<p class="Default">Total laryngectomy leads to drastic anatomical changes in the swallowing and breathing mechanism. Reduced tongue base retraction, poor pharyngeal clearance, stricture and prominence in the posterior pharyngeal wall have been reported to cause swallowing difficulties in individuals with total laryngectomy. The present case study describes the swallowing characteristics in an individual with total laryngectomy and discusses efficacy of swallowing maneuvers in total laryngectomy. A 55 yr/male, operated case of total laryngectomy using TEP reported with complaint of food getting stuck in the throat and vomiting sensation. He was fed orally and could tolerate only thick liquids. Swallowing assessment was carried out by clinical and instrumental tools (modified barium swallow) which revealed cricopharyngeal prominence, poor pharyngeal clearance and significant post swallow residue. Effortful swallow was attempted to explore the change in swallowing mechanism. Relaxation of pharyngeal wall with better pharyngeal clearance was seen with the maneuver. Clinically, the case reported of ability to swallow semisolids and solids well with the maneuver. At 3 month follow up, the case reported no difficulty in swallowing semisolids and solids orally. The above case study highlights on radiological evidence of swallowing abnormalities following total laryngectomy and further on the efficacy of maneuver on mechanism of swallowing. Hence, it can be noted that swallowing intervention has strong implications in patients with total laryngectomy. </p>

Pharyngitis is inflammation of the pharynx, which affects in back of the throat. It is most often referred to simply as sore throat; it is one of the most common reasons for doctor visit. The condition is the usual phenomenon in paediatric population due to intake of cold drinks, junk food etc. This disease is more recurrent now a days than earlier As per ayurvedic classics the condition has close similarity with the disease Galagraha. Aim: In this article management of pharyngitis with ayurvedic modalities were discussed. Material & Method: A 32 years male patient visited ENT OPD of Shalakya Tantra, I.P.G.T&R.A Hospital on 16/12/2019 with complaints of pain and foreign body sensation of throat, Recurrent moderate cough, mild fever and head ache, difficulty in swallowing along with congestion in posterior pharyngeal wall aggravate since 10 days. He consulted a local hospital near his house but didn’t get a satisfactory relief. History reveals the reccurents of disease since 3 years. We started Triphala + Darvi kwatha kawala thrice/Day, Haritaki Kwatha pana Twice BD with honey (20 ml Before food), Khadiradi vati (2 Tab -4 times/ day) for Chushanartha, Sitopaladi churna -3gm +Yastimadhu 1 gm, Naradiya Lakshmi vilasa rasa-125 mg-2 times a day for 14 days Followed by Virechana and Nasya. Marked relief was found in signs and symptoms within 7 days and cured completly within 21 days of treatment. Conclusion: Study concluded that the above Ayurvedic treatment can prove to be an effective alternate management in Chronic Catarrhal Pharyngitis with proper diet and regimen.

ABSTRACT Near total laryngectomy can be safely combined with partial or subtotal pharyngectomies when the disease is lateralized rendering the interarytenoid joint and the contralateral cricoarytenoid joint free of tumor. A case of post radiotherapy recurrent carcinoma of the posterior pharyngeal wall which could be salvaged by a circumferential pharyngectomy combined with a near total laryngectomy is reported with a discussion about its potential advantages. The clinical records of 225 patients undergoing primary or salvage near-total laryngectomy (NTL) for laryngeal and pyriform cancer were analyzed for local control and morbidity. If the primary cancer was laryngeal in origin, patients underwent a simple NTL; if it was pyriform, a minor modification called near-total laryngopharyngectomy (NTLP) was used. When NTLP was extended to include necessary portions of the tongue base or posterior pharyngeal wall, pharyngeal reconstructions were added. The principal outcomes studied were 1) 5-year local control of the primary cancer, 2) achievement of lung-powered shunt speech, and 3) incidence of aspiration. The local control of cancer was similar to that expected with total laryngectomy or laryngopharyngectomy. Conversational voice was achieved in 85% of patients surviving beyond 1 year. Some patients required additional surgery for voice — Usually endoscopic dilation. Aspiration was absent if primary healing was achieved. It was troublesome in wound breakdown if the shunt was directly affected. Secondary anti-aspiration procedures were required in 9% of our patients — usually preserving shunt speech.

We present a rare case of a schwannoma which is localized on the posterior pharyngeal wall. It caused severe difficulty in swallowing and breathing in a 3-year-old child. Schwannomas of the posterior pharyngeal wall have been rarely reported in children. To our knowledge, only 7 cases of posterior pharyngeal wall schwannoma have been previously reported in the literature, but none of them is associated with a child. The tumor was removed with plasma radiofrequency excision via a transoral route. Computed tomography or magnetic resonance imaging examination showed no recurrence of the tumor up to 2 years. To our knowledge, for the first time, a case of posterior pharyngeal wall schwannoma has been reported in a 3-year-old child, and the schwannoma was removed via plasma radiofrequency transoral excision. This case study illustrates that schwannoma may occur in children, and it is safe to treat this disease through plasma radiofrequency transoral excision route.

Twin studies have mostly focused on the pattern of maladaptive behaviour manifested by the twins and their biological basis but the findings have remained controversial till date. The present case study explores the psychopathology in 14 year old twins of Indian origin. They were referred for psychometric assessment and psychotherapy for their conduct problems. The tools administered on them during psychometric assessment are Wechsler Intelligence Scale for Children IV (WISC-IV), Rorschach Inkblot Test (RIBT) and Thematic Apperception Test (TAT). Findings are discussed in terms of the personality processes and relationship quality of the twins.

In the last decades, geophysical methods such as magnetic survey have become a common technique for prospecting archaeological sites. At sub-Saharan archaeological sites, however, magnetic survey and correlated techniques never came into broad use and there are no signs for an immediate change of this situation. This paper examines the magnetic survey undertaken on the Nigerian site of Zilum, a settlement of the Gajiganna Culture (ca 1800-400 BC) located in the Chad Basin and dated to ca 600-400 BC. By means of the present case study, we demonstrate the significance of this particular type of investigation in yielding complementary data for understanding the character of prehistoric settlements. In conclusion, we point out that geophysical methods should play a more important role in modern archaeological field research, as they furnish a class of documentation not achievable by traditional survey and excavation methods, thus creating new perspectives for interpreting the past of African societies.
