TÓM TẮT Đối với ung thư vú (UTV) xạ trị là điều trị bổ túc cần thiết giúp giảm tái phát tại chỗ tại vùng, gia tăng sống còn. Tuy nhiên, xạ trị có tác dụng phụ lên tim mạch nhất là với UTV trái. Nhiều nghiên cứu đã chứng minh biến chứng và tử vong do tim mạch tăng tỉ lệ thuận với liều trung bình lên tim. Các kỹ thuật xạ trị mới trong xạ trị ung thư vú trái giúp tối ưu hóa liều vào thể tích xạ và bảo vệ tốt hơn cơ quan lành, càng đòi hỏi độ chính xác cao khi đặt bệnh. Các sai số do thiết bị hoặc đặt bệnh sẽ dẫn đến nguy cơ quá liều dung nạp hoặc thiếu liều và có nguy cơ tái phát. Vì vậy, xạ trị đòi hỏi sự chính xác cao trong suốt quá trình từ mô phỏng đến lập kế hoạch và tiến hành xạ trị. Xạ trị ung thư vú trái phối hợp hít sâu nín thở (DIBH - Deep Inspiration Breath - hold) và hệ thống quản lý bề mặt quang học (OSMS - Optical Surface Management System) làm cho tim di chuyển ra xa hơn khỏi vú, thành ngực trong quá trình xạ trị, giúp giảm liều tim một cách rõ rệt, vẫn đảm bảo được liều xạ lên thể tích đích. Ưu điểm của OSMS ngoài áp dụng cho đặt bệnh nhanh không cần xăm dấu trên da còn giám sát thời gian thực trong toàn bộ quá trình điều trị. Khi nhịp thở của bệnh nhân vượt quá một ngưỡng nhất định (0,3cm) chùm tia bức xạ sẽ bị tắt để ngăn ngừa độc tính. Do đó, hiểu và nắm rõ lợi ích quy trình đặt bệnh giúp đạt mục tiêu điều trị đồng thời giảm thời gian đặt bệnh để người bệnh có tâm lý thoải mái hơn, giảm áp lực về số lượng bệnh, nhất là đối với các trung tâm xạ trị lớn. ABSTRACT BENEFITS OF LEFT BREAST CANCER RADIATION COMBINATION OF DEEP INSPIRATION BREATH - HOLD AND OPTICAL SURFACE MANAGEMENT SYSTEM OF TRUEBEAM MACHINE AT CANCER HOSPITAL HO CHI MINH CITY Background: For breast cancer, radiation therapy is an essential adjuvant treatment to help reduce local recurrence and increase survival. However, radiation therapy has adverse effects on the cardiovascular systemespecially for left breast cancer. Several studies have demonstrated that cardiovascular morbidity and mortality increase in proportion to the mean cardiac dose. New radiotherapy techniques in radiation therapy for left breast cancer help optimize dose to radiation volume and better protect healthy organs, which requires high accuracy when patient set - up. Errors due to equipment or patient set - up will lead to the risk of overdosage or underdosage and risk of relapse. Therefore, radiation therapy requires high precision throughout the process from simulation to planning and conducting radiation therapy. Radiation therapy for left breast cancer combined with Deep Inspiration Breath - hold (DIBH) and Optical Surface Management System (OSMS)causes the heart to move further away from the breast and chest wall during radiation therapy, helping to reduce the cardiac dose significantly, still ensuring the radiation dose to the target volume. The advantage of OSMS is that in addition to being applied for fastpatient set - up, without tattooing on the skin, it also provides real - time monitoring during the entire treatment process. When the patient’s breathing rate exceeds a certain threshold (0,3cm) the radiation beam is turned off to prevent toxicity. Therefore, understanding and understanding the benefits of patient set - up helps to achieve treatment goals, while reducing patient set - up time for more comfort, reducing pressure on the number of patients, especially for patients large radiotherapy center. Nodular fasciitis is rarely reported in breast. It is benign reactive proliferative lesion of fibroblast. A 65 year old female presented to surgical department for left breast mass since 2 months duration, rapidly enlarging without any regional lymphadenopathy. Mammography showed hyperdensity masses with irregular margin. On FNAC reported as benign spindle cell lesion. Left sided two breast masses measuring 4 x 3.3 x 2.5cm and 2.0 x 1.6 x 0.5cm were excised and on histopathology reported as Nodular Fasciitis of left breast. We are presenting this extremely rare case of nodular fasciitis of breast for its clinical, radiological and histopathological findings.

Primary neuroendocrine carcinomas of the breast are rare of all breast carcinomas. They may be welldifferentiated, poorly differentiated, or invasive breast cancers with neuroendocrine differentiation. They are staged and treated similarly to conventional breast cancer. Herein, we report a case of invasive ductal carcinoma with neuroendocrine differentiation of the breast in a 73 years female with a history of breast lump initially in the lower inner quadrant of left breast and a month later, similar lump at the same site in right breast. Patient underwent Modified Radical Mastectomy bilaterally followed by adjuvant chemotherapy based on Carboplatin and Etoposide regimen.

Breast cancer is the most common cause of cancer-related deaths in the female population worldwide. To the best of our knowledge, breast cancer (BRCA)1/2 gene mutations have not been described yet on breast cancer cytological specimens. Here we describe the case of a 38-year old woman with a family and personal history for breast cancer, who underwent a fine needle aspiration (FNA) procedure for a novel 30 mm lesion located in the external quadrants of the contralateral (left) breast. Cytological findings and ancillary immunostaining confirmed the diagnosis of a triple negative NST carcinoma. BRCA1/2 molecular assessment was carried out on DNA extracted from cytological (November 2020), biopsy (December 2014) and surgical resection (July 2015) specimens, as well as on the resection of a benign fibroadenoma, by using a next generation sequencing approach. Molecular analysis showed a pathogenic BRCA1 insertion (c.5266dupC; p.Q1756PfsTer74) in the cytological specimen (allelic fraction 92.0%), biopsy (allelic fraction 84.2%), surgical resection (allelic fraction 87.8%) and fibroadenoma (58.9%), demonstrating a germinal BRCA mutated status.

The number of patients undergoing bariatric surgery is increasing worldwide. Different types of free flaps are often used for breast reconstruction following mastectomy. We present a not previously described case using a vertical myocutaneous gracilis flap for breast reconstruction in a massive weight loss patient. The patient was a 61 year-old woman who previously had a lumpectomy for an in situ ductile carcinoma of her left breast. Subsequently the patient underwent a full mastectomy in 2020 due to a recurrence. The massive weight loss population poses a challenge in reconstructive surgery, due to their higher risk of complications. However, we still believe free flaps should be considered as a valid option for breast reconstruction in these patients. Due to the often increased size of perforator vessels in these patients, other flaps may prove more suitable than the usually preferred ones.

A 63-year-old woman with grade 2 infiltrating left breast carcinoma who was started on ribociclib complained of exertional dyspnoea along with dry cough. There were bilateral interscapular crackles and chest X-ray evidence of bilateral mid and lower zone non-homogeneous opacity. The patient’s pulmonary function test (PFT) showed moderate restrictions and desaturation. CT was suggestive of organising pneumonia and drug administration was stopped. The patient was treated with steroids in tapering doses, which led to improvements. The drug was restarted with the probability of other aetiologies for interstetial lung disease (ILD). It was also considered the superior efficacy of ribociclib in managing breast cancer. But due to evidence indicating the reappearance of organising pneumonia following drug administration, it was again stopped, and steroid use was restarted for treatment. The patient showed improvements in subsequent follow-ups.

Abstract Background Primary breast lymphoma (PBL) is an extremely rare neoplasm in children; by definition, it manifests in the breast without evidence of lymphoma elsewhere, except ipsilateral axillary nodes. Case presentation We report a case of a 15-year-old girl diagnosed with diffuse large B-cell lymphoma (DLBCL) of the right breast: the patient received chemotherapy and rituximab, achieving complete remission. A literature review revealed other 11 cases of pediatric PBL; it mainly affects female adolescents and can involve right and left breast equally. Different histologic subtypes have been described, arising from both B-cell and T-cell. Therapeutic approaches were very different, from chemotherapy to local treatment with surgery and/or radiotherapy. Conclusions Our case is the first in which rituximab was administered, suggesting to be a promising therapy in B-cell PBL, as already demonstrated in pediatric B-cell lymphoma from other sites. Further investigations are needed to identify prognostic factors and establish the most effective treatment.

Extravasation of doxorubicin, vincristine or vinblastine leads to necrosis, damage of the muscles and nerves, deep ulceration, as well as limb dysfunction. Necrosis and deep ulcers develop within 7 to 28 days. Like necrotomy, Lucilia sericata maggot therapy is recognised as a method enabling effective, safe and quick removal of necrotic tissue. The purpose of the study was to present local treatment of hypodermic necrosis caused by docetaxel extravasation in course of systemic cancer therapy. A woman, 59 years of age, in course of systemic therapy due to advanced cancer of the left breast (T2N1M1 CS IV) with confirmed metastases within the body of the fourth lumbar vertebra and in the liver, receiving a combination treatment with pertuzumab, trastuzumab, and docetaxel. During the therapy, a conservative treatment was applied due to extravasation for over three months. Effects in the right forearm included swelling, redness, signs of 4x10cm inflammatory infiltrate, with 1x4cm necrotic crust visible in the central region. Hypodermic necrosis was debrided using L. sericata maggots, and subsequently specialist dressings were applied to promote granulation and healing. In the case discussed here, effectiveness of MDT was rather poor, however the treatment minimised the risk of infection associated with evacuation of necrosis. Attempts to use MDT should be continued to enable more comprehensive understanding of problems related to management of necrosis in wounds developing during cancer therapy.

Chronic myeloid leukemia is an insidiously progressive condition and comparatively rare type of blood cell malignancy that begins in the bone marrow. Chronic myeloid leukemia typically affects adult population and is documented to be caused by chromosomal mutation that usually occurs spontaneously. Chronic myeloid leukemia is more common in males than in females (male: female ratio of 1.4:1) and appears more commonly in the elderly with a median age at diagnosis of 65 years [1] Exposure to ionising radiation is one of the risk factors, based on a 50 fold higher incidence of CML in Hiroshima and Nagasaki nuclear bombing survivors [1] The rate of CML in these individuals seems to reach at its peak about 10 years after the exposure [1]. Carcinoma breast on the other hand is one of the most common causes of death in middle aged women in western countries. There are numerous factors contributing as its etiological factors such as age, gender, diet, endocrinal factors, previous radiation exposure, genetic factors and geographical factors. We present a case report of a 44 old female who came to Acharya Vinoba Bhave Rural Hospital (Datta Meghe Institute of Medical Sciences and Research), with presenting complaint of lump in the left breast since 2 days and abdominal mass for 1 month. On investigations, patient was diagnosed with a rare case of chronic myeloid leukemia on the complete blood count and peripheral smear and the lump in the left breast also revealed invasive ductal carcinoma of the left breast.
