Objective The present study is a case report of a 57-year-old female with controlled hypertension who presented with spontaneous spinal epidural hematoma (SSEH) mimicking a cerebrovascular accident (CVA) and was successfully treated by surgical decompression. Methods A 57-year-old woman with a medical history of hypertension presented with a sudden onset of weakness in the right upper and lower extremities. Weakness of grade 3/5 was noted in her right upper and lower extremities, but there was no motor weakness of the right facial muscles. A magnetic resonance imaging (MRI) exam of the cervical spine revealed an epidural hematoma extending from level C5 to level C7, causing spinal cord compression. Results During surgery, a cervical spinal epidural hematoma (SEH) was evacuated. Postoperatively, the power in both limbs improved to grade 5/5 just after surgery. Conclusions A high degree of suspicion, meticulous history taking, and physical examination have a great importance in these rare conditions because anticoagulant therapy as a routine treatment for ischemic CVA could be life-threatening.

<p>Dissection aneurysms of the vertebral artery are a rare pathology occurring mainly in young people. Their most frequent manifestation is pain, which might be accompanied by cerebral ischemia events. Also rare is hemorrhage, with just one case described in literature. In our study, hemorrhage from a dissection aneurysm of the vertebral artery in a 37-year-old male led to the formation of an epidural hematoma of the cervical spine, which is unique and has never manifested itself when treating this pathology. Nevertheless, reconstructive surgery was a success, with the aneurysm removed and the vertebral artery lumen preserved.</p><p>Received 28 April 2018. Revised 10 May 2018. Introduction. Cervical epidural hematoma is a rare phenomenon in children. MRI scan of the cervical spine is the method of choice in establishing diagnosis and a quick evacuation of hematoma and decompression. Case Outline. We present a case of 11-year-old boy whose lower extremities were paralysed after a trivial injury of the cervical spine which he sustained while playing. Seven days after the injury, severe clinical symptoms appeared, and MRI of the spine showed the presence of cervical epidural hematoma. Conclusion. Cervical epidural hematoma in children is very rare. Adequate neurological examination, diagnostics and urgent surgical intervention are of paramount importance for accomplishing good results. Early and persistent rehabilitation are the condition for achieving maximal recovery.
