Aim: This case report describes the use of a surgical guide in a crown-lengthening procedure to improve the aesthetics of the anterior interdental papillae in an adult female patient. Methodology: A 40-year-old female underwent a crown-lengthening procedure with guidance provided by a surgical template to correct unsatisfactory aesthetics in the anterior gingiva and prosthesis. Results: The patient’s functional and aesthetic expectations were met successfully with interdisciplinary treatment, including surgical crown lengthening and the placement of all-ceramic restorations. Conclusions: This case showed that satisfactory anterior aesthetics can be achieved by comprehensive examination, careful planning of treatment employing a surgical guide, and prosthodontist–periodontist teamwork, even in the complete absence of interdental papillae.  How to cite this article: Kılıç D, Alkan BA, Kılıç K. Crown lengthening is a procedure widely used in periodontal practice, for esthetic procedures and also for pre-prosthetic ones; situation that is present with great frequency in the daily practice in periodontics. It is very important the knowledge of the anatomic structures of the periodontal anatomy in order to do a proper surgical procedure, preceded by a deep clinical and radiological exam and a pertinent treatment prosthetic plan. It is also important that the clinician knows all the surgical techniques, because not all the procedures need the same approach and there are certain anatomic and surgical characteristics that must be taken in concern at the moment of planning a surgical crown lengthening. In this article we present a clinical case where it is integrated the periodontal and prosthetic discipline.

Digital flow has become a part of currently practiced dentistry. Virtual planning ensures predictable aesthetic and functional rehabilitation, painless postoperative recovery, and better communication with patients, thus meeting their expectations. The purpose of this case report is to demonstrate the digital planning for the correction of a gummy smile with a personalized preparation using a piezoelectric surgical guide (PerioGuide) for gingival contouring and flapless osteotomy. The guide was designed using Nemo Studio software, based on the patient’s facial aesthetic analysis, through photos, videos, and facial scanning. These images were aligned with the scan and placed over the cone beam computed gingival tomography for prediction of results, based not only on the distance from the cementoenamel junction to the bone crest but also on the best gingival margin contour according to virtual aesthetic planning. Digital planning, combined with the use of a piezoelectric device, allows for a flapless guided surgical technique for gingival contouring and osteotomy. As a result, the surgical procedure is safer, faster, and more predictable with better postoperative outcomes.

A healthy and beautiful smile can affect appearance and confidence. One of the aesthetic problems in dentistry that is often complained of by patients is excessive gingival display (gummy smile). The excessive gingival display can be caused by several factors, one of which is altered passive eruption (APE). One of the treatments to correct gummy smile related to APE is crown lengthening. Crown lengthening can be with bone reduction (gingivectomy with bone reduction) or without bone reduction (gingivectomy). Crown Lengthening with bone reduction is a surgical procedure that aims to maintain the dentogingival complex and to improve smile aesthetics. The purpose of this case report is to determine the crown lengthening with bone reduction (gingivectomy with bone reduction) procedure as a gummy smile treatment related to APE .A23-year-old female patient, came to Dental Hospital of Universitas Airlangga with complaints of her upper gum which not in the same length and the teeth looked short, she considered her smile was less aesthetic. After conducting analyses relating to aesthetics and periodontal tissue, crown lengthening with bone reduction was chosen for this patient treatment. The treatment results are quite good, visible gingival margins that matched the gingival zenith and improved patient's smile profile. APE as the etiology of patient's gummy smile can be corrected. There are no post-surgical complications such as excessive pain and infection. A proper diagnosis, treatment plan, and good techniques can produce a harmonious smile on the patient.
