Objective – the choice of tactics for the treatment of breast cancer in a kidney transplant recipient. The article presents a clinical case of a 51-year-old patient with a clinical diagnosis: left breast cancer IIA stage рТ2N0М0G3; triple-negative type. About chronic renal failure 10 years ago the patient was kidney transplantation was performed. The development of transplantation has expanded the use of immunosuppressive therapy in clinical practice. As a result, the risk the development of secondary tumors, including breast cancer, increases.

Представленное в статье наблюдение представляет большой интерес с точки зрения подтверждения общих закономерностей и выработки конкретных рекомендаций по лечению рака молочной железы у пациентов после трансплантации органов. Представленное наблюдение также подтверждает данные некоторых исследований, свидетельствующих о повышении риска развития онкологических заболеваний даже через много лет после применения поли- или моноклональных антител как для индукции иммуносупрессии, так и для лечения острого отторжения (как в случае, приведенном в статье).

Breast angiosarcoma (BAS) is an extremely rare and poorly studied neoplasm, the etiology of primary BAS remains controversial, and secondary BAS is most often radio-induced. Radio-induced breast tumors usually appear 10–20 years after the initial treatment; however, for BAS this period is much shorter and is about 4–7 years. This review presents literature data on the features of the clinic, diagnosis and treatment of BAS, as well as own clinical case observation of radio-induced sarcoma of the left breast, that developed 4 years after the primary breast cancer treatment.

Experimental and clinical studies analyzing the impact of AVF on cardiovascular and renal parameters, as well as outcomes, in kidney transplant recipients are lacking. On the other hand, it is not known whether AVF ligation after transplantation modifies hemodynamic parameters and kidney function. We report a case of a renal transplant recipient who developed an acute congestive heart failure accompanied by renal failure, which were triggered by femorofemoral AVF angioplasty. Prompt AVF ligation rapidly reversed clinical symptoms and normalized cardiac and renal functions. This paper illustrates the potential deleterious consequences of high-output AVF after kidney transplantation and raises considerations regarding the impact of the fistula on cardiac status and kidney function after kidney transplantation and, consequently, the management AVF after transplantation.
