Acute appendicitis is the most common surgical emergency. A bowel obstruction due to the appendicitis is in most cases functional with a paralytic ileus mechanical bowel obstructions are rare or exceptional. We describe a rare case of a mechanical bowel obstruction due to a strangulation of the last ileal loop by the appendix.

Acute appendicitis is a common surgical emergency in older adults. In the elderly, like in younger cohorts, acute appendicitis most commonly arises without neoplastic underpinnings. However, the occurrence of acute appendicitis in a patient with a concurrent abdominopelvic malignancy should trigger suspicion for the possibility of a metastatic appendiceal neoplasm. We present the case of a 66-year-old man with a background of a biochemically recurrent prostatic adenocarcinoma who presented to the emergency department with acute appendicitis. Histopathologic examination of the resected appendix revealed an unexpected metastatic spread from his prostatic adenocarcinoma.

Appendicitis is a surgical emergency that can mimic many other abnormalities. Early diagnosis is critical to reduce the risk of perforation and potentially life-threatening complications. The use of sonography has proven to aid in accurately diagnosing an inflamed appendix without adversely affecting the rate of perforation. However, the accuracy of sonography is highly operator dependent. This case demonstrates the classic clinical presentation of appendicitis with the incorporation of sonographic findings to support the diagnosis.
