Mature cystic teratoma (Dermoid) of ovary is a benign germ cell tumor consisting predominantly of ectodermal derivatives. In rare occasion sebaceous material may get aggregated to form marble like appearance because of continuous mobility of Dermoid and its content. Such a case in a menopausal woman has been presented in this report.

Background: Ovarian cancers are common among females. The ovarian tumors are classified according to its histogenesis. In the present study, we analyzed the germ cell ovarian tumors reported in our institute. Materials and Methods: This study was a hospital based retrospective study. All ovarian germ cell tumor reported from Jan 2001 to Feb 2013 were retrieved and analyzed. The specimens were routinely processed and the slides were stained with H&E and special stains wherever required. Results: A total of 340 cases of ovarian tumors were reported and 150 cases (44%) were of germ cell origin. We had 137 cases of benign and 13 cases of malignant germ cell tumors. Most common subtype was mature cystic teratoma comprising 125 cases. Among the malignant cases, the most common was immature teratoma (5 cases). Age range in our series was from 8 to 82 years (mean 28 years). The most common age group affected was 21-30 years. Most of the malignant cases (76.9%) were in less than 30 years. Among the malignant cases majority (84.6%) were more than 10 cm in size. None of the malignant germ cell tumors were bilateral. Conclusion: Among germ cell tumors of ovary, mature cystic teratoma is the most common tumor and malignant subtypes are uncommon. Malignant tumors tend to occur in younger age group. OBJECTIVE: Mature cystic teratoma, also known as a dermoid cyst, is the most common germ cell tumor of the ovary. In this retrospective study, it is aimed to evaluate clinicopathological findings of the patients who were operated on due to the preliminary diagnosis of dermoid cyst. STUDY DESIGN: Between May 2013 and May 2018, the findings of a total of 143 patients who were operated on with a preliminary diagnosis of dermoid cyst in our institution were analyzed retrospectively. In addition to demographic characteristics such as age, parity number and tumor size, lateralization, preoperative tumor markers, surgical procedure, presence of other pathological findings, and the rate of malignant transformation were analyzed. RESULTS: The median age of patients was 35.4 years and the ratio of nulliparous, primiparous, and multiparous patients was 46.9%, 25.2%, and 28%, respectively. The median tumor diameter was 6.7 cm, 50.3% of which were right-sided, 46.9% were left-sided and the remaining 2.8% were bilateral. Intraoperative torsion was found in 15 (10.5%) cases. The treatment consisted of 60.1% cystectomy by laparoscopy, 3.5% unilateral salpingo-oophorectomy by laparoscopy, 9.8% unilateral salpingo-oophorectomy by laparotomy, while the rate of 16.1% cases underwent hysterectomy with bilateral salpingo-oophorectomy by laparotomy, 10.5% of the cases, cysts were excised during cesarean section. As the final pathology report revealed, of 143 patients, immature teratoma was observed in five cases (3.5%), in one of them (0.7%) mixed germ cell tumor and in three of them (2.1%) borderline mucinous cancer, squamous cell carcinoma, and carcinoid tumor on mature cystic teratoma base were observed. CONCLUSION: Torsion is the most common complication with mature cystic teratoma. The size of the tumor is an important prognostic factor in terms of the malignant transformation of teratoma. Fertility sparing surgery with a minimally invasive surgical approach is available for fertility patients diagnosed with Mature cystic teratoma.

Background Intracranial teratomas are rare tumors that usually present during the second decade of life. Among intracranial teratomas, suprasellar germ cell tumors in particular are extremely rare. We present the case of an 18-year-old girl who underwent a successful endoscopic transnasal resection of a suprasellar teratoma with a mixed germ cell component. Case Description An 18-year-old girl presented with progressive visual disturbance, headache, weight loss, and fatigue for 2 weeks. She had decreased vision in her right eye and double vision when looking to the right side. On examination, she had right-side complete abducens nerve palsy and partial oculomotor nerve palsy bilaterally. Brain imaging revealed a large suprasellar lesion consistent with a germ cell tumor. The patient underwent a transnasal endoscopic resection of the lesion. Pathology showed a mixed germ cell tumor (80% mature cystic teratoma, 18% yolk sac tumor, and 2% germinoma). The mature teratomatous elements identified included skin with sebaceous material, hair, cartilage, acini, colonic mucosa, muscle, nerve bundles, and adipose tissue. Postoperative magnetic resonance imaging (MRI) showed residual tumor in the left cavernous sinus and anterior clinoid process superiorly. At a 1-year follow-up, the double vision had resolved, and her vision showed progressive improvement. The patient was maintained on treatment for diabetes insipidus, hypothyroidism, and adrenal insufficiency. Her last MRI showed no signs of tumor progression. Conclusions The present case demonstrates a safe endoscopic transnasal surgical resection of a rare suprasellar tumor, mixed teratoma, with intra-axial extension. The endoscopic transnasal approach can be considered a valid option for a suprasellar lesion.

ABSTRACT Mature cystic teratomas are a form of ovarian germ cell tumor and often called ‘dermoid cysts’ because they are mostly cystic. Many teratomas have teeth with morphologically identifiable crowns and roots. The objective of this study was to carry out morphometric measurements of teratomatous teeth extracted from a mature cystic teratoma excised from a 28-year-old female patient. The extracted teeth were stored in normal saline till they were subjected to morphometric analysis. All measurements were done using digital calipers (Derby, France), repeated twice and the average was recorded. The teeth were labeled as teratomatous tooth 1 (TT1), teratomatous tooth 2 (TT2) and teratomatous tooth 3 (TT3). The gross morphology of TT1 resembled a maxillary deciduous canine whereas TT2 and TT3 resembled a deciduous incisor and a mesioden respectively. The teratomatous broadly resembled normal intraoral teeth. With all of them primarily resembling maxillary deciduous teeth it would not be out of place to postulate that the displaced embryonic tissue leading to the formation of these was from the upper front region of the stomodeum.
