BACKGROUND: Chronic kidney disease (CKD) is a structural or functional kidney disease for more than 3 months. In predialysis CKD patients, the serum parathyroid hormone levels increase progressively since the early stages of the disease to maintain phosphate homeostasis. Glomerular filtration rate (GFR) has been widely accepted to assess renal function. The GFR assessment is used to determine the CKD stadium. AIM: This study aims to analyze the correlation between GFR and parathyroid hormone levels in predialysis CKD patients undergoing treatment at Sanglah General Hospital Denpasar. METHODS: A cross-sectional observational study was conducted among predialysis CKD patients undergoing treatment at Sanglah General Hospital Denpasar who met the inclusion and exclusion criteria. The inclusion criteria were adult patients (≥18 years) who were accepting to participate in the study. Exclusion criteria were patients with predialysis CKD after thyroidectomy and/or parathyroidectomy and liver disease. The parathyroid hormone levels, blood urea nitrogen, creatinine, and GFR were examined and analyzed by SPSS version 17 for Windows. RESULTS: A total of 77 patients with predialysis CKD in this study obtained a median of e-GFR of 21.09 (4.72–75.80) mL/min/1.73 m2. The median level of parathyroid hormone was 82.07 pg/mL (15.83–716.60 pg/mL). Spearman’s correlation analysis results obtained a strong and significant negative correlation between the e-GFR value and parathyroid hormone levels (r = −0.540; p = 0.000). CONCLUSION: The parathyroid hormone levels assessment could be used as a recommendation in evaluating the CKD progressivity among predialysis adult patients at Sanglah General Hospital, Bali, Indonesia, due to the strong significant correlation.

Aim. To validate the accuracy of estimated glomerular filtration rate (eGFR) equations in Malay population attending our hospital in comparison with radiolabeled measured GFR.Methods. A cross-sectional study recruiting volunteered patients in the outpatient setting. Chromium EDTA (51Cr-EDTA) was used as measured GFR. The predictive capabilities of Cockcroft-Gault equation corrected for body surface area (CGBSA), four-variable Modification of Diet in Renal Disease (4-MDRD), and Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) equations were calculated.Results. A total of 51 subjects were recruited with mean measured GFR 42.04 (17.70–111.10) ml/min/1.73 m2. Estimated GFR based on CGBSA, 4-MDRD, and CKD-EPI were 40.47 (16.52–115.52), 35.90 (14.00–98.00), and 37.24 (14.00–121.00), respectively. Higher accuracy was noted in 4-MDRD equations throughout all GFR groups except for subgroup of GFR ≥ 60 ml/min/1.73 m2where CGBSA was better.Conclusions. The 4-MDRD equation seems to perform better in estimating GFR in Malay CKD patients generally and specifically in the subgroup of GFR < 60 ml/min/1.73 m2and both BMI subgroups.

Objective Carboplatin is a cytotoxic chemotherapy drug developed in the 1980s which is still widely used today across various tumour types. Despite its common application, there remains a significant controversy and practice variation on its unique method of dosing by area under the curve (AUC). One potential reason for this variability stems from the reliance of using an estimated glomerular filtration rate (eGFR) as an extrapolation of the measured GFR (mGFR) which the commonly used Calvert equation was originally validated for. This review takes a novel and collaborative nephro-oncology approach to highlight the historical evolution of carboplatin dosing, methods for estimating GFR and its relative performance in the application of carboplatin dosing for adult patients. Data sources We reviewed all pertinent publications comparing carboplatin AUC-based dosing in adult patients based on the various methods of GFR measurements or estimations in order to provide a comprehensive description of each method’s advantages and risks. Data summary and conclusions The Cockcroft-Gault equation has been widely studied but newer eGFR equations, such as the CKD-Epidemiology Collaboration (CKD-EPI) or Janowitz-Williams equation have outperformed the Cockcroft-Gault in recent studies.
