Efficacy of A Novel File in Removing Root Canal Filling Material from Root Canals of Endodontically Treated Teeth - An In Vitro Study

O tratamento endodôntico permite a permanência de dentes na cavidade oral. No entanto, procedimentos inadequados podem ocasionar o escurecimento dentário, acometendo a estética do sorriso e levando à baixa autoestima do paciente. O objetivo do presente estudo é relatar um caso clínico de retratamento endodôntico não cirúrgico, seguido de clareamento endógeno em dentes anteriores escurecidos para reabilitar os elementos tanto funcional quanto esteticamente. Paciente do gênero feminino, 25 anos, compareceu à clínica de Odontologia de uma Universidade relatando insatisfação com seu sorriso. A queixa principal se fundamentava na baixa autoestima em decorrência do escurecimento dos incisivos central e lateral superiores esquerdos após tratamento endodôntico realizado oito meses antes. Ambos os dentes estavam assintomáticos, mas clinicamente exibiam exposição de guta-percha sem selamento coronário. Radiograficamente o elemento 21 apresentava sobre-extensão de material obturador. A conduta adotada foi a reintervenção endodôntica dos elementos 21 e 22 com sepultamento do fragmento de guta-percha extrarradicular e posterior clareamento dentário interno, além de restaurações em ambos os elementos. O retratamento endodôntico não cirúrgico e o clareamento endógeno foram eficazes para a resolução do caso, pois possibilitaram a manutenção dos elementos dentários em função na cavidade oral e o restabelecimento da estética do sorriso, garantindo satisfação da paciente.Descritores: Endodontia; Retratamento; Clareamento Dental.ReferênciasTorabinejad M, White SN. Background: To investigate the efficiency of Hedstrom (H; Dentsply Maillefer) file, Protaper Universal Retreatment system (PTUR; Dentsply Maillefer), Mani NRT-GPR files (Mani, Japan) and WaveOne (WO; Dentsply Maillefer) Reciprocating files in removing Gutta Percha (GP) and sealer from root canals. Methods: Sixty canals with less than 10° curvature from extracted human maxillary incisors were used. Root canal reinstrumentation was performed with H, PTUR, Mani NRT-GPR and WO files. Amount of residual root filling material (Gutta-percha and or sealer) were measured by using stereomicroscope. Data of GP or sealer remnants were statistically analyzed using one-way analysis of variance and Tukey test. Results: There were no significant differences among 4 groups in terms of residual root filling material in the apical third (p > 0.05). H-file group showed highest amount of root residual root filling as compared to groups. Conclusions: Reciprocating file system proved to be more efficient in removing the root canal fillings material than other retreatment file systems. Objectives: The aim of the present study was to assess the effect of calcium hydroxide as a root canal dressing material on dentin microtensile fracture strength in human primary teeth in vitro. Study design: Thirty primary anterior teeth with root canals packed with calcium hydroxide were divided into groups of ten and immersed in saline at room temperature for 7, 30 and 90 days. Ten teeth with root canals filled with sterile saline were the control group. Microtensile fracture strength was measured in Mechanical tester Lloyd testing machine. Results: There was a significant difference (P < 0.05) between the fracture strength of the calcium hydroxide-filled teeth after 90 days (19.1 MPa) compared with the control (35.8 MPa). Dentin microtensile fracture strength of the calcium hydroxide-filled teeth decreased at an average of 0.142 MPa per day. Conclusion: Calcium hydroxide placed in root canals for an extended time had a significantly negative effect on root strength. Long-term success of root canal treatment in primary anterior teeth is estimated as 65% with most of the failures result from trauma recurrence. Clinical Relevance: Our results stress the need to evaluate the pros and cons of root canal treatment compared to extractions of non-vital primary incisors.
