Management of space problems is an important factor in the mixed dentition. The Clear Aligner can carry out minor tooth movement with no brackets bonded to the teeth. In addition, it can provide tooth movement for the following: crowding resolution, space closure, arch expansion or constriction, space maintenance or regaining, intrusion or extrusion, guidance of the eruption, and other interceptive orthodontic treatment.

ABSTRACT Hypodontia is the most prevalent craniofacial malformation in mankind. It may present a wide variety of manifestations and, depending on the number and location of missing teeth, it may affect the esthetics, mastication, speech and occlusal balance. This paper discusses the therapeutic approaches to solve this condition, describing a case report with hypodontia of one mandibular lateral incisor, which treatment option included space closure at the region of hypodontia associated with composite resin restorations in the mandibular central incisors. The three-year follow-up after treatment revealed occlusal stability, adequate intercuspation in Class I relationship and excellent micro and macroesthetics.

Background: Tipped lower molar over edentulous space is very common in orthodontics practice when adults seek treatment. The segmented arch technique features a predictable force system that provides a controlled release of force that can produce light and continuous tooth movement. Case Description: A female adult patient, who lost a permanent lower first molar, needed correction of the position of her permanent first molar place. Instead of making space for rehabilitation, it was closed after second molar uprighting and a balanced interdigitation was created without prosthetics. The patient was successfully treated with segmented arch technique using root correction spring activated with geometry VI to promote uprighting of a tipped molar and Niti spring coil to promote space closure. Practical Implications: Segmented arch technique is known to provide predictable light and continuous forces, which is very much indicated in adult treatment. There are several things to consider when orthodontically treating adult patients. Their periodontal conditions might not be ideal, less bone apposition may occur, and side effects of orthodontic tooth movement are expected. Thus, a predictable and controlled orthodontic treatment is needed.

A 26-year-old man with an aggressive periodontitis sought for orthodontic treatment to improve the appearance of his smile. He presented with generalised anterior spacing, missing lowerleft central incisor and deep traumatic bite. He was treated successfully with a combination of orthodontic and periodontal treatment. After 18 months of orthodontic treatment and follow upby the periodontist, his alignment of teeth was improved, a stable occlusion was achieved, and occlusal trauma was prevented. As a result, the patient’s smile appearance and selfconfidence were improved. Orthodontic tooth movement is not only to correct the alignment of his teeth but also to improve the bone level especially at the anterior region. This case report shows the successful treatment outcome in aggressive periodontitis patient which requires good collaboration between the orthodontist and the periodontist.

Clear aligner technology has become the preferred choice of orthodontic treatment for malocclusions for most adult patients due to their esthetic appeal and comfortability. However, limitations exist for aligner technology, such as corrections involving complex force systems. Composite attachments on the tooth surface are intended to enable active control of tooth movements. However, unintended tooth movements still occur. In this study, we present an effective attachment design of an attachment that can efficiently induce tooth movement by comparing and analyzing the movement and rotation of teeth between a general attachment and an overhanging attachment. The 3D finite element modes were constructed from CBCT data and used to analyze the distal displacement of the central incisor using 0.5- and 0.75-mm-thick aligners without an attachment, and with general and overhanging attachments. The results show that the aligner with the overhanging attachment can effectively reduce crown tipping and prevent axial rotation for an intended distal displacement of the central incisor. In all models, an aligner with or without attachments was not capable of preventing the lingual inclination of the tooth.

In the case report showcased, we describe orthodontic treatment of a female patient with an excessive gingival display on smiling and severe dental crowding, with maxillary canines positioned excessively buccal (ectopic) to and in near transposition to the lateral incisors. The treatment involved extractions and initial use of unmitigated forces leading to excessive gingival thinning and buccal root positioning of the ectopic canines. Eventually, the root position was corrected and periodontal equilibrium was attained. On finishing the treatment, all objectives were achieved with a good esthetic outcome as well as excellent dental and occlusal relationships.

Patients presenting with severe hypodontia can be challenging to manage. They often present with significant spaces within the dental arches and the lack of permanent teeth can make anchorage control difficult. This case report demonstrates a novel technique for diastema closure in a 14-year-old with severe hypodontia. The technique allows maintenance of root parallelism during space closure in cases with reduced anchorage support. The treatment plan was agreed on a multidisciplinary hypodontia clinic and involved an upper sectional fixed orthodontic appliance to close a 7-mm midline diastema aiding subsequent partial denture provision. Treatment commenced by bonding upper primary canines and central incisors. However, attempting space closure on a 0.019 × 0.025-inch stainless-steel working archwire resulted in tipping of the incisor crowns. Two brackets were therefore placed on the labial surface of each central incisor (one incisal and one gingival). Subsequently two stainless steel working archwires and elastomeric chain were used for diastema closure. This four-bracket system provided superior control allowing space closure by bodily tooth movement. Excellent root parallelism was achieved with this innovative technique.

The duration of treatment is the most concern among orthodontic patients. It is of a challenge for an orthodontist to shorten the treatment time. A new surgical technique was introduced to accelerate tooth movement during orthodontic treatment. This new technique is the surgical reduction of bone mass in the pathway of desired tooth movement before orthodontic treatment. This case report explained the treatment of a 21-year-old male patient with severe-to-moderate crowded dentition and he requested to get the treatment done in a short period of time. Upper right first premolar and lower left second premolar were extracted, the bone in the desired pathway of all teeth movement was reduced and the dentoalveolar distraction procedure performed using nickel-titanium closing coil spring and rubber power chain. Full teeth alignment was achieved in 3 months period and the anchorage teeth were able to withstand the retraction forces with minimal anchorage loss.

Abstract Mesiodens is a supernumerary tooth occurring in the anterior maxilla, located between the maxillary central incisors. Extraction of a mesiodens at a time appropriate for promoting self-eruption in the early mixed dentition may result in better alignment of the teeth and may minimize the need for orthodontic treatment. This case report features management of erupted mesiodens which caused the permanent maxillary right central incisor to be displaced labially in a ten year old female who was treated successfully by timely intervention in her mixed dentition.
