Case: A 5-year-old boy presented with left elbow pain after a fall. Radiographs revealed a radial head dislocation without ulnar involvement which was treated with closed reduction in the emergency department. Two-week follow-up radiographs revealed a periosteal reaction along the medial epicondyle at the supracondylar region, consistent with a type 1 supracondylar humerus fracture. The elbow was treated with closed reduction and casting for 2 weeks. One year after injury, the patient had full painless range of motion. Conclusion: This case report highlights an injury pattern not previously described in the literature, and no previous recommendations exist regarding treatment. Although rare, radial head dislocation with simultaneous supracondylar humerus fracture can occur in pediatric patients. Our patient obtained a good result without surgical treatment.

<p class="abstract"><strong>Background:</strong> Delayed presentation of pediatric displaced supracondylar humerus fracture is relatively common. Management of such cases have higher incidence of perioperative complications and usually require open reduction and pinning. Open reduction can be done by various approaches, each having its own advantage and disadvantages.</p><p class="abstract"><strong>Methods:</strong> A prospective study was done comprising 20 children with displaced Supracondylar fracture presented 2-14 days of injury, Mean patient age was 6 years. 15 were boys and 5 were girls. Children in whom closed reduction and percutaneous pinning was achieved, vascular injury and more than 2 weeks old fracture cases were excluded. Paratricepital approach was used for Open reduction and pinning for all the cases. The functional outcome was assessed using Flynn criteria.<strong></strong></p><p class="abstract"><strong>Results:</strong> In all cases the fracture had united at complete follow-up and the mean follow-up period was 16 months. The outcome was excellent in 15 (75%), good in 3 (15%), fair in 1 (5%), and poor in 1 (5%) patients. The mean Baumann angle was 76º in the affected elbow and 73º in the normal elbow. Average time for complete union in the current study was 7 weeks. Pin tract infection was seen in 2, stiffness in 2 patients, cubitus varus in 1 patient. No case of compartment syndrome or iatrogenic nerve injury was seen was recorded.</p><p class="abstract"><strong>Conclusions:</strong> Finally, we concluded that triceps sparing paratricepital approach is an easy, simple and safe approach for exposure and internal fixation of supracondylar humeral fractures in children with excellent functional outcome.</p>

Monteggia fractures are uncommon injuries, despite a high incidence of upper limb fractures in teenage children. We describe a case of a type 2 Monteggia variant in an adolescent. This injury pattern consisted of ulnohumeral dislocation in combination with type 2 Monteggia lesion. We discuss the possible injury mechanisms of this rare injury and review available literature.

Proximal humerus fracture is common fracture 4-6% of all fractures (1), third most common non vertebral fracture in elderly >65 years (2), affecting female more than males in ratios of 2:1.Approximately half (51%) of these fractures are displaced, the majority of which involve the surgical neck (77%). (3) Surgical treatment (mainly internal fixation or humeral head replacement) is being increasingly used (4).Management of each fracture is dependent of patient factors, fracture pattern, and complexity.Case scenario:33 years old female not known to has any medical illness, presented to the emergency department with right shoulder pain and inability to move it after motor cycle accident. X-rayshowed proximal humerusfracture dislocation Upon Post-operative follow up patient developed sever inferior humeral head subluxation, With intact axillary nerve sensation and motor exam.Discussion:proximal humerus fracture is common, fracture pattern and type, with patient factors will guide your management.proximal humerus fracture dislocation is more difficult and challenging to surgeon in managing such fracture, with inferior hemeral head subluxation is common can reach to 42% in the radiographic incidence among proximal humerus fracture.
