Annotation. The aim of the study is to improve the treatment outcomes of patients with duodenal fistulas. The object of retro- and prospective study were 14 patients with external unformed high-flow duodenal fistulas, who were treated in the Shalimov National Institute of Surgery and Transplantology during 2016–2019. All patients were performed a complex examination, which included general clinical, laboratory, instrumental and statistical research methods. We have developed and patented a minimally invasive method of treatment of duodenal fistulas, which involves endoscopic placement of a nitinol-coated self-healing stent in the area of the defect and VAC therapy from the outer opening of the fistula. Statistical processing of research results was performed using spreadsheets Microsoft® Office Excel (2017) and statistical processing program Statgraphics Professional 16.0.03. Student's test and Wilcoxon-Mann-Whitney test were used. The use of the developed method of treatment allowed reducing the number of postoperative complications by 32%, to reduce the duration of treatment (from 41,16±5,14 to 27,2±4,7 days, p>0.05). The overall mortality in the experimental group decreased by 43% compared with the mortality of the comparison group. Our proposed minimally invasive method of treatment of duodenal fistulas helps to improve the results of treatment of patients, namely to reduce the duration of treatment and reduce the number of complications and mortality rate.

The immediate and long-term results of treatment of 30 patients with severe pain syndrome of the lumbar region, who underwent operations on denervation of DOS, were studied. The aim of the study was to evaluate the effectiveness of minimally invasive technologies for the treatment of pain in the lumbar region (denervation of DOS), to study the near and distant results of these treatment methods. Denervation DOS is an effective minimally invasive method for the treatment of facet syndrome caused by spondylarthrosis. It allows in the early and distant postoperative periods to significantly reduce the pain syndrome and improve the quality of life of patients.

The article presents an analysis of the results of treatment of a patient with purulent-necrotic complications of diabetic foot syndrome, who, in the framework of surgical treatment, used a minimally invasive method of treatment of purulent-necrotic complications of diabetic foot syndrome, developed at the departments of general surgery and normal anatomy of the S.M. Kirov Military Medical Academy. The effectiveness of the developed method has been demonstrated, which makes it possible to sanitize the purulent cavity in a short time, stop pain syndrome and restore the support ability of the foot (4 figs, 1 table, bibliography: 7 refs).

The authors analyze the results of treatment of 60 women with various diseases — genitourinary menopausal syndrome, stress and mixed type of mild urinary incontinence, vaginal relaxation syndrome. According to the results of this study, the use of therapeutic radiofrequency exposure is an effective minimally invasive method in the treatment of patients with urogynecological diseases.

The aim: To improve the results of treatment of patients with polyps of the external urethral orifice by using minimally invasive surgery. Materials and methods: The materials of the work are based on clinical examination and treatment of 22 patients with polyps of the external urethral orifice in the treatment of which, along with classical treatment were used minimally invasive methods of removal of polyps of the external urethral orifice using high-intensity laser. Results: We managed to reduce the duration of surgery and treatment twice less, to avoid typical complications, which accelerated the regeneration process and the rehabilitation period. Conclusions: The use of minimally invasive surgical methods to remove urethral polyps can reduce the duration of treatment by reduction of the thermal and mechanical load on the surrounding tissues and reducing the time of surgery.

INTRODUCTION: Parathyroid neoplasms are tumors seen in about 0.1% of the population.Although parathyroid adenomas are the most common,carcinomas are also rarely seen.Our aim in this study is to evaluate the clinical, laboratory and histopathological characteristics of patients who underwent parathyroidectomy in our hospital in the last 5 years. METHODS: 156 patients who underwent parathyroidectomy in our hospital and examined in the pathology department were included in the study.The types of operations performed in cases were grouped as those who underwent only parathyroid excision and those who underwent thyroid partial/total excision with parathyroid excision.Age,gender, pre- and post-operative parathyroid hormone values, size,weight,pathological diagnosis of the parathyroid gland, presence of complications,comorbidities and clinical follow-up were evaluated. RESULTS: 84% of 156 cases were female,16% were male.The mean age is 53.82+13.49.The mean preoperative parathyroid hormone levels of the cases are 415.18+491.15 mg/dl, and the mean post-operative parathyroid hormone levels are 64.02+110.72 mg/dl.The mean volume of parathyroid tissues was calculated as 5.13+8.85 and the mean weight as 2.78+2.88 g. Pathological diagnoses are 83.3% adenoma,10.9% hyperplasia,2.6% atypical adenoma, 1.9% carcinoma, 0.6% normal parathyroid and 0.6% lymph node.It was observed that as the postoperative PTH level increased,the weight of the parathyroid gland increased and the lesion weight was higher in males.The addition of thyroidectomy to the treatment, conventional parathyroidectomy instead of the minimally invasive method,and the high size and volume of the parathyroid gland were found to be associated with complicated complications in the postoperative period. DISCUSSION AND CONCLUSION: In terms of thyroid pathologies that may accompany,it will be more appropriate to evaluate preoperative cases in detail and to continue the treatment with minimally invasive methods for the comfort of the patient in postoperative processes.It is highly important to always take a multidisciplinary management in the approach to parathyroid masses and to evaluate the clinical, laboratory, intraoperative and pathological findings of the case together.

A new minimally invasive technique is presented to improve the results and reduce the duration of treatment of patients with stenosing ligamentitis of dorsal carpal ligament (de Quervain’s disease). The high efficiency of the proposed method (on the Disabilities of the Arm, Shoulder, and Hand scales and Visual Analogue Scale) is established, comparable with de Quervain’s open ligamentotomy but with the reduction of rehabilitation period after the surgery from 16-21 days to3-4 days. Clinical approbation of minimally invasive ligamentotomy in all patients revealed excellent long-term results of treatment, with minimal risk of complications. Comparison of the results of treatment of patients of both groups showed the advantage of minimally invasive ligamentotomy before open ligamentotomy in the early postoperative period with the same long-term results, which allows patients to start their labor and professional duties in the early stages. The proposed method can be performed in an outpatient mode and does not require special post-operative treatment, as well as special instruments and equipment, which allows reducing the costs of both the patient and the medical institution.

The OBJECTIVE of the study was to improve the results of treatment of patients with fluid collections in acute pancreatitis with a minimally invasive method. MATERIAL AND METHODS. Results of percutaneous-puncture interventions under ultrasound in 62 patients with acute pancreatitis in the presence of fluid clusters were analyzed. In the first day from the moment of the disease, 39 % of patients admitted during the second and the third days – 31 % and more than three days – 20 % of patients. RESULTS. Diagnostic punctures of fluid collections under ultrasound control were performed. In 49 (79 %) of 62 patients, the puncture was effectively transformed into percutaneous drainage. In 13 of 49 patients were further operated. Lethal outcomes after puncture-draining interventions were in 5 cases. Among 44 patients operated with a minimally invasive method of acute pancreatitis, recovery was noted in 24, recurrence of fluid collections – in 7, 13 patients were operated on. CONCLUSION. Thus, early diagnosis and timely implementation of conservative and minimally invasive methods of treatment contributed to improving the immediate results of treatment of patients with fluid collections in acute pancreatitis.

Abstract Background Spondylodiscitis is an unusual infectious disease, which usually originates as a pathogenic infection of intervertebral discs and then spreads to neighboring vertebral bodies. The objective of this study is to evaluate percutaneous debridement and drainage using intraoperative CT-Guide in multilevel spondylodiscitis. Methods From January 2002 to May 2017, 23 patients with multilevel spondylodiscitis were treated with minimally invasive debridement and drainage procedures in our department. The clinical manifestations, evolution, and minimally invasive debridement and drainage treatment of this refractory vertebral infection were investigated. Results Of the enrolled patients, the operation time ranged from 30 minutes to 124 minutes every level with an average of 48 minutes. Intraoperative hemorrhage was minimal. The postoperative follow-up period ranged from 12 months to 6.5 years with an average of 3.7 years. There was no reactivation of infection in the treated vertebral segment during follow-up, but two patients with fungal spinal infection continued to progress by affecting adjacent segments prior to final resolution. According to the classification system of Macnab, one patient had a good outcome at the final follow-up, and the rest were excellent. Conclusions Minimally invasive percutaneous debridement and irrigation using intraoperative CT-Guide is an effective minimally invasive method for the treatment of multilevel spondylodiscitis.
