Abstract Background The aim of this study was to assess the accuracy of virtual planning of computer-guided surgery based on the actual outcomes of clinical dental implant placement. Methods This retrospective study enrolled patients among whom implant treatment was planned using computer-guided surgery with cone beam computed tomography (CBCT). The patients who received implant according to the guide with the flapless and flapped approach were classified as group 1 and 2, respectively, and the others who could not be placed according to the guide were allocated to the drop-out group. The accuracy of implant placement was evaluated with the superimposition of CBCT. Results We analyzed differences in the deviated distance of the entrance point and deviated angulation of the insertion of implant fixtures. With regard to the surgical approach, group 2 exhibited greater accuracy compared to group 1 in deviation distance (2.22 ± 0.88 and 3.18 ± 0.89 mm, respectively, P < 0.001) and angulation (4.27 ± 2.30 and 6.82 ± 2.71°, respectively, P = 0.001). The limitations of guided surgery were discussed while considering the findings from the drop-out group. Conclusions Computer-guided surgery demonstrates greater accuracy in implant placement with the flapless approach. Further research should be conducted to enhance the availability of guides for cases with unfavorable residual bone conditions.

Abstract Objectives The aim of this research was to develop a sensor of approximation by reflectance for guided surgery with dental implants without flap detachment, and verify the effectiveness of this system. Materials and Methods Ten models of total edentulous arches were divided into two groups. Two implants of 3.5 × 11.5 mm (NeoDent) were inserted in each model; in Group 1 (G1), a stereolithographic guide NeoGuide system was used. In Group 2 (G2), the experimental approximation sensor was used for the insertion of the implants. The evaluation of the results was performed by overlapping the virtual planning images with the tomographies of the models of the implants inserted. Results There were no statistically significant differences between the guide and the sensor groups. The averages and standard deviations observed at the angulation of the guide was 4.15 (2.65 degrees) and 5.48 (2.85 degrees) at the sensor. The linear deviations at the cervical level were 0.002 (1.37) and 0.11 (1.47) mm and at the apical level 0.19 (1.28) and 0.21 (1.42) mm, respectively. Conclusions The use of a guide is important for the stabilization of the drills; the greatest challenge is to control the apical position of the implants, especially in highly reabsorbed edges. The experimental sensor can become an auxiliary tool to the stereolithographic guides; however, several difficulties must still be overcome to recommend the use of a sensor.

This study was designed to evaluate the accuracy of a novel computer-designed and selectively laser sintered surgical guide for flapless dental implant placement in the edentulous jaw. Fifty dental implants were placed in 11 patients with at least one totally edentulous jaw. Initially, cone-beam computed tomography (CBCT) was performed in each patient to define the virtual position of the dental implants based on the assessment of bone availability and the proposed dental prosthesis. After virtual planning, 3D surgical guides were printed using selective laser sintering. CBCT was repeated after the surgery, and the pre-and postoperative images were overlapped in the CAD software to compare the planned and actual positions of the dental implants using a one-sample t-test. The mean ± angular standard deviation between the long axes of the planned and final dental implant positions was 4.58 ±2.85 degrees; The linear deviation in the coronal position was  0.87 ± 0.49 mm and  in the apical region of the dental implants was 1.37 ± 0.69 mm. These differences were statistically significant (p &lt;0.001). The proposed modifications reduced the deviations, resulting in an improvement in the technique. We were able to place implants and temporary prostheses using the present protocol, taking into account the differences between the planned and final positions of the dental implants.

With the advancement in the dental implant treatment, many new innovative improvements have been developed in the designs and components of implants. These improvements helps to handle the compromising situations where sometimes the good treatment option can become more challenging. One of the such grueling condition is implant placement in the anterior maxilla where dentist has to fulll both aesthetic and functional demand. Many aesthetic failures of implant have been reported in the literature in the anterior maxilla. One of them is placing implant in sites with deciency of facial bone thickness which sometimes resulted in malposition of implant. This case report discusses about the prosthodontic correction of malpositioned implant in the anterior maxilla by using castable abutment. The use of castable abutment is concluded as successful treatment option for complex misaligned and malposed single dental implant.

The purpose of this review is to examine various assessment methods in order to compare the accuracy between the virtually planned and clinically achieved implant positions. In this review, comparison methods using pre- and post-operative computed topography (CT) data and digital impressions for definitive prosthesis will be described. The method for the displacement and strain for quantification of the error will also be explored. The difference between the planned and the actual implant placement position in guided implant surgery is expressed as an error. Assessing the accuracy of implant-guided surgery can play an important role as positive feedback in order to reduce errors. All of the assessment methods have their own inevitable errors and require careful interpretation in evaluation.

ABSTRACT Since the alveolar process is tissue “dental dependent,” after the extraction of the dental element, this process suffers some degree of atrophy during the healing process, which can be reduced with the installation of immediate implants, aiming to maintain the original bone architecture. The aim of this study was to investigate the influence of the time of implant placement on bone formation around them. Seven dogs were selected and randomly divided into two groups: Group 1, where implants were placed immediately after extraction of two lower premolars without flap elevation, and group 2, where implants were delayed by 4 months after extractions. Each group received 14 implants, and 4 months after the second surgery, the samples were processed and analyzed histomorphometrically. A mean average analysis and the Kruskal–Wallis test (p < 0.05) were performed. The buccal bone–implant contact (BIC) mean average was found larger in immediate implants (42.61%) compared with delayed implants (37.69%). Group 1 had statistically higher outcomes in bone formation and BIC on the buccal bone wall. It was concluded that performing immediate implants with the palatal approach technique and leaving a buccal GAP enables a higher or at least equal rate to BIC and bone area around them, when compared with delayed implants. Actually, the patients and dentists want to do a shorter treatment with satisfactory results, but it is necessary to understand whether different times of implant placement can influence the results and longevity of the treatment. This is the first comprehensive review of the classification, preventative measures, diagnosis, treatment methods, and determination of success criteria of buccal bone plate fenestrations (BPFs) secondary to posterior implant surgeries. The purpose of this review is to present and discuss the current literature from peer-reviewed journals, recent studies, and international implantology guidelines and to provide practitioners with guiding points to identify and understand whether BPFs are complications or accidents of implant surgeries. In addition, this review sets forth a detailed set of criteria for the evaluation and diagnosis of BPFs and for the subsequent classification of BPFs as either complications or accidents of posterior implant surgeries. From the literature analyzed, it is clear that BPFs are disqualified from the class of implant treatment failures because BPFs neither impair nor significantly delay treatment. A comprehensive outline of preventative measures and surgery aids to avoid fenestrating the buccal bone plate during implant placement, and a variety of repair methods are included in this review. Considerations of treatment outcomes and patient sensitivities are also included in this comprehensive review.
