Aesthetic Crown Lengthening for Gummy Smile Treatment Related to Altered Passive Eruption: A Case Report

A healthy and beautiful smile can affect appearance and confidence. One of the aesthetic problems in dentistry that is often complained of by patients is excessive gingival display (gummy smile). The excessive gingival display can be caused by several factors, one of which is altered passive eruption (APE). One of the treatments to correct gummy smile related to APE is crown lengthening. Crown lengthening can be with bone reduction (gingivectomy with bone reduction) or without bone reduction (gingivectomy). Crown Lengthening with bone reduction is a surgical procedure that aims to maintain the dentogingival complex and to improve smile aesthetics. The purpose of this case report is to determine the crown lengthening with bone reduction (gingivectomy with bone reduction) procedure as a gummy smile treatment related to APE .A23-year-old female patient, came to Dental Hospital of Universitas Airlangga with complaints of her upper gum which not in the same length and the teeth looked short, she considered her smile was less aesthetic. After conducting analyses relating to aesthetics and periodontal tissue, crown lengthening with bone reduction was chosen for this patient treatment. The treatment results are quite good, visible gingival margins that matched the gingival zenith and improved patient's smile profile. APE as the etiology of patient's gummy smile can be corrected. There are no post-surgical complications such as excessive pain and infection. A proper diagnosis, treatment plan, and good techniques can produce a harmonious smile on the patient.

An attractive smile enhances the appearance and acceptance of an individual in society. Gum exposure more than 3 mm is generally considered unattractive and known as a gummy smile, which is usually considered an aesthetic problem. At present, patients have a greater desire for more aesthetic results that may influence the planning of dental treatments. This case report aimed to describe the surgical sequence of aesthetic crown lengthening to improve smile profile and eliminate gummy smile. We reported a 21-year-old non-smoking woman with no pertinent medical history who presented with a chief complaint of an unattractive smile due to excessive gingival display. The gingival display in the smile was 5 mm, and the width to height ratio of the central incisor was 121%. Neither periodontal problems nor teeth mobility was detected. Assessment for the condition was excessive gingival display due to altered passive eruption. The overall prognosis for this case was good. The primary treatment plan proposed to the patient was an aesthetic crown lengthening. Altered passive eruption class I subtype A was a case conclusion, and aesthetic crown lengthening with gingivectomy without osseous reduction was the selected treatment. In conclusion, aesthetic crown lengthening should be considered as a surgical component of aesthetic therapy to improve smile profile and eliminate gummy smile.

The aesthetic treatment for anterior teeth requires a series of clinical and technical evaluations to obtain a predictable result, following a well-defined operating sequence. Today, the clinical–technical team can use different digital tools in the different steps of the workflow. A preventive assessment, the knowledge of limits, and the possibilities of surgical and prosthetic procedures allow to use these devices. Sharing goals of the treatment with the patient according to their expectations and needs is the key point of the treatment plan. Setting a defined treatment plan avoids invasive procedures. In this clinical case report, a 27-year-old patient affected by Class II div 2 malocclusion with deep bite was treated with a full digital workflow. Previewing the aesthetics was through dedicated software, which shows operators and patients the objectives of the therapy and guides the dental technician in the first phase of the work. The use of digital flows in the prosthetic phases reduces the working time. In this case, it shows the impact of a digital workflow on peri-prosthetic therapy for the aesthetic rehabilitation of the upper central incisors in a young adult.

Peg shape is one of the abnormalities of dental anatomy, i.e. tooth size is smaller than average and tapered shape. These peg-shaped teeth are often found in the maxillary lateral incisors and third molars. The form on the lateral incisor often causes aesthetic problems because of its anterior position. One of the treatments for peg-shaped teeth is Veneer. Case report: 26 years old woman had a peg-shaped shape on both lateral incisors. Indirect veneer aesthetic treatment was performed using Ceramage to correct the form and cover the diastema to achieve better aesthetics. Wax-up is done to see the suitability of the shape and proportion of the Veneer. Patients get a visual picture of the expected treatment results. The treatment results in Ceramage gave an excellent condition in improving the shape of the Peg-shaped incisor. The veneer colour resembles natural teeth, and the patient is satisfied with the treatment results. Ceramage can be used as a treatment option for anterior teeth with deformities. Ceramage Veneer is an alternative that can be done to improve the aesthetic appearance at a more economical cost for anterior teeth.

Background: Metal frame denture design is common used for partially dentate patient, one of the problem was a metal appearance from wrought wire or a metal casting clasp as retainer, which is not really aesthetic. Telescopic overdenture with its double crown will be the best solution for those cases. Purpose: The purpose of this case report was to reported telescopic overdenture design and fabrication where the aesthetic was the main consideration. Case: Sixty years old female patient, a businesswoman, came to Dental Hospital of Universitas Airlangga, complained about her upper partially dentate and 6 years lower fixed partial denture, which is not comfortable to wear and her upper anterior teeth doesn’t look quite good. She wanted to make crowns and removable denture. Case management: After the diagnosis were made, the pretreatment plan were remove crown for the upper jaw, remove the bridges on lower jaw, and transitional dentures were made to maintain the VDO. The crowns and veneer were fabricated and inserted while the inner copings were tried up on prepared teeth. After the the dentures were produced, the inner copings were luted to the abutment teeth, and then the dentures were inserted using FGP (Friction GriP) cement for the friction effect. Discussion: Telescopic overdenture is consists of a double crown system known as “the telescopic”, the procedure involves fitting the remaining natural teeth with inner metal crowns, followed by outer crowns as part of an over denture that can be removed by the patient. Conclusion: This technique ensures that telescopic overdentures give natural aesthetic result.

Clinical Relevance Combined techniques to expose circumferential tooth structure associated with subsequent restoration may represent a valid option in many situations. This case provides an example of the successful management of the anterior tooth rehabilitation combining root displacement and crown lengthening. SUMMARY Rehabilitation of an extensively compromised single anterior tooth represents an intriguing challenge for dentists, particularly when the rehabilitation involves esthetic, psychosocial, and functional requirements. The success in rehabilitating a patient with a conservative approach depends on a critical evaluation of the remaining structures, precise treatment plan, systematic treatment strategies, and patient compliance. In this case, the patient’s chief complaint was the undesirable appearance of the maxillary right lateral, caused by the displacement of the post and crown. Clinical examination revealed a remaining tooth with coronal fracture, severe loss of tissue due to caries, and absence of ferrule effect. Radiographic examination revealed that the fracture margin was located subgingival. This case report describes a single-tooth rehabilitation involving a combination of root displacement via orthodontic extrusion and crown lengthening. The rehabilitation was followed by post-and-core restoration using a prefabricated glass fiber post associated with a disilicate lithium crown. The clinical decision making and combined effect of both treatment strategies are explained in this report. The treatment required three months, including recovery times after surgery and the placement of the provisional crown. The patient was esthetically and functionally satisfied with the restoration. Patient follow-up examination was performed 24 months after the treatment. This clinical report contributes to the clinical practice and exemplifies the possibility of rehabilitating the natural tooth using combined techniques, which may offer particular advantages regarding prognosis and invested efforts.

O sorriso gengival apresenta-se como uma exposição excessiva do tecido gengival, a qual influencia diretamente na estética do sorriso. O sorriso gengival apresenta uma variabilidade de fatores causais, dentre elas podemos mencionar a erupção passiva alterada, em que uma grande parte da coroa anatômica permanece recoberta pelo tecido gengival. Dessa forma, este estudo teve como objetivo relatar o caso de tratamento do sorriso gengival com procedimentos de cirurgia plástica periodontal. Paciente do gênero feminino, 20 anos de idade, compareceu ao atendimento da clínica de periodontia do Unifunec, queixando-se de dentes pequenos e excesso de gengiva ao sorrir. Após avaliação clínica e radiográfica, constatou-se que a margem gengival estava posicionada coronalmente, recobrindo parte da coroa clínica dos dentes e a crista óssea alveolar localizava-se próxima à junção cemento- esmalte, sendo que o diagnóstico definitivo foi de sorriso gengival, devido à erupção passiva alterada associada à erupção ativa alterada do tipo 1. A paciente foi submetida, posteriormente, a procedimentos de cirurgia para aumento de coroa clínica por gengivoplastia associada à osteotomia, com intuito de obter regularização dos tamanhos das coroas anatômicas e correta acomodação dos tecidos supracrestais dos dentes 15 a 25. Após um ano de acompanhamento, observou-se estabilidade da estética e completa satisfação da paciente. Podemos concluir que a cirurgia de gengivoplastia associada à osteotomia no tratamento do sorriso gengival decorrente da erupção passiva alterada com a erupção ativa alterada tipo 1 mostrou-se uma técnica previsível com alto grau de estabilidade das dimensões teciduais e satisfação estética por parte da paciente.   CLINICAL CROWN LENGTHENING FOR GUMMY SMILE CORRECTION: CLINICAL CASE REPORT   ABSTRACT The gummy smile is presented as an excessive exposition of the gum tissue, which directly influences the smile aesthetics. The gummy smile presents a number of causal factors, among them we can mention altered passive eruption, where a great portion of the anatomical crown remains covered by gum tissue. Accordingly, this paper aims to report a case of gummy smile treatment under periodontal plastic surgical procedures. A female patient, 20 years old, attended the periodontics care clinic at Unifunec, complaining of small teeth and excessive gum when smiling. After clinical and radiographic evaluations, it was found that the gingival margin was coronally positioned, covering part of the teeth crown and, the alveolar bone crest was positioned next to cementoenamel junction, leading to a final diagnosis of gummy smile, due to altered passive eruption type 1. Subsequently, the patient underwent gingivoplasty associated with osteotomy, crown lengthening surgical procedures, with a view to equalizing the anatomical crown sizes and the correct teeth supracrestal tissue positioning 15 to 25. After a one year follow-up period, it was observed the aesthetic stability and the patient was completely satisfied. We may conclude that gingivoplasty surgery associated with osteotomy for treating gummy smile caused by altered passive eruption with active eruption type 1 resulted in a predictable outcome with a high level of stability for tissue dimensions and the patient’s aesthetic satisfaction.   Keywords: Osteotomy. The aim of dental treatment is to restore function and improve oral health condition. It is an advantage if the treatment conducted can also improve aesthetic. However, current trend of patient’s demand for aesthetic treatment is challenging. This alarming scenario sometimes does not comply with Shariah and Islamic jurisprudence. Crown lengthening is a procedure to facilitate restoration in certain clinical conditions simultaneously improves the aesthetic in anterior region.  It involves manipulation of gingiva which requires removal of soft tissue and/or hard tissue. This paper was aimed to discuss the management of missing teeth with crown lengthening surgery from Islamic perspective. In the selected case, a lady requested to replace her denture with a fixed prosthesis that requires surgery. Initially, the surgical procedure was only planned on the right anterior region. However, during the preliminary stage of determining the treatment outcome; crown lengthening on both sides is advice to harmonize the gingival line. The final decision was to carry out surgical procedure on the site of complaint and crown lengthening surgery on both sides. As expected, the outcome of the treatment significantly improved the aesthetic of the patient. This study employs mainly the comparative case study approach where the detail description of the selected case is mapped to the Shariah principles and guidelines related to aesthetic treatment. The selected Shariah principles and guidelines are based on the commonly accepted school of law.  In conclusion, this study found that the treatment proposed by the specialist that is based on function first, followed by correction of aesthetics is in line with the guidelines and principles of Shariah as well as legal maxim that falls under acts are judged by their goals and purpose (al-’Umur-bimaqasidiha). Therefore, basic knowledge on Islamic principle is favourable for dentist and patient in deciding the final treatment plan.

Latar belakang. Kehilangan gigi anterior rahang atas mengakibatkan gangguan fungsi fonetik dan estetik. Gangguan fungsi estetik menyebabkan pasie menjadi rendah diri. Kondisi ini dapat diatasi oleh dokter gigi, salah satunya dengan pembuatan cantilever bridge. Tujuan. Penulisan ini yaitu untuk memberi informasi bahwa pada kasus kehilangan gigi-gigi anterior rahang atas dengan space yang telah menyempit dan malposisi gigi dapat dibuatkan protesa berupa gigi tiruan cekat dengan desain cantilever bridge. Kasus dan perawatan. Laporan kasus ini membahas tentang pasien perempuan umur 39 tahun yang datang ke Rumah Sakit Gigi dan Mulut Prof. Soedomo, dengan keluhan merasa kurang percaya diri karena gigi depan rahang atas hilang sejak 5 tahun yang lalu akibat kecelakaan. Gigi-gigi anterior rahang atas yang masih ada mengalami malposisi akibat pemakaian gigi tiruan sebagian lepasan yang tidak baik. Perawatan yang dilakukan adalah dengan pembuatan cantilever bridge pada gigi 11, 12, 13 dan 21, 22, 23. Kesimpulan. Gangguan fungsi estetik pada gigi anterior rahang atas dapat diatasi dengan pembuatan cantilever bridge. Background. Maxillary anteriortooth loss resulting in impaired function of phonetic and aesthetic. Impaired function of aesthetic cause patients to become self conscious. This condition can be treated by a dentist, one with a cantilever bridge. Purpose. To inform that in case of missing anterior teeth of the upper jaw with a space that has been narrowed, and malposition of teeth can be made prosthesis denture fixed bridge with a cantilever design. Case and treatment. This case report discusses the 39 years old female patient who came to he Dental Hospital Prof. Soedomo, with complaints of feeling less confident due to the maxillary front teeth missing since 5 years ago due to an accident. Anterior teeth of the upper jaw are still experiencing malposition due to the use of removable partial dentures are not good. The treatment is done is by cantilever bridge on teeth 11, 12, 13 and 21, 22, 23. Conclusion. Impaired function of the aesthetic in the maxillary anterior teeth can be solved by a cantilever bridge.
