We present a very unusual case of a woman with an intrauterine contraceptive device (IUCD) who developed pelvic actinomycosis during long course chemoradiotherapy for rectal cancer, which presented a significant diagnostic challenge and eventually rendered the cancer unresectable. IUCDs are often implicated in the development of pelvic actinomycosis but there is no current evidence or guideline to suggest that they should be removed prior to oncological treatment. Owing to the devastating consequences of this combination of disease, we suggest that it may be prudent to remove IUCDs in this setting.

An unusual case of primary leiomyosarcoma presenting as a cystic mass is reported. An 86-year-old man underwent resection of a large cystic mass measuring 15 cm in diameter, arising from the left lower lobe of the lung. Histology of the resected specimen showed primary pulmonary leiomyosarcoma. The patient made a good recovery.

Tubulovillous adenomas are benign lesions, which are difficult to interpret because of their malignant potential. They have similar clinical, radiological and pathological features to malignant lesions. Usually, they are asymptomatic but may present with symptoms similar to adenocarcinomas. We present an unusual case of giant tubulovillous adenoma mimicking rectal cancer. The patient underwent neoadjuvant chemoradiation with no change in tumor size. He then underwent laparoscopic low anterior resection and final histopathology revealed giant tubulovillous adenoma.

In clinical practice, broad ligament (BL) tumors are seldom encountered. Paraganglioma of the BL is exceedingly rare entity. Here we present an unusual case of broad ligament paraganglioma, presenting as a cystic mass, in a 50-year-old postmenopausal female. A high degree of suspicion along with detailed immunohistopathological work-up is needed for arriving at an accurate diagnosis.

Primary intramuscular hydatid cyst of thigh is a very rare parasitic disease caused by Echinococcus granulosus. We present an unusual case of primary hydatid cyst in a 21-year old male who presented with slowly growing painless lump in the anteromedial aspect of proximal right thigh. Ultrasonography of the mass revealed a multiloculated cyst in the medial compartment of right proximal thigh without detectable primary any other location. MRI clearly displayed the lesion showing cystic mass with multiple well defined daughter cysts and diagnosis of hydatid cyst was made. The patient was treated surgically and cyst was excised. Macroscopic and microscopic histopathological examination confirmed the diagnosis of muscular hydatid cyst. Primary muscular hydatidosis is kept in mind in the differential diagnosis of a cystic mass of a skeletal muscle especially in endemic areas.TAJ 2011; 24(2): 142-144

ABSTRACT Paranasal sinus mucocele is slowly expanding cystic lesions leading to sinus expansion, bony erosion and extension into the orbit, cranial cavity or into subcutaneous tissue. Mucoceles most commonly involve the frontal and ethmoidal sinuses. We describe a frontal mucocele presenting with forehead subcutaneous cystic mass which was successfully managed by endoscopic sinus surgery and by external approach (frontal trephination). We discuss an unusual case of a large cystic mass arising in the left upper quadrant of a 48-year-old woman. Radiological investigations could not confirm the origin or the nature of the mass. A laparatomy revealed a large retroperitoneal cystic mass sandwiched between the left adrenal, spleen and the gastro-oesophageal junction. Histological analysis confirmed a mature teratoma of the retroperitoneum with neuroendocrine carcinoma arising within it. To our knowledge this is only the second reported case of its kind.
