Astrans Putra Logistik Ltd. is a logistics service company with shipping routes in Sidoarjo, Denpasar, Lombok and NTB. Some of the problems that occur, such as delays in the pick-up process of goods to the load that large but not enough for departure, cause the quality of the supply chain system to be poor. So this research needs to be done with the aim of knowing the point of waste in the supply chain, identifying the sigma value, and determining the priority for its improvement. Quality improvement is carried out using the Lean Six Sigma method with AHP integration as a priority selection for improvement. From the research results, it was found that there was waste that caused 3 (three) CTQ, namely delivery was delayed, goods were damaged or leaked, and there was a difference in the number of goods. The company's DPMO value is 34272 with a sigma value of 3.34. The priority for improvements that can be made based on AHP weight, namely check the package packages before they are sent (0,353), make SOP for the preparation of goods (0,167), and apply SOP for goods checking documents (0,142).

Abstract The main problem of the salt supply chain system is the oligopoly market structure dominated by middlemen which reduces the bargaining power of farmers. It has, however, been discovered that vertical collaboration (farmers to cooperatives) and horizontal collaboration (farmers to farmers) models have the ability to increase farmers' revenue. Therefore, this research was conducted to determine the effect of both horizontal and vertical collaboration models on the salt supply chain system with the expectation to increase farmers' revenue. This involved the application of the cooperative game theory with Shapley's value used as the basis for decision making. The result showed that the collaboration of stakeholders in the salt supply chain system has the ability to increase farmers' revenue, especially when they sell a maximum of 20% of their products through cooperatives and the rest through middlemen. This means the existence of farmers as cooperative members has a positive impact as observed in the revenue increment. Therefore, it is recommended that cooperatives improve their functions and roles as providers of savings and loans and market seekers, determine the appropriate prices for salt, and improve the quality of the products produced by their members. Moreover, a collaboration between farmers is mutually beneficial and this means efforts should be made to ensure cooperation, especially for small farmers.

PurposeThis paper aims to depict the practical execution of the problem-solving structure provided by the define, measure, analyze, improve and control (DMAIC) framework in combination with the analytical power provided by process mining capabilities, to improve the supply chain quality of a health-care provider. Design/methodology/approachPrior to the study, a literature review was conducted to identify existing frameworks combining six sigma with process mining. The authors use a descriptive case study approach to explain how the two methodologies blend across the different phases of DMAIC in a health-care setting. FindingsThis case study describes how analyzing data extracted from core information systems has significant value to improvement initiatives when complemented by traditional quality methods. By intersecting process mining techniques with lean six sigma tools, the researchers found 65% of orders not complying with the target ordering time and 200 redundant purchases with high operational costs. Research limitations/implicationsBy depicting how the two methodologies can be intertwined, this paper complements existing research by presenting it as a viable quality improvement approach. Practical implicationsThis paper provides insights for six sigma and process mining practitioners on the benefits of combining both methodologies within the DMAIC structure. Implementing this blended approach can bring visibility to operations and accelerate process improvement initiatives. Originality/valueThe prime value of this paper lies in the integration of traditional six sigma methods with process mining as a technological approach in a health-care context, going beyond existing research, which, to the best of the knowledge, lacks descriptive case studies.

Taking the four-level supply chain participated by two-echelon logistics service providers as the research object, this paper deeply studies the influence of different power structures on the optimal price and logistics service level decisions of the whole supply chain system. On this basis, this paper points out the optimal power structure of the whole supply chain system and then discusses the optimal cooperative decision-making mode to improve the operation efficiency of the supply chain system under optimal power structure. The results show that compared with the two power structures of manufacturer-led and logistics service integrator-led, the retailer-led structure is the optimal power structure, whether from the perspective of maximizing the total profit of the supply chain or the surplus of consumers. When the supply chain is facing consumers who are less sensitive to the retail price of products and more sensitive to the logistics service level, the advantages of retail-led decentralized supply chain are more significant. The centralized decision of product supply chain (PSC) and logistics service supply chain (LSSC) under different power structures is the dominant strategy, and the equilibrium strategy is Nash game adopted by both parties. At this time, the total profit of the supply chain system is the largest and the total social welfare is the largest, while the total profit and consumer surplus of the supply chain under LSSC-led structure are the smallest. When the supply chain is facing consumers with low sensitivity of logistics service level and product retail price, PSC and LSSC have more significant advantages in Nash game rather than Stackelberg game.

Excessive waste is one of the problems in the food supply chain system. This study aims to identify supply chain players, map, and reduce waste that occurs in the Papaya California agribusiness supply chain process in Cikarang, Bekasi. Data collection was obtained from the results of field studies and interviews with parties involved in the supply chain such as farmers, distributors, retailers, and consumers. Increasing the effectiveness of Papaya's supply chain network is by improving the quality of packaging (modified atmosphere packaging (MAP), smart packaging, active packaging, and intelligent packag­ing), sharing data among chain players supply, implement refri­ge­ration and monitor transportation technology and promote the long term.  Good coordination between parties involved in the Papaya supply chain has a significant role in creating an effective and effici­ent supply chain system.

The aim of this paper is to evaluate the importance of lean and six sigma concepts and techniques on the basis of systematic knowledge gathered from relevant scientific sources, both for discovering supply chain processes where potential waste can occur and for improving the quality of those processes. The paper will first outline the key features and effects of applying lean and six sigma concepts in manufacturing and supply chains. The focus is on the frameworks and potentials of implementing the lean concept in the Republic of Serbia. Then, the possibilities of integrating these concepts into the lean six sigma concept are presented and its role in eliminating defects in business processes. The penultimate part of the paper outlines the key features of lean supply chain while the final part contains the mathematical frameworks for designing six sigma supply chains.

ABSTRACT. The study aims to determine  the priority  of quality improvement of hospital  service based  on perception,  expectations  and importance  of customers  by using lean six sigma approach. The results of the study are  expected to contribute  to hospital  management  in providing  the best service  for the community. This research  determines  the priority  of service  quality  improvement of hospital  based  on service  quality dimension consisting of (1) tangible,  (2) reliability,  (3) reponsiveness,  (4) assurance and (5) emphaty. This study is a qualitative  descriptive research  intended to analyze the responses of respondents  on the quality of hospital services based on perception,  expectations  and importance  will be used to determine the priority of improving the quality of hospital services. Research  respondents  were 240 service users in four hospitals in Lumajang  district.  Data  analysis  technique  using  lean  six sigma  approach by first  testing  the  research instrument  with validity and  reliability  test. The results  of the study there  are  differences  in the order  of priority of service quality improvement at four hospitals. Priority  improvement in dr Haryoto hospital is the ability and dexterity in dealing with patient complaints, at the Bhayangkara hospital is the timeliness of open counter service, at Wijaya Kusuma hospital is completeness of examination equipment, and Islam hospital is the ability of the officer to explain thhe results of the examination.

Purpose: Four incident reports involving missed doses of myeloid growth factors (MGFs) triggered the need for an outcome-driven initiative. From March 1, 2015, to February 29, 2016, at University of California Irvine Health Chao Infusion Center, 116 of 3,300 MGF doses were missed (3.52%), including pegfilgrastim, filgrastim, and sargramostim. We hypothesized that with the application of Lean Six Sigma methodology, we would achieve our primary objective of reducing the number of missed MGF doses to < 0.5%. Methods: This quality improvement initiative was conducted at Chao Infusion Center as part of a Lean Six Sigma Green Belt Certification Program. Therefore, Lean Six Sigma principles and tools were used throughout each phase of the project. Retrospective and prospective medical record reviews and data analyses were performed to evaluate the extent of the identified problem and impact of the process changes. Improvements included systems applications, practice changes, process modifications, and safety-net procedures. Results: Preintervention, 24 missed doses (20.7%) required patient supportive care measures, resulting in increased hospital costs and decreased quality of care. Postintervention, from June 8, 2016, to August 7, 2016, zero of 489 MGF doses were missed after 2 months of intervention ( P < .001). Chao Infusion Center reduced missed doses from 3.52% to 0%, reaching the goal of < 0.5%. Conclusion: The establishment of simplified and standardized processes with safety checks for error prevention increased quality of care. Lean Six Sigma methodology can be applied by other institutions to produce positive outcomes and implement similar practice changes.

In a closed-loop supply chain consisting of a manufacturer and a retailer, this paper studies the pricing strategies and coordination mechanism of supply chain when the remanufacturing cost is random caused by the proportion of reusability parts in design stage and quality condition of recycling product. The results show that the wholesale price and retail price are negative correlation, while the recycling rate and total profit of supply chain system are positive correlation with the proportion of reusability component designed in new product and quality of recycling product. Moreover, there are conclusions that the wholesale price and retail price are lower while the recycling rate and total profit of supply chain system are higher with centralized decision. Then, in order to coordinate the closed-loop supply chain, this paper develops a revenue-sharing contract, in which the revenue share parameter is determined based on absolute deviation approach. The theoretical results are illustrated by a numerical example.
