We present an uncommon case report of simultaneous pulmonary embolism and aortic saddle embolism. This patient had risk factors for thromboembolic events that included recent surgery for tibia fracture and immobilisation. She underwent emergency bilateral pulmonary artery catheter-directed thrombolysis, followed by bilateral transfemoral embolectomy. The potential benefit of thrombolysis was weighed against the risk of haemorrhage during the surgery after thrombolysis. Treatment guidelines are not well established for such a condition. This case report illustrates our experience and challenges faced in treating this condition.

Abstract Introduction: The S1Q3T3 pattern is neither sensitive nor specific for pulmonary embolisms; however, it can help guide clinical management in a patient with a vague presentation including syncope. Case Presentation: An elderly male presented to the hospital after an unwitnessed syncopal event. His initial EKG on admission revealed an S1Q3T3 pattern which prompted concern for a possible pulmonary embolism as the cause of his syncopal event. Further imaging confirmed the diagnosis.Conclusion: Syncope offers a long list of possible causes, but pulmonary embolism is commonly missed on the differential diagnosis. Our patients’ acute EKG findings ultimately helped guide our management to correctly diagnose our patient with a newfound pulmonary embolism as the cause of his syncopal event.

Background: Uterine fibroid embolization (UFE) is as an effective alternative to hysterectomy and myomectomy. However, UFE is associated with a wide spectrum of complications including in rare instances thromboembolic events and even pulmonary embolism (PE). Case Presentation: A case of a 36-year-old woman suffering from a symptomatic uterine fibroid is described as follows. The patient underwent UFE and overnight she suddenly collapsed with signs of massive PE. In consequence of cardiopulmonary resuscitation and urgent systemic thrombolytic therapy, cardiac activity was restored successfully. Discussion & Conclusion: This constitutes another report of PE developed following UFE. In the case described here PE occurred 26 hours after the procedure was performed. Before UFE all patients should undergo complete examination for exclusion of pre-existing venous deep thrombosis and coagulopathies.
