, 1.04-1.18) but not DBP (p=0.25; OR: 1.06 (95 % CI, 0.96-1.18). The mean plasma folate level was high (115.2 ± 48.0 ng/mL) in the hypertensive subjects. The hyperhomocysteinaemic subjects showed a 2.8 times Odds of developing hypertension. Conclusion: This study showed higher mean plasma homocysteine levels in hypertensives than controls not accounted for by sub-optimal folate levels. Hyperhomocysteinaemia showed a positive relationship to systolic hypertension after adjusting for confounders. Objective To investigate the effects of Shaolin Ba Duan Jin on patients with hypertension. Methods In the hypertensive patients detected in the national physical fitness test of public officials in Guiyang City, 40 patients with hypertension were selected. The experiment was started after Shaolin Ba Duan Jin has been practiced for seven days and the subjects had learned it. Experimental method: Frequency of subjects practicing Ba Duan Jin is 7 days a week, subjects practice once a day, practicing time is 6:00-7:30 am or 18:00 -19:30 pm, practicing lasts 1.5 hours each time (practicing has 3 groups, 14 minutes in each group;subjects rest 2 minutes between groups,;preparing part is 8 minutes;ending part is 5 minutes), record the blood pressure of the subjects before and after each experiment, and fill in the quality of life scale (WHOQOL-BREF scale) regularly for a period of 12 weeks. Questionnaire method: Fill in the quality of life scale every three weeks and measure heart rate and blood pressure before and after each exercise. Mathematical statistics: The paired sample t test was used to analyze the changes in blood pressure before and after the 12-week experiment. Results 1) The systolic and diastolic blood pressures of the subjects were significantly lower than before the experiment (P <0.01). 2) The WHOQOL-BREF scale after 12 weeks has improved significantly in all areas than before the experiment. Conclusions 1) Martial arts Ba Duan Jin has a good influence on the circulatory system of hypertensive patients. The performance is that the heart rate is slow and the systolic and diastolic blood pressures have a downward trend. 2) Martial arts Ba Duan Jin has significant improvement in the physiology, psychology, social relations, environment and other fields .
