Bone conduction is an efficient pathway of sound transmission which can be harnessed to provide hearing amplification. Bone conduction hearing devices may be indicated when ear canal pathology precludes the use of a conventional hearing aid, as well as in cases of single-sided deafness. Several different technologies exist which transmit sound via bone conduction. Here, we will review the physiology of bone conduction, the indications for bone conduction amplification, and the specifics of currently available devices.

Objective. To evaluate the cutaneous and the inner ear tolerance of bioactive glass S53P4 when used in the mastoid and epitympanic obliteration for chronic otitis surgery.Material and Methods. Forty-one cases have been included in this prospective study. Cutaneous tolerance was clinically evaluated 1 week, 1 month, and 3 months after surgery with a physical examination of the retroauricular and external auditory canal (EAC) skin and the presence of otalgia; the inner ear tolerance was assessed by bone-conduction hearing threshold 1 day after surgery and by the presence of vertigo or imbalance.Results. All surgeries but 1 were uneventful: all patients maintained the preoperative bone-conduction hearing threshold except for one case in which the round window membrane was opened during the dissection of the cholesteatoma in the hypotympanum and this led to a dead ear. No dizziness or vertigo was reported. Three months after surgery, healing was achieved in all cases with a healthy painless skin. No cases of revision surgery for removal of the granules occurred in this study.Conclusion. The bioactive glass S53P4 is a well-tolerated biomaterial for primary or revision chronic otitis surgery, as shown by the local skin reaction which lasted less than 3 months and by the absence of labyrinthine complications.

The use of potentially ototoxic topical drugs is controversial. Few experimental reports of audiometric data from human subjects exist. The purpose of this study is to determine if a significant difference between bone conduction hearing sensitivity before and after otic drop treatment for children with chronic suppurative otitis media and patent ventilation tubes exists. A statistical analysis of sensorineural threshold status after treatment of chronic suppurative otitis media with otic drops is presented.

Twelve young adult men with normal hearing and no history of ear disease took part in our study of the relation of vibrator surface area and static application force to the vibrator-to-head coupling. For vibrator surface areas of 1.125, 2.25, and 4.5 cm 2 coupled to the forehead under static forces of 150, 300, and 600 gm, monaural thresholds of bone-conduction hearing were determined at frequencies 250, 500, 1000, and 2000 Hz. With surface area constant, threshold improvement was frequency dependent. It decreased with increasing frequency until at 2000 Hz it was minimal. In contrast to this, with force constant, the influence of surface area was observed to begin at 2000 Hz. Preliminary study suggests this influence would extend to 4000 Hz. In view of the respective influence of application force and surface area to bone-conduction hearing, equating vibrator-to-head coupling on the basis of pressure (force per unit area), when there are two or more vibrators, may not represent an adequate control.
