Aim. To perform the comparative analysis of osteosynthesis stability in splintered fractures on the example of modeled fractures of femur with metalwork. Methods. Bone fragments fixation methods using internal fixators, blocking intramedullary nails, external fixation devices. Round wooden rod with external diameter of 35 mm, internal diameter of 25 mm, which was saw in the middle with two cuts directed at an angle of 45° to the rod axis to form the triangle fragment, was picked for femur model for the purpose of experiment. The stability of bone fragments fixation using plates with angle stability, pin- and nail-type external fixation devices and blocking intramedullary nails was assessed. To test the stability of the «bone-fixing device» complex, torsional test machine КМ-50-1 №66, power press П-10 №92, pull test machine Р-10 №1677 were used. Results. Compared to external fixation device, fixation using plates with angle stability was 6 times more stable at axial load and by 43.5% at lateral load. The stability of blocking intramedullary nail at axial load was higher compared to external fixation device. At rotary load, the stability of blocking intramedullary nail was 2 times lower compared to external fixation device. Conclusion. On a model of splintered double-lever femur fractures, fixation using plates with angle stability was the most stable fixation mode.

A special place in the methods of surgical treatment of bone fractures is occupied by transosseous osteosynthesis using external fixation devices of various designs.Objective: to analyze the lower jaw exoskeleton in finite element programs.Materials and research methods. During the research, 36 human lower jaws were involved from the museum of the Department of Oper-ative Surgery and Topographic Anatomy of the Volgograd State Medical University. They were scanned in 3D. All obtained images were loaded into a virtual scene reconstruction program. Fractures of the lower jaws in the corner area were modeled, an apparatus for external fixation of the lower jaw exoskeleton was installed, and the chewing load on the lower jaw was simulated. The place of application of the force was an area on a small fragment of a repositioned 3D model of the mandibular bone corresponding to the place of attachment of the masseter muscle. The evaluation of virtual studies was carried out according to the results of the stress-strain states of the bones and apparatus, the schedule of displacements of objects and the results of the analysis of the safety factor.Research results and discussion. In the course of evaluating the virtual placement of the mini-fixator wires of the apparatus for external fixation of the lower jaw exoskeleton, it was revealed that the main load is applied to the mini-fixator wires on a large fragment and the bone in the area of the wires. For the possibility of precise positioning of the osteofixers of the external fixation device, a device for the safe installation of the spokes of the lower jaw exoskeleton was also developed.Conclusion. Thus, on the basis of computer mathematical analysis, it can be argued that the developed design of the apparatus for external fixation of the lower jaw exoskeleton works under conditions of the maxillofacial region, performs reposition and fixation of bone fragments of the lower jaw under conditions of chewing functioning of the restored fracture of the lower jaw.

The experience in treatment of hallux valgus is presented. There were 90 patients, aged 19-77, 199 feet. Seven patients had first degree of deformity, 24 patients - second degree and 59 patients - third degree. Surgical treatment was performed in the period from 2004 to 2009. Depending on the deformity degree various types of osteotomy were applied followed by biomechanically stipulated fixation of bone fragments with small-sized rod and pin-rod external fixation devices. In all patients stable elimination of valgus deformity was achieved. Excellent long-term results were obtained in 67,5% of cases (112 feet), good results in 29,5% (45 feet) and satisfactory results in 3% (5 feet). Neither long-term failed results nor complications were observed. Advantages of surgical treatment of forefoot deformity with external fixation device are the restoration of foot anatomical shape and function in stable fixation of osteotomic fragments without limitations of active functional load on the operated foot.

Transosseous osteosynthesis continues to occupy a leading position among the methods of treatment of damage to the tubular bones of the extremities. This article presents a survey of 47 patients: 22 men and 25 women aged 25 to 60 years with fractures in the middle third of the radial shaft with displacement of bone fragments. The peculiarity of the external fixation device we used is that all transosseous elements are represented by 1.5 mm pins, inserted through both cortical layers of each bone, at levels from I and VIII. The pins are inserted simultaneously through both bones, thus excluding the rotational function of the forearm.The essence of the study is a comparative analysis of the blood system parameters in patients with diaphyseal injuries of the radial bone, operated by the transosseous osteosynthesis method using the external fixation device of pin configuration. Any damage to the body, (a fracture or related surgery), is accompanied with various changes in the system of hemodynamics and metabolism.The laboratory blood tests performed in the group of patients with damage to the radial shaft treated with transosseous osteosynthesis by external fixation device of pin configuration, revealed certain regularities in changing of blood parameters: increased number of platelets, increased level of fibrinogen and level of soluble fibrin complexes, which promotes the activation of the internal mechanism of blood clotting.

Tactics of surgical treatment of spondyloptosis including use of transpedicular external fixation device is presented. There were 4 patients with L5 spondyloptosis. The tactics is characterized by minor traumatization of lumbosacral spine with external fixation device, decrease of neurologic complications rate owing to gradual L5 reduction, possibility of spine deformity elimination, significant technical simplification of internal transpedicular osteosynthesis performance. Clinical results showed normalization of anatomic interrelation in lumbosacral spine, elimination of vertebroradicular conflict, restoration of trunk balance and anthropometric proportion as well as stabilization of L5-S1 segment by osteo-metalic block.
