Abstract Background Cervical radiculopathy is a pathology of the cervical nerve root and mostly caused by a cervical disk herniation leading to chronic pain and disability. Objectives This study was conducted to show the effect of the combined application of intermittent cervical traction with median nerve mobilization on flexor carpi radialis (FCR) muscle H-reflex latency of median nerve in patients with unilateral cervical radiculopathy due to disk lesion in a pre-post design. Methods Fifteen patients (10 females and 5 males) with a mean age of 38.07 ± 5.85 years received simultaneous application of intermittent cervical traction and median nerve mobilization. Six sessions were given every other day for 2 weeks. Also, patients perormed chin in exercises and upper back extension with scapular retraction. FCR H-reflex latency was measured pre- and post-treatment. Results Statistical analysis showed that there was a significant reduction of H-reflex latency at post-treatment in comparison to pretreatment (t = 5.447, p value = 0.0001*). Conclusion Simultaneous application of intermittent cervical traction and median nerve mobilization are effective in improving FCR H-reflex latency in patients with unilateral cervical radiculopathy.

The effects of electrical stimulation of median nerve with a continuous theta burst pattern (EcTBS) on the spinal H-reflex were studied. Different intensities and durations of EcTBS were given to the median nerve to 11 healthy individuals. The amplitude ratio of the H-reflex to maximum M wave (H/M ratio), corticospinal excitability and inhibition measured using motor evoked potentials (MEPs), short-interval intracortical inhibition and facilitation (SICI/ICF), spinal reciprocal inhibition (RI), and postactivation depression (PAD) were measured before and after EcTBS. In result, the H/M ratio was reduced followed by EcTBS at 90% H-reflex threshold, and the effect lasted longer after 1200 pulses than after 600 pulses of EcTBS. In contrast, EcTBS at 110% threshold facilitated the H/M ratio, while at 80% threshold it had no effect. Maximum M wave, MEPs, SICI/ICF, RI, and PAD all remained unchanged after EcTBS. In conclusion, EcTBS produced lasting effects purely on the H-reflex, probably, through effects on postsynaptic plasticity. The effect of EcTBS depends on the intensity and duration of stimulation. EcTBS is beneficial to research on mechanisms of human plasticity. Moreover, its ability to modulate spinal excitability is expected to have therapeutic benefits on neurological disorders involving spinal cord dysfunction.

Our aim was to evaluate sensitivity and specificity of conduction studies parameters for prognosis and differential diag nosis in children with acute motor axonal neuropathy (AMAN) & acute inflammatory demyelinating polyneuropathy (AIDP). Methods. 40 children were included: 20 healthy controls (7-14 years) and 20 patients (8-15 years) with AIDP or AMAN. All underwent conduction studies on 3-7 day since the clinical symptoms onset. We registered and evaluated motor conduction velocity, compound muscle action potential (CMAP) amplitude of nn. tibialis, peroneus, medianus, ulnaris; sensory conduction velocity & sensory nerve action potential (SNAP) amplitude for nn. medianus, suralis, peroneus superficialis, ulnaris, H-reflex threshold & latency, reactivity of neural conductivity (RNC) in short-term hand ischemia in acute phase (3-14 day since the disease onset) and in early recovery period (15-30 day since the symptoms onset). ROC-analysis was performed. Results. In 95% of the patients with Guillain-Barré syndrome H-reflex was absent. In first 10 days SNAP amplitude of median nerve >8.9 µV, peroneal nerve >3.6 µV, CMAP of peroneal nerve ≤0,4 µV with normal motor conduction velocity indicates AMAN presence. Motor axons of peripheral nerves in children in acute and recovery phase of AIDP are resistant towards ischemia. Prognostic criteria for long period of walk recovery (more than 30 days) in these patients are RNC on 10th minute of local ischemia ≤2.5%, ulnar nerve CMAP amplitude ≤1,1 mV and distal CMAP amplitude from median nerve ≤1.6 mV. Conclusions. Conduction studies may be implemented on all phases of Guillain-Barré syndrome in children for prognosis and differential diagnosis between its axonal and demyelinating forms. H-reflex absence in children in the first 5 days of acute polyneuropathy may serve as additional diagnostic criteria for Guillain-Barré syndrome. RNC parameters may be implemented for the prognosis of the walk period recovery duration.
