AbstractAim: The main aim is to show that the life of a patient depends on the decisions the doctor makes as well as the proper assessment of the case. The decision to avoid a surgical procedure and continuewith the conservative treatment following the vital parameters, was beneficial for the patient who was later discharged recovered.Case report: The case is about a patient who was in labor for the fifth time with the delivery complicated by severe postpartum hemorrhage in the secondary health care institution. Despite all conservative measures taken, the hemorrhage hasn’t stopped, therefore, the subtotal hysterectomy was performed, after which the patient was directed to the Clinical Centre Kragujevac. Regarding the fact that the postpartum hemorrhage hasn’t stopped and abdominal hematoma as well as intracranial hemorrhagewere diagnosed, the main dilemma was if the surgical procedure should be redone or if the conservative treatment should be continued. By applying the conservative treatment andcontinued consultations of the multidisciplinary team, the patient was discharged from the Clinical Centre Kragujevac.Conclusion: The main issue with severe cases like this one, is to define and direct the treatment towards the lower risk rate – repeated surgery could be fatal with the current state of the patient. The estimation was correct, at the end, the patient was released after the-lengthy treatment, recovered.

Congenital absence of gall bladder is a very rare entity found in clinical practices. Due to miss interpretation of  sonographic findings as they are not familiar with the condition it possess a great difficulty in management the  patient. Though the patient present with the feature of acute  cholecystitis, conservative treatment is enough to cure the disease. Modern radiological intervention make the diagnosis confirm, thus preventing unnecessary surgical procedure. Here we report two cases of Congenital absence of gall bladder presenting as acute cholecystitis ultimately  diagnosed and treated. Abstract Objective: In intractable postpartum hemorrhage(IPPH), uterotonic lost efficacy in myometrum contraction formation. While the key target in uterotonic, OTR(oxytocin receptor) and internal environment in myometrum might have some changes when IPPH. Methods: Three groups of myometrial tissue at different contraction states were collected:(1) term not at labour (TNL) group(10 cases); (2) term at labour (TAL) group(10 cases);(3) intractable postpartum hemorrhage(IPPH) group(10 cases).Expressions of OTR and internal environment markers(ASIC1a ,HIF1α, GlUT, FATP and apoptosis ) were compared in 3 groups by Immunoreactivity Scores method. Results: The expression of OTR in myometrium in IPPH group was 100% negative ,compared to 30%,20% in TNL and TAL group(p < 0.05).70% positive HIF-1α expression in TNL group increased to 80% in TAL group,but decreased to 20% in IPPH group(P<0.05). While the positive expressions of ASIC1a, FATP and GLUT in three groups all had no significant difference.But the apoptosis related markers Bax,caspase-3 and caspase-9 positive expressions in IPPH were significantly all decreased(10%,10%,0%) compared to TNL and TAL group,compared to a high positive expression of Bcl-2 in 3 groups.Conclusion: OTR expression and HIF-1α self-protection might be lost in myometrium of IPPH, which could lead to uterotonic lost efficacy and conservative treatment failure.

Objective: To evaluate the use of Pereira suture as a conservative surgical management in postpartum hemorrhage due to uterine atony. Methods: The study was performed in the Department of Obstetrics and Gynecology, Temanggung General Hospital, Central Java, Indonesia. Data was retrieved retrospectively from medical records, from January 2011 until December 2013. Results: Pereira suture was done in four cases of uterine atony with failed conservative management, and the procedure was found to be successful in all cases. Conclusion: Pereira suture is an alternative surgical procedure for the treatment of uterine atony after failed conservative management. This chapter seeks to explore from the reflexivity approach the current state of virtual and augmented technologies in various surgical procedures, analyzing the most relevant technological progress as well as the latest practical applications that have been developed. Bibliographic, documentary, and descriptive were the methodologies of research; the information was collected from various scientific articles from indexed journals and websites, using keywords such as “virtual reality,” “augmented reality,” and “surgical procedure” in search engines. Furthermore, technological progress in various branches that include surgery is explored in this chapter, which is focused on the research and technology application of virtual and augmented reality as well as the challenges.

Here, we present a case report of a woman who presented with a large sacral rheumatoid nodule. This patient failed conservative treatment and presented in search of a surgical solution. We successfully removed her rheumatoid nodule using a surgical approach typically reserved for traumatic coccydynia. We show how coccygectomy, although a rare surgical procedure, was effective in treatment of a large rheumatoid nodule.
