Abstract OBJECTIVE A penetrating injury to the brainstem is usually a fatal injury. The number of cases in which the brainstem is traversed during the injury and the patient survives is exceedingly small. CLINICAL PRESENTATION We report a case of an automobile collision in which blunt injury to the face of a 22-year-old man resulted in a fragment of the clivus being impacted posteriorly traversing through the pons in a left-sided through-and-through manner. The striking radiographic images demonstrate a potentially devastating brainstem injury. Surprisingly, the patient was able to follow commands and move his left side on arrival at the hospital. INTERVENTION The patient required a temporary tracheostomy and underwent surgical repair of his facial fractures. His hospital course was complicated by meningitis, and he required a long stay on the rehabilitation service. The patient made a remarkable recovery and became able to converse and walk with assistance. He has been able to live independently. CONCLUSION The striking radiographic images of this rare case illustrate an unusual pathology with an even more unusual outcome.

Isolated duodenal injury following blunt trauma do not occur often. A patient with complete damage to the duodenum presents a challenge to the surgeons. The choice of procedure must be tailored to the nature of the defect and the amount of tissue lost. A 19-years-old male presented with features of bowel perforation following blunt injury abdomen. After initial conservative management, the patient was taken up for laparotomy. A complete transection of duodenum with avulsion of pylorus was found. Primary closure of duodenum and pylorus with omental patch support was done. The patient made an uneventful recovery. The choice of procedure may vary and it depends on the patient condition and surgeon capability. Primary closure of disturbed ends without any gastric diversion procedure is the excellent choice in a young patient with blunt injury without major intraoperative contamination. Primary closure is also a challenging procedure compared to other natural tailoring techniques.

Blunt injury abdomen (BIA) is an ever-increasing problem. Isolated injury to duodenum following BIA is rare (1-4%). It can be a challenge to the surgeon and failure to manage it properly can lead on to devastating results. Blunt duodenal injury can occur in isolation or with pancreatic injury. We report a case of an isolated transection of third part of the duodenum following BIA. Initial clinical changes in isolated duodenal injury may be subtle before life-threatening peritonitis develops. High index of suspicion, knowledge of mechanism of injury, physical examination and proper imaging techniques are the key in early detection of duodenal injury.

AbstrakFraktur penis merupakan kegawatdaruratan urologi yang jarang dan sebagian besarterjadi pada trauma tumpul pada penis yang mengalami ereksi. Fraktur penis sering terjadipada saat hubungan seksual dan jarang terjadi pada trauma tumpul karena benturan. Traumamenyebabkan robekan dan rembesan darah pada tunika albuginea. Intervensi bedah dapatmemberikan hasil penyembuhan yang baik secara fungsional dan kosmetik. Dilaporkankasus pada laki-laki usia 22 tahun yang datang dengan riwayat nyeri dan bengkak pada penissetelah benturan pada stang motor saat kecelakaan lalu lintas satu jam sebelum masuk rumahsakit. Pemeriksaan fisik didapatkan bengkak, hematoma pada batang penis, deformitasberupa angulasi ke kiri disertai nyeri tekan. Pasien masih bisa buang air kecil. Hasil urinalisisdidapatkan eritrosit 60-70/LPB, sedangkan darah rutin dalam batas normal. Kami memutuskanmelakukan eksplorasi, tampak rupture pada corpus cavernosum dan diperbaiki dengan jahitanprimer pada corpus cavernosum yang robek. Pasien dirawat selama 3 hari dan pulang denganfoley-catheter terpasang. Kasus pasien ini adalah salah satu contoh kasus dari fraktur peniskarena trauma tumpul penis secara langsung yang bukan disebabkan karena hubunganseksual.AbstractPenile fracture is a rare urological emergency that occurs almost exclusively due to blunttrauma on erect penis. Penile fracture often occurs during sexual intercourse and is rare in blunttrauma from impact. Trauma caused rupture and hematoma of tunica albuginea. Early surgicalintervention gives good cosmetic and functional outcome. We report the case of 22 -year-oldmale who presented to the emergency department 1 hour following blunt injury of his penisafter traffic accident. In physical examination we found swelling and haematom of penile shaft,angulation to the left and tenderness. Mixturation was normal. There were 60-70 eritrocytes inroutinely urinary examination. There were no abnormality in routinely blood examination. Weperformed an exploration and found rupture at corpus cavernosum. The rupture was repairedwith primary suture. The patient was hospitalized for 3 days and discharged with folley catheterinserted. This patient was an example of penile fracture caused by direct blunt trauma outsidesexual intercourse which is a rare case.

Abstract Background Traumatic injury of the trachea is rare, especially complete transection. Its operative revision requires an interdisciplinary approach. Case Description We hereby present a rare case of complete transection of the trachea by accident. To stabilize the patient and to allow for safe surgery, veno-venous extracorporeal support was initiated via the subclavian artery and the femoral vein. The patient was subsequently operated, and the trachea re-anastomosed with favorable outcome. Conclusion This rare case of an accidental transection of the trachea shows the importance of a good emergency rescue chain and the ability to facilitate interdisciplinary approaches in tertiary hospitals.

Electron microscopy and energy dispersive x-ray microanalysis (EDXA) are found to be very useful tools for identification of etiologic agents in pneumoconiosis or interstitial pulmonary disorders. Pulmonary interstitial fibrosis and granulomatosis are frequently associated with occupational and environmental pollution. Numerous reports of pneumoconiosis in various occupations such as coal and gold miners are presented in the literature. However, there is no known documented case of pulmonary changes in workers in the sandpaper industry. This study reports a rare case of pulmonary granulomatosis containing deposits from abrasives of sandpaper diagnosed by using EDXA.
