Idoxuridine: an antiviral drug for ocular herpes simplex

Because thymidine and deoxycytidine are essential components of desoxyribonucleic acid (DNA), these and similar substances have been modified to produce antimetabolites for the treatment of cancer, and experimentally against tumour-producing viruses. One modification, idoxuridine (Kerecid - SKF), is offered for the treatment of herpes simplex keratitis, of which the commonest form is dendritic ulcer. The drug does not kill the virus, but it does interfere with its multiplication. Strains of virus resistant to idoxuridine have been observed in experimental studies; whether these are important clinically is not known.

A case of posterior corneal abscess with extension into the anterior chamber, in the presence of an intact corneal epithelium and clear anterior corneal stroma, occurring in a young patient is described. The patient responded to treatment with systemic and topical acyclovir. The diagnosis of presumed herpes simplex keratitis was made. We report this unusual presentation of ocular herpes manifesting as a posterior corneal and anterior chamber abscess. In experimental studies on rats (males, females) at their infantile stage starting from 10 days, a potential delayed toxic effect of the antiviral drug Kagocel on the reproductive system was studied. The drug was administered for 12 days in a therapeutic dose and at a dose 10-fold higher than the therapeutic one. Reproductive safety was estimated after animals reached the reproductive age (2.5 months). It was found out that the drug, when administered in both doses, does not decrease the fertility of animals, does not induce morphological and pathological changes in the sex glands, and does not have toxic effect on the offspring. Obtained data characterize Kagocel as a preparation with a wide reproductive safety profile and show that it can be used in pediatric practice for infants.

Acyclovir (Zovirax; Wellcome) is now widely prescribed for the treatment of herpes infections. In 1984 we hailed acyclovir as an important advance for the “early treatment of primary skin and ocular herpes simplex infections and for varicella-zoster infections requiring systemic treatment”, but warned against using it “indiscriminately in view of the cost, the lack of data on toxicity and the increasing number of acyclovir-resistant herpes viruses being reported.”1 In this article we appraise the place of acyclovir in general practice and so for the treatment of patients likely to be immunocompetent and without severe systemic infection.
