Abstract Background Cervical radiculopathy is a pathology of the cervical nerve root and mostly caused by a cervical disk herniation leading to chronic pain and disability. Objectives This study was conducted to show the effect of the combined application of intermittent cervical traction with median nerve mobilization on flexor carpi radialis (FCR) muscle H-reflex latency of median nerve in patients with unilateral cervical radiculopathy due to disk lesion in a pre-post design. Methods Fifteen patients (10 females and 5 males) with a mean age of 38.07 ± 5.85 years received simultaneous application of intermittent cervical traction and median nerve mobilization. Six sessions were given every other day for 2 weeks. Also, patients perormed chin in exercises and upper back extension with scapular retraction. FCR H-reflex latency was measured pre- and post-treatment. Results Statistical analysis showed that there was a significant reduction of H-reflex latency at post-treatment in comparison to pretreatment (t = 5.447, p value = 0.0001*). Conclusion Simultaneous application of intermittent cervical traction and median nerve mobilization are effective in improving FCR H-reflex latency in patients with unilateral cervical radiculopathy.

ABSTRACT Objectives: To investigate the effect and complications after transforaminal injection for cervicobrachialgia caused by cervical disc herniation. Methods: We retrospectively reviewed all patients undergoing fluoroscopy-guided transforaminal injection for radiculopathy caused by cervical disc herniation. During the last seven years, 57 patients (39 female, 18 male, mean age 45.6 years) experiencing cervical radiculopathy underwent cervical foraminal block guided by fluoroscopy by postero-lateral approach. The position of the needle was verified after injection of a small amount of contrast. A glucocorticosteroid was injected after 0.5 ml of 2% lidocaine. Results: The local with the highest prevalence of procedures was C6 root (31 procedures); 14 patients underwent C7 block, 7 had C5 block, and 5 in C4. Eight patients (14%) had complications (3 syncopes, 3 transient hoarseness, one patient had worsening of symptoms and one patient had soft tissue hematoma). In total, 42.1% were asymptomatic after the procedure and therefore did not require surgery after the procedure. Other 57.9% had transient improvement, became asymptomatic for at least 2 months but required surgery due to the recurrence of symptoms. Conclusion: Cervical foraminal block for cervical disc herniation is a safe way to avoid surgery. Some patients still need surgery after the procedure, but the temporary improvement in symptoms gives the patient some relief while awaiting surgery.

Sowing and application of mineral and organic fertilizer is generally done manually in the Sahel, resulting in low precision and delayed application. The objective of this paper is to present a new mechanical planter (Gangaria) for the combined application of seeds and soil amendments (mineral fertilizer, compost, etc.), and to assess the effects of using this planter in pearl millet on labor use, yield and economic return. The labor study showed that the mechanized application of seeds and compost reduced time use by a factor of more than six. The on-station experiments were completely randomized experiments with six replications and six treatments: T0 (control), T1 (0.3 g NPK hill−1), T2 (25 g compost hill−1), T3 (25 g compost + 0.3 g NPK hill−1), T4 (50 g compost hill−1) and T5 (50 g compost + 0.3 g NPK hill−1). Treatments T1 to T5 were sown by the planter with seeds that were primed in combination with coating of seeds with a fungicide/insecticide. The treatment T5 increased grain yield and economic return compared to the control by 113% and 106%, respectively. The advantages for farmers using this approach of agricultural intensification are timelier sowing of dryland cereal crops, easy application of organic fertilizer and more precise delivery of input, thereby making this cropping system more productive and less vulnerable to drought.

Cervical radiculopathy Syndrome (CRS) is a common neuro-musculo-skeletal disorder causing pain and disability. Manual therapy interventions including cervical traction with other treatment modalities have been advocated to decrease pain and disability caused by cervical radiculopathy (CR). Al-Qudah & AL-Jazzazi (2021) conducted a new method of Spinal Decompression Therapy (SDT) in patients with Chronic Lumbar Disc Herniation (CLDH) which includes Combination of Lumbar Traction With Cervical Traction (CLTCT) as one intervention. Despite of that this new method clinically reduces pain and disability more effectively than the conventional types of Traction, CLTCT method was not previously used in CR patients nor with Cervical Disc Herniation (CDH). The clinical effectiveness of this new method with other treatment modalities in patients with CRS was not approved yet.  OBJECTIVE: The purpose of the presented work is to identify the effectiveness of rehabilitative program on patients with Cervical Radiculopathy, by (15) sessions for (4) Weeks. The suggested Rehabilitative program consisted of: 1.Supine Soft Full Back, Shoulders and Neck Cupping Massage (CM) for (10) minutes, 2.CLTCT: Combined Lumbar Traction with Cervical Traction as one intervention for (20) minutes, 3.Gradual Therapeutic Exercise Package of Neck Stretching and Strengthening Exercise for approximately (15) minutes. METHODS: In this study, Five outdoor male patients had accepted to participate  and were randomly chosen from Al-Karak Governmental Hospital, All subjects applied the proposed rehabilitative program. The results were analyzed using the SPSS system. RESULTS: indicates that there was statistically significant difference between the pre and post measurements in favor of the post measurements in terms of Pain, Disability. CONCLUSIONS: The present study demonstrated that the use of proposed rehabilitative program has a positive effect on patients with Cervical Radiculopathy.

Objective. To estimate the efficacy of prophylactic method for regional infection while performance of laparoscopic cholecystectomy, using simultaneous application of container for removal of the specimen and a two-ring wound protector of the wound. In accordance to the literature data, the infection rate in laparoscopic cholecystectomy conduction constitutes 2.4 - 9.6%. Materials and methods. The work was based on experience of performance of 759 laparoscopic cholecystectomies during the period of 2015 - 2018 yrs. There were performed 679 operations with the second class of microbial contamination, 17 - with the third one and 6 - with the fourth. In 21 patients the conversion into laparotomy was applied. The patients with third and fourth classes of microbial contamination of wounds were divided into two groups: the first - 61 patients, in whom a container was used for the specimen removal, and the second - 19 patients, in whom the method of combined application of container and the wound protector was applied for removal of the specimen. Results. Infection of region of the surgical intervention performance in laparoscopic cholecystectomy was revealed in 41 (5.40%) of 759 patients. In surgical interventions In surgical interventions, referring to second class of microbial contamination , the infection rate in the surgical intervention region have constituted 2,5%, while referring to the third class - 11.76%, and the fourth class - 34.92%. Conversion in laparoscopic cholecystectomy is accompanied with high rate of the wound infection - 38.09%. Conclusion. Application of the method of combined usage of container for removal of preparation and a two-ring protector of the wound in the third and the fourth classes of the wounds while performance of laparoscopic cholecystectomy have permitted to lower the infection rate in region of the surgical intervention performance from 37.70 to 5.26%.
