ABSTRACT The aim of the current study is to present a treatment approach in a case of severe early childhood caries (ECC). A 5-year-old girl was referred to the Pediatric Dentistry Division with multiple premature tooth loss, masticatory difficulties, prolonged breast-feeding, and low quality of life, who was diagnosed with severe ECC. A three-phased treatment plan was implemented: 1. Disease control – consisting of behavioral changes in oral hygiene habits, diet guidance, professional biofilm removal followed by fluoride application and temporary restorations; 2. Infection control – teeth extractions and pulpotomy; and 3. Functional rehabilitation – direct and semi-indirect resin restorations, a removable partial and total prostheses in the lower and upper jaws, respectively. In conclusion, the proposed multidisciplinary approach resulted in a positive impact on the patient’s nutrition and growth, speech production, communication, self-image, and social functioning leading to an improved quality of life.

This paper reports the case of a 5-year-old patient with early childhood caries (ECC) and presents an alternative prosthetic treatment with a tooth-supported overdenture. Primary canines were endodontically treated and received intraradicular posts with ball-type attachments to attach a tooth-supported overdenture. The patient was followed for 18 months both clinically and radiographically. In addition to esthetic and functional oral rehabilitation, the prosthetic treatment had an important psychological impact on recovery of patient's self-esteem.

Early Childhood Caries (ECC) can be defined as the presence of one or more carious lesions, may or may not be cavitated, and missing teeth due to caries in children up to five years of age. It is multifactorial involving the presence of bacterial plaque, poor oral hygiene, frequency and time of consumption of beverages containing sugar. It has several deleterious effects like it can damage speech, swallowing, feeding, development, esthetics and self-esteem of the child. Therefore, complete oral rehabilitation in such patients is utmost important for their normal growth and development. The current case report involves the oral rehabilitation using different treatment options.

Introdução: A cárie severa da infância gera um impacto econômico, emocional e físico nas crianças e em suas famílias, além de determinar alterações funcionais, estéticas e comportamentais, as quais devem ser consideradas na efetivação de um tratamento odontológico eficaz. Objetivo: Este trabalho teve como objetivo realizar um relato de caso clínico de reabilitação estética e funcional de um paciente infantil, do sexo masculino, 5 anos de idade, acometido por CSI, atendido na disciplina de Clínica Odontológica Infantil da Universidade Federal do Amazonas. Caso Clínico: Ao exame clínico e radiográfico, foram observadas lesões cariosas cavitadas nos elementos dentários 51,52,61,62 e 64, além de mancha branca ativa no 74. O tratamento proposto para este paciente incluiu orientação de hábitos de higiene bucal e dietéticos adequados, aplicações tópicas de flúor nas lesões cariosas ativas, necropulpectomia e restauração com resina composta fotopolimerizável. O dente 64, bem como nos dentes 51,52,61 e 62, severamente destruídos pela doença cárie, usando-se coroas de acetato. Conclusão: O tratamento mostrou-se bem sucedido, sendo possível sua realização em ambiente ambulatorial, permitindo restabelecer a função mastigatória e a estética do sorriso, além de promover a educação em saúde por meio da conscientização da criança/núcleo familiar acerca da importância do autocuidado na eliminação dos fatores etiológicos e no controle da doença cárie. Abstract Aim The purpose of this case report was to describe the oral rehabilitation of a 4-year-old girl presenting early childhood caries (ECC). Background ECC is highly prevalent in developing countries and its severity increases with age. This disease implies serious consequences for the development of the stomatognathic system and for the child's quality of life. As young children are usually anxious about dental treatment, their level of co-operation is limited, leading to a challenging situation. Case Report A 4-year-old girl was brought by her mother for dental treatment with the complaint of pain. The clinical examination revealed extensive carious coronary destructions. After preventive and curative measures, the oral rehabilitation was performed; it included the use of stainless steel crowns, resin filled celluloid crowns with previous cementation of glass post pins when necessary, an amalgam restoration, orthodontic treatment, and a partial removable prosthesis. The child has been monitored in the Pediatric Dentistry Clinic at 4-monthly intervals. Summary The oral rehabilitation was able to reestablish the oral health in the primary arches mutilated by early childhood caries. This is important for the establishment of an adequate mixed and permanent dentition, for proper facial and maxillary growth, and to the child's psychological and social development. Clinical Significance The full management, including preventive, psychological, and curative measures of a young child with severe ECC was found successful after 8 months of follow up. This result can encourage the clinicians to seek a cost-effective technique such as stainless steel crowns, resin filled celluloid crowns, and partial removable prosthesis to reestablish the oral functions and improve the child's psycho-social development. For prevention of dental caries, S. mutans numbers must be reduced and prevented from returning to the original level. An antibacterial agent that is effective and also acceptable to young children can help to establish a favorable oral environment and halt the caries process. Purpose: This study was conducted to evaluate the efficacy of topical antimicrobial (10% Povidone- Iodine) on S.mutans counts in children with Early Childhood Caries after full mouth rehabilitation. Also its effects on the clinical outcomes in terms of incidence of new caries and secondary caries were evaluated. Method: Full mouth rehabilitation was done in 30 children (mean age 4.2 years) suffering from Severe Early Childhood Caries (SECC). Stimulated saliva samples were collected to determine the post operative baseline S.mutans counts. Thereafter the subjects were randomly divided into two groups. The experimental group received 10% Povidone Iodine at 3 months interval for a period of 12 months while the control group received placebo (deionized water) at similar intervals. Change in the S.mutans count at 6 and 12 month intervals were compared to the baseline post-operative mutans score. Results revealed that application of 10% Povidone Iodine caused a significant reduction in the rise of Streptococcus mutans levels from the baseline postoperative score after 12 months of treatment thus decreasing the oral load of the organisms. Reduction in counts; in turn decreased the relapse of caries in these children. Conclusion: Thus oral rehabilitation coupled with regular application of 10% Povidone Iodine application can be a good alternative to control dental caries in children affected with Early Childhood Caries (ECC).
