Background and Aim: The third molar tooth lies with symptoms such as periodontal problems, caries, crowding, root resorption, cyst and tumor formation of the adjacent second molar. This study was to evaluate the position of mandibular third molar teeth and its effect on second adjacent molar teeth. Materials and Methods: Panoramic radiography was obtained from 264 patients with mandibular third molar hidden teeth referred to the Birjand School of Dentistry in 2018. The material and direction of their placement and the vertical position of the third molar tooth (based on the PELL & Gregory classification) and its placement angle (based on Winter classification) were determined. Also, the prevalence of distal caries, external root resorption, the amounts of displacement, t inflammatory lesions and the cystic was examined in the adjacent second molar tooth. Data were analyzed using SPSS software (Version 19), and Chi-square statistical test, p-values less than 0.05 were considered as statistically significant. Results: The data from this study showed that most of the wisdom hidden teeth were in Class B and III of Pell and Gregory classification, had Mesial angle based on Winter classification. In 20.8% of patients with caries, there was 3% root canal resorption and 1.1% displacement of the second molar tooth, while no cases of inflammatory and cystic lesions in the second molar tooth were found due to the hidden third molar tooth. The Mesial and horizontal angles of impacted wisdom teeth were associated with increased risk of caries and root resorption of the second molar. Also, Class B's third molar teeth increased caries in second molar teeth. Conclusion: Most pathological problems were observed in Mesial, horizontal, and class B impaction. According to the observed relationship, it seems that the angle and depth of the hidden third molar should be taken into account c when deciding whether or not to extract it.

Objective: The purpose of this study is to assess the impact of mesioangular mandibular 3rd molar impaction on the periodontal health of neighbouring teeth in patients. Study Design: Dental College HITEC-IMS Taxilla/Gulraiz Dental Clinic Quaid Avenue Main Road, Gulraiz 3, Rawalpindi. Jan 2021-Aug 2021 Place and Duration: Methods: Total of ninety patients of both genders was presented in this study. After receiving informed written consent, detailed demographic information about the recruited cases was collected, including age, gender, body mass index, symptoms, and complications. All of the patients experienced mesioangular third molar impactions on their mandibular teeth. A total of 90 IMTMs were evaluated using panoramic radiographs to determine their impaction depth, relationship with the ramus, and angulation, as well as their association with 2nd molar distal caries and root resorption, pathological states, and proximity to the mandibular canal. The position of the impacted third molar was determined using the Pell and Gregory categorization system. The odds ratio (OR) and 95 percent confidence interval (CI) of IMTM's position on the associated problems are shown in the table below (credible interval for Bayesian models). P-values with two-tailed confidence intervals of less than 0.05 were considered statistically significant. SPSS 27.0 version was used to analyze complete data. Results: There were 55 (61.1%) males and 35 (38.9%) females with mean BMI 24.61±5.22 kg/m2. Most of patients 41 (45.6%) were aged between 20-35 years. Mean attachment level was 2.99±4.74 and probing depth was 3.78±4.61. Caries, pain, and swelling were the most often reported symptoms, according to our research. The most common diseases were distal second molar root caries, which accounted for 30 (33.3%) of all cases, and pocket development, which accounted for 21 (23.3%). According to histological findings, periapical inflammation was the most common finding in 46 (51.1%) of the cases, followed by dental follicle in 24 (26.7%), cyst in 16 (17.8%), and other malignancies in 4 (4.4%) cases. Conclusion: According to the findings of this study, the impacted mandibular third molar was most closely associated with distal second molar root caries and the formation of a pocket between the impacted tooth and the second molar tooth in the lower jaw. Most common pathology linked with the impacted molar was distal second molar root caries and the formation of a pocket between the affected tooth and the second molar. Objective: To investigate whether the use of cone beam CT (CBCT) changes the diagnosis of external root resorption (ERR) or marginal bone loss (MBL) involving a second molar adjacent to an impacted third molar. Methods: A systematic search was applied in PubMed, EMBASE, Scopus, Web of Science, LILACS, Google Scholar, OpenGrey, and ProQuest. Studies assessing the detection of ERR or MBL in a second molar adjacent to an impacted third molar through CBCT and panoramic radiography (PAN) were included. Prevalence and agreement between PAN and CBCT on the detection of ERR and MBL were collected. The risk of bias was assessed using the MAStARI. Results: A total of 593 papers were identified, and after a 2-phase selection, 5 studies were included in the narrative synthesis. Regarding ERR, its prevalence in PAN was reported from 5.31 to 19.5% and from 22.8 to 62.0% in CBCT. The percentage of agreement varied from 28.5 to 74.0%. The prevalence of MBL varied from 21.9 to 62.9% in PAN, while those values varied from 21.6 to 80% in CBCT images. The percentage of agreement between PAN and CBCT for the detection of MBL ranged from 66.0 to 85.0%. Four studies presented low risk of bias and one had moderate risk. Conclusions: More ERR and MBL are assessed in CBCT compared to PAN. There is a considerable agreement between PAN and CBCT assessment of ERR and MBL, however, mostly related to the absence of the pathology rather its presence.

Objective: This study sought to assess the frequency and severity of second molar external root resorption (ERR) due to the adjacent third molar and its correlation with the position of third molar and other related factors using cone-beam computed tomography (CBCT). Materials and Methods: This cross-sectional study evaluated 320 second molars and their adjacent impacted third molars on CBCT scans of patients over 16 years, retrieved from the archives of Azad University Radiology Department. Presence/absence of second molar ERR, its location and severity (if present), and position of adjacent third molar were determined on CBCT scans, and recorded in a checklist. Data were analyzed using a logistic regression model. Results: The frequency of second molar ERR was 33.4% in the mandible and 14% in the maxilla. The severity of ERR was significantly correlated with the involved jaw (P=0.001) but had no correlation with age, gender, or depth of impaction of adjacent third molar (P>0.05). The mesioangular and horizontal positions of impacted third molars had a significant correlation with the frequency of second molar ERR (P<0.006). Conclusion: According to the results of this study, ERR occurring in second molars adjacent to third molars is common, especially in the mandible. Mesially inclined third molars have a greater potential of being associated with ERR in second molars.

AbstractThere have been infrequent case reports of bilateral root resorption in the lower second molar secondary from impacted third molar removal with bone regeneration without pain. Root resorption is commonly observed during third molar surgical impaction that affects mandibular second molars. The bone regeneration into the adjacent affected tooth is evident simply after the surgical extraction of the impacted tooth. However, there have been a few previous reports on the prognosis of the remaining resorbed tooth. Most dentists often choose to extract them when damaged if root resorption is observed. This report case involved bilateral root resorption of the lower second molar due to lower adjacent impacted third molar teeth and bone regeneration without pain sensation. After surgical extraction of bilateral lower impacted third molar teeth, the remaining teeth retained vital pulp and survived as functional teeth.

Objectives: To evaluate impacted mandibular third molar tooth region and obtain linear measurements using CBCT images and to assess the relationship between the impacted third molar and the mandibular canal. Methods: CBCT scans of 351 patients (208 females, 143 males) were assessed. Age, gender, and impaction site were recorded for each patient. The relationship of third molars with the vertical axis of second molars, 2nd molar resorption and the relationship between third molar apices and the mandibular canal were assessed. In addition, the distance between ramus and second molar, mesiodistal width of the third molar, the angle between third molar and second molar, and width of the third molar capsule were measured. Binary Logistic Regression, Chi-Square Test, and General Linear Model were used for statistical analysis. Results: The highest percentage of impaction was found for mesioangular followed by transversal and vertical. The transversal impacted third molars revealed a significant association with adjacent second molar root resorption (p<0.001). There was a statistical significance between the second molar resorption and distance between ramus and second molar (p<0.001). The mesioangular impacted third molars revealed significant relation with the mandibular canal (p<0.05). The most frequent variation found was the dental canal followed by the retromolar canal. In general, higher measurement values were obtained for men when compared to women (p<0.05). Conclusion: CBCT assessment of the third molar region provided useful information regarding impacted mandibular third molar surgery operations.

Objective: The aim of this study is to determine the impact of mesioangular mandibular 3rd molar impaction on periodontal health of adjacent tooth in patients. Study Design: Retrospective/observational study Place and Duration: Department of Oral & Maxillofacial Surgery, MMDC Multan, during from Oct 2020 to September 2021. Methods: Total of seventy patients of both genders was presented in this study. Detailed demographic data of enrolled cases age, sex, body mass index, symptoms and complications were recorded after taking informed written consent. All the patients had mesioangular impactions of mandibular third molars. The impaction depth, relationship with ramus, and angulation of 70 IMTMs and their association with 2nd molar distal caries and root resorption, pathological states, and closeness to the mandibular canal were assessed on panoramic radiographs. Pell and Gregory classification was used to determine position of impacted third molar. The odds ratio (OR) and 95% confidence interval (CI) of IMTM's position on the related complications (credible interval for Bayesian models). Statistical significance was defined as a two-tailed p-value 0.05. SPSS 24.0 version was used to analyze complete data. Results: There were 42 (60%) males and 28 (40%) females with mean BMI 23.13±6.46 kg/m2. Most of patients 31 (44.3%) were aged between 20-35 years. Mean attachment level was 3.03±2.34 and probing depth was 3.34±3.35. Caries, pain and swelling were the most common symptoms found among cases. Distal second molar root caries 21 (30%) and pocket formation 17(24.3%) were the most common pathologies. As per histological findings periapical inflammation was majority found in 37 (52.9%) cases followed by dental follicle in 19 (27.1%) and cyst in 11 (15.7%). Conclusion: In this study, we found that the impacted mandibular third molar was most linked with distal second molar root caries and the creation of a pocket between the impacted tooth and the second molar tooth. The most common pathology associated with the impacted molar was distal second molar root caries and the creation of a pocket between the affected tooth and the second molar.
