Enterocutaneous and enteroatmospheric fistulas are the most challenging complications of abdominal sepsis to deal with a high mortality. This is even more challenging with a stoma in an open abdomen. In the setting of chronic granulomatous inflammation like abdominal tuberculosis or inflammatory bowel disease, recovery is prolonged and management involves multidisciplinary care. We present a rare case that was a complication of tuberculous peritonitis.

Tubulo-interstitial nephritis (TIN) in patients affected by inflammatory bowel disease, both ulcerative colitis and Crohn’s disease, is usually considered as drug-associated to aminosalicylate. We report a rare case of granulomatous active tubulo-interstitial nephritis in a young patient with a recent diagnosis of ulcerative colitis naïve to aminosalicylate treatment. The patient has been successfully treated with steroids administration. Our purpose is to sensitize that TIN should always to be considered in differential diagnosis an extra-intestinal manifestation of bowel disease.

Persistent intra-abdominal sepsis is a challenging complication of abdominal typhoid with a high mortality and difficult to manage. Delayed perforations following initial operative intervention is one of the rarer complications of this disease, causing wound breakdown and open abdomen, resulting in fistula. We present the management of a rare case, which presented with multiple perforations and multiple organ system dysfunction then complicated post-operatively with multiple fistula and open abdomen.

Granulomatous vulvitis/cheilitis may occur rarely as an extraintestinal manifestation of Crohn’s disease (CD) and may precede the development of gastrointestinal symptoms. Guillain-Barre syndrome (GBS) is associated with a wide variety of illnesses including inflammatory bowel disease. Though the immunologic abnormalities in inflammatory bowel disease may encompass both granulomatous inflammation as well as autoimmune components, the combination of CD, GBS and granulomatous vulvitis/cheilitis in the same patient has not been described in literature. We hereby reported a 14 year old girl with granulomatous vulvitis/cheilitis for 4 years preceding the development of gastrointestinal manifestations of CD, who also had GBS during the course of hospitalization.
