Non-invasive ventilation (NIV) is the treatment of choice for patients symptomatic for respiratory muscle dysfunction. It can normalize gas exchange and provide up to continuous non-invasive ventilator support (CNVS) as an alternative to intubation and tracheotomy. It is usually provided via non-invasive facial interfaces or mouthpieces, but these can be uncomfortable and uncosmetic. The intermittent abdominal pressure ventilator (IAPV) has been used for diurnal ventilatory support since 1938 but has been off the market since about 1990. Now, however, with greater emphasis on non-invasive management, a new IAPV is available. A patient with chronic ventilatory insufficiency post-ischemic cervical myelopathy, dependent on sleep NVS since 2003, developed symptomatic daytime hypercapnia for which he also used diurnal NVS via nasal pillows. However, he preferred not having to use facial interfaces. When not using diurnal NVS he was becoming dyspnoeic. Diurnal use of an IAPV was introduced. Arterial blood gas analysis using the IAPV decreased his blood pH from 7.45 to 7.42, PaCO2 from 58 to 37mmHg, and improved PaO2 from 62 to 92mmHg. At discharge, the patient used the IAPV 8 h/day with improved mood and quality of life. Consequently, he returned to work as a painter.

AbstractTic disorders are common, affecting approximately 0.5 to 1% of children and adolescents. Treatment is required only when symptoms are bothersome or impairing to the patient, so many do not require intervention. However, on occasion tics may cause significant morbidity and are referred to as “malignant.” These malignant tics have resulted in cervical myelopathy, subdural hematoma secondary to head banging, biting of lips leading to infection of oral muscles, self-inflicted eye injuries leading to blindness, skeletal fractures, compressive neuropathies, and vertebral artery dissection. We describe a case of malignant tic disorder, with accompanying video segment, resulting in cervical myelopathy and quadriparesis in a child. We also discuss aggressive management strategies for neurologists to prevent potential lifelong disability. This case emphasizes that these malignant tics must be treated with all due haste to prevent such complications.

The aim of the paper is to conduct a comparative assessment of bowel decompression methods in operations on toxic widespread peritonitis. Materials and Methods. The authors examined 162 patients with widespread peritonitis of various etiologies. Using generally accepted methods, they compared treatment results using one of the three methods of intestinal decompression: cecostomy, nasointestinal intubation, and cecostomy with intubation of the small intestine. Results. It was detected that in case of small intestine intubation through the cecostomy (in comparison with other methods), intra-abdominal pressure returned to normal on the 2nd day, and in case of cecostomy or nasointestinal intubation – on the 5th and 7th day, respectively. Also, decrease in intoxication indicators was observed 2 days earlier than usual. Conclusion. Intubation of the small intestine through the cecostoma provides the most adequate intestinal decompression in patients with advanced toxic peritonitis. Early normalization of intra-abdominal pressure in patients with advanced toxic peritonitis improves treatment outcomes. Keywords: widespread peritonitis, intestinal decompression. Цель – сравнительная оценка способов декомпрессии кишечника при операциях по поводу распространённого перитонита в токсической стадии. Материалы и методы. Наблюдали 162 пациента с распространённым перитонитом различной этиологии. С использованием общепринятых методик сравнивались результаты лечения при применении одного из трёх способов декомпрессии кишечника: цекостомии, назоинтестинальной интубации и наложения цекостомы с интубацией тонкой кишки. Результаты. Установлено, что при использовании интубации тонкого кишечника через цекостому, в сравнении с другими применявшимися нами способами, внутрибрюшное давление нормализовалось на второй день, а при использовании цекостомии или назоинтестинальной интубации – на 5-й и 7-й дни соответственно. Также на 2 сут раньше происходило снижение показателей интоксикации. Выводы. Наиболее адекватную декомпрессию кишечника у пациентов с распространённым перитонитом в токсической стадии обеспечивает интубация тонкого кишечника через цекостому. Ранняя нормализация внутрибрюшного давления у пациентов с распространённым перитонитом в токсической стадии позволяет улучшить результаты лечения.
