ObjectiveTo examine perceptions and experiences regarding providing spiritual care at the end of life of elderly care physicians practising in nursing homes in the Netherlands, and factors associated with spiritual care provision.MethodsA cross-sectional survey was sent to a representative sample of 642 elderly care physicians requesting information about their last patient who died and the spiritual care they provided. We compared their general perception of spiritual care with spiritual and other items abstracted from the literature and variables associated with the physicians’ provision of spiritual care. Self-reported reasons for providing spiritual care were analysed with qualitative content analysis.ResultsThe response rate was 47.2%. Almost half (48.4%) provided spiritual end-of-life care to the last resident they cared for. Half (51.8%) identified all 15 spiritual items, but 95.4% also included psychosocial items in their perception of spirituality and 49.1% included other items. Physicians who included more non-spiritual items reported more often that they provided spiritual care, as did more religious physicians and those with additional training in palliative care. Reasons for providing spiritual care included a request by the resident or the relatives, resident’s religiousness, fear of dying and involvement of a healthcare chaplain.ConclusionMost physicians perceived spirituality as a broad concept and this increased self-reported spiritual caregiving. Religious physicians and those trained in palliative care may experience fewer barriers to providing spiritual care. Additional training in reflecting upon the physician’s own perception of spirituality and training in multidisciplinary spiritual caregiving may contribute to the quality of end-of-life care for nursing home residents.

ZusammenfassungSpiritual Care und palliative Care machen sowohl auf Seiten des Patienten als auch des therapeutischen Teams eine mehrdimensionale Blickweise erforderlich, die die Grenzen des rein Somatischen überschreitet. Ihnen gemeinsam ist die Sorge für den Betroffenen – eine Perspektive, die der Medizin immanent sein könnte, aber nicht immer als selbstverständliche, nicht zu delegierende Aufgabe angesehen wird. Dabei gilt der Grundsatz der Subsidiarität: Priorität haben immer die Bewältigungsressourcen des Patienten und seines Umfeldes, die von den professionell und ehrenamtlich Helfenden unterstützt werden. In der Herausforderung dieses Arbeitsumfeldes geraten die Professionellen unweigerlich in die Konfrontation mit der eigenen Endlichkeit des Lebens, den damit verbundenen existenziellen Ängsten und den eigenen Widerständen. Diese Auseinandersetzung mit der Unheimlichkeit des Lebensendes, ihr respektvoll zu begegnen und nicht angstvoll zu verdrängen, kann im positiven Sinne als Burnout-Prophylaxe wirksam werden.

Co-design is a process in which designers and users collaborate as ‘equals’ to develop innovative solutions. Co-design methods are increasingly used by professional designers to facilitate and enable users to co-develop innovative solutions for ‘themselves’. For example, the Design Council is advocating the use of co-design methods to support the development of practical innovative solutions to social problems such as increased cost of elderly care and tackling child poverty. The involvement of users in developing solutions acknowledges that their take up is dependent on the ways users create and negotiate meanings of objects and services.

What is the perception of Turkish migrants in elderly care? The increasing number of elder migrants within the German and Austrian population is causing the challenge of including them in an adequate (culturally sensitive) way into the German/Austrian health care system. Here I introduce the perception of elder Turkish migrants within the predominant paradigm of intercultural opening of health care in Germany as well as within the concept of diversity management of health care in Vienna (Austria). The qualitative investigation follows a field research in different German and Austrian cities within the last four years and an analysis based on the Grounded Theory Methodology. The meaning of intercultural opening on the one hand, and diversity management on the other hand with respect to elderly care will be evaluated. Whereas the intercultural opening directly demands a reduction of barriers to access institutional elderly care the concept of diversity is hardly successful in the inclusion of migrants into elderly care assistance – concerning both, migrants as care-givers and migrants as care-receivers. Despite the similarities between the health care systems of Germany and Austria there are decisive differences in the perception and inclusion of migrants in elderly care that is largely based on an 'individual care' concept of the responsible institutions. Finally, this investigation demonstrates how elderly care in Germany and Austria prepares to encounter the demand of 'individual care' in a diverse society.
