Pseudomyxoma peritonei: the ‘controversial’ disease

Pseudomyxoma peritonei (PMP) is a rare disease that is characterized by a large amount of mucinous ascites with peritoneal and omental implants. The etiology of the disease remains unclear. Histologically, two main categories have been described: disseminated peritoneal adenomucinosis (DPAM) and peritoneal mucinous carcinomatosis (PMCA). It is commonly diagnosed incidentally at laparotomy. Most investigators agree that radical surgical debulking and appendectomy are the cornerstone of treatment, but the optimal management of the disease remains controversial. The role of intraoperative and intraperitoneal chemotherapy has been evaluated by a number of authors. The clinical outcomes vary widely between the benign and the malignant forms and between the different treatment modalities. We discuss the pathology, origin, clinical presentation, diagnosis, treatment, and prognosis of PMP.

The purpose of this study is to assess clinicopathologic features of pseudomyxoma peritonei (PMP) that has ovarian pathology and its relationship with the prognosis. From 1995 to 2004, the clinical records and follow-up data of 35 patients with PMP, which had primarily originated from the ovary, were collected from 11 institutes of gynecologic oncology in Korea and retrospectively analyzed. All patients had ovarian lesion histologically confirmed with PMP. The mean age at diagnosis was 53.7 years (range: 16–82 years). There were 25 (71.4%) patients with disseminated peritoneal adenomucinosis, 5 (14.3%) with peritoneal mucinous carcinomatosis with intermediate group, and 5 (14.3%) with peritoneal mucinous carcinomatosis. The clinical stages at diagnosis were IA in 2 patients, IIIB in 4, IIIC in 23, IV in 1, and unknown in 5. In preoperative tumor markers, the positive rates were 72% (CA125), 47.4% (CA19-9), and 84.6% (CEA). Thirty-four patients underwent surgical staging or cytoreduction, and then 27 patients (77%) received adjuvant chemotherapy that was given to patients in a nonuniform fashion. The 5-year survival for 35 patients was 87%. Survival rate was significantly lower in patients >50 years of age (P= 0.002). Our data showed that age of the patient is the only significant prognostic factor in PMP that has ovarian lesion.

Introduction: Rhinosporidiosis is a chronic mucocutaneous infection caused by Rhinosporidium seeberi, which mainly affects the mucus membranes of the nose, oropharynx, skin, eyes, and genital mucosa. The purpose of this case report was to illustrate the importance of clinicians in understanding the unusual incidence of this fungal infection, clinical presentation, treatment modalities, and recurrence rate. Observations: A clinical case report showed rare occurrence of rhinosporidiosis in the lower third of the face. The lesion was surgically excised by electrocauterization at its base. Commentaries: The patient showed no signs of recurrence even after 3 years of follow-up. Further research should be carried out to analyze the role of genetic predisposition in causing zoonotic infection. Conclusion: Rhinosporidiosis should also be considered as the differential diagnosis when facial swelling is encountered.

Aims and background Pseudomyxoma peritonei is a rare disease characterized by a complete redistribution of mucin within the peritoneal cavity. It can be classified into three histologic groups: disseminated peritoneal adenomucinosis, peritoneal mucinous carcinomatosis, and an intermediate group. The aim of the present study was to evaluate the feasibility of cytoreductive surgery requiring peritonectomy procedures associated with intraperitoneal hyperthermic perfusion, a technique that combines hyperthermia and high drug doses administered locally. Methods Twenty-seven patients with pseudomyxoma peritonei (19 males and 8 females) were enrolled in a phase II clinical trial. Twenty-two cases underwent cytoreductive surgery plus intraperitoneal hyperthermic perfusion, and 6 received de-bulking surgery only. One patient was operated on twice for disease recurrence. All patients with peritoneal mucinous carcinomatosis presented serous ascites, whereas all but one patient with disseminated peritoneal adenomucinosis or in the intermediate group presented mucinous ascites. Cytoreductive surgery was performed with peritonectomy procedures. The closed abdomen technique was adopted for intraperitoneal hyperthermic perfusion using a preheated poly-saline perfusate containing cisplatin (25 mg/m2/L) plus mitomycin-C (3.3 mg/m2/L) through a heart-lung pump at a mean flow of 600 mL/min for 60 mins from the hyperthermic phase (42.5 °C). Results All but one of the patients with disseminated peritoneal adenomucinosis and 2 of the 3 patients in the intermediate group were optimally cytoreduced. Patients with serous ascites (all patients with peritoneal mucinous carcinomatosis and 1 patient with disseminated peritoneal adenomucinosis) were considered ineligible for treatment because of tumor diffusion. The morbidity rate was 22%. There was one case of treatment-related mortality 30 days after treatment. Conclusions The following conclusions can be drawn from this phase II clinical trial: 1) patients with pseudomyxoma peritonei originating from undifferentiated mucinous adenocarcinoma (peritoneal mucinous carcinomatosis), with complete distribution into the peritoneal cavity, are not eligible for the cytoreductive surgery plus intraperitoneal hyperthermic perfusion technique; 2) the presence of serous ascites would seem to exclude patients from the treatment; 3) cytoreductive surgery associated with intraperitoneal hyperthermic perfusion is the most suitable approach for patients with disseminated peritoneal adenomucinosis and in the intermediate group.

The background of this research is the fact that the management of improving teacher‟s professionalism is still not effective. The role of teacher as planner, implementer, and developer of curriculum is also not optimal. Management of teachers is needed because teachers have a significant role in the success of learning process. This research was aimed at knowing Guardian Angel‟s concept of teacher management suggested by Munif Chatib and how it develops according to him. The implementation of Guardian Angel model of teacher management was presented through a testimony and direct interviews with Guardian Angel‟s trainees. This research used qualitative approach by studying Munif Chatib‟s books and direct interviews. Data were analyzed with content analysis, which included: (1) literary study of Munif Chatib‟s books, (2) description of Guardian Angel model of teacher management by Munif Chatib. This research found that (1) the concept of Guardian Angel teacher management uses humanistic approach, especially in regard to schedules of consultation, teaching strategies, and assessment through four kinds of teacher reports, including reports of morals, creativity, lesson plan, and student‟s learning results; (2) the basis used is the cornerstone of the scientific human resources and philosophical foundation that teachers carry out the work of teaching, which includes planning, teaching, evaluating, and learning. The first three points are teacher‟s obligations, while the last is their right. Penelitian ini dilatarbelakangi adanya keprihatinan manajemenpeningkatan profesionalitas guru yang belum berjalan efektif. Peranan guru sebagai perencana, pelaksana dan pengembang kurikulum yang belum optimal. Manajemen guru diperlukan mengingat bahwa peran guru amat signifikan bagi setiap keberhasilan proses pembelajaran.Penelitian ini mengungkap tentang manajemen guru Guardian Angel menurut Munif Chatib. Penelitian ini bertujuan untuk mengetahui konsep manajemen guru model Guardian Angel menurut Munif Chatib, dan berkembangannya Guardian Angel dalam pemikiran Munif Chatib. Implementasi manajemen guru model Guardian Angel dihadirkan melalui testimoni dan wawancara langsung dengan peserta pelatihan Guardian Angel. Penelitian ini menggunakan pendekatan kualitatif, dengan melakukan kajian terhadap buku - buku karya Munif Chatib dan wawancara langsung. Teknik anaisis data content analysis. Analisis meliputi: (1) kajian pustaka buku-buku karya Munif Chatib, (2) mendeskripsikan manajemen guru model Guardian Angel dalam pemikiran Munif Chatib.Hasil penelitian menunjukkan bahwa: (1) konsep model manajemen guru Guardian Angel menggunakan pola pendekatan manajemen humanis, terutama dalam jadwal konsultasi, strategi mengajar dan penilaian melalui empat rapor guru, yaitu rapor akhlak, rapor kreativitas, rapor lesson plan dan rapor hasil belajar siswa (2) landasan yang digunakan adalah landasan keilmuan sumber daya manusia dan landasan filosofi bahwa profesi guru mengemban pekerjaan manajemen, yaitu perencanaan , mengajar dan mengevaluasi dan belajar. Tiga hal pertama difahami sebagai kewajiban, sedangkan belajar dimaknai sebagai hak bagi seorang guru (3) Guardian Angel sebagai manajemen quality control yang meliputi: lesson plan, konsultasi, observasi dan umpan balik.
